[Jejunal cystic lymphangioma in adult with rapid growth].

Abdominal cystic lymphangiomas are a rare pathology that are related to abnormal development of the lymphatic system. They predominate in a children and are very uncommon in adults. Here the authors report a case of a cystic intra-abdominal lymphangioma in a 64-years old woman in which the cyst was situated in the mesentery of the jejuneum, with rapid growth. Ultrasonography, computed tomography and magnetic resonance imaging led us to evaluate a cystic characterizations and his site. Surgical excision remain the treatment of choice because we can establish an histologic diagnosis. Complete surgical excision of cyst, although it can be difficult, consent a definitive healing.

A painless treatment for patients undergoing Milligan-Morgan haemorrhoidectomy.

BACKGROUND: Postoperative pain associated with Milligan Morgan haemorrhoidectomy (MMH) remains problematic. Most patients complaint a severe pain on defecation and in the 1st postoperative week because of secondary infection and sphincter spasm. We studied the effect of metronidazole, lactulose and glyceryl-trinitrate on pain after MMH. METHOD: Thirty patients were randomly assigned to two groups. The 1st received a treatment with oral laxative (lactulose 66.7%) metronidazole and a topical glyceryl-trinitrate ointment at 0.2% both pre and postoperative; the 2nd received a placebo at the same dosage. The amount of pain in the 1st week and at the time of two defecation, postoperative analgesic requirement and time to return to normal activities were documented. RESULTS: A significant differences in the score pain on 2nd and 3rd day and on the 1st two defecation were observed among group. Analgesic consumption was highest in the placebo group. CONCLUSION: The treatment with lactulose, metronidazole and topic glyceryl-trinitrate in patients undergoing MMH, seems to reduce secondary pain and increase patients satisfaction and earlier return to work with improvement of cost and benefit.

[Endoscopic study in reflux esophagitis. Our experience].

One hundred twenty one cases of oesophagitis by gastroesophageal reflux were observed through a retrospective study of 1250 endoscopies of the upper digestive tract. The oesophagitis was staged according to Savary-Miller classification. The prevalence (93.3%) of cases were in I and II stage. The Authors found that the most frequent associated diseases with oesophagitis by gastroesophageal reflux were: hiatal hernia, gastric and duodenal peptic disorders, gastric resection according Billroth II and biliary disorders. In 86.7% of cases, endoscopic diagnosis was confirmed by histological findings. From these data and from those reported in literature the authors can ascertain that endoscopy, associated with histological findings has an important role in diagnosing oesophagitis and in monitoring medical and surgical treatment used.

[Familial polyposis].

The F.A.P. is an hereditary disease autosomic dominant, characterized by the growth, from a minimum of 100 to several thousands, of adenoma of the large bowel that degenerate in carcinoma if not intervene surgically. The gene of the F.A.P. has been founded in the chromosome 5, region 5q 21-q22. The incidence is of one case above 6,850-30,000 inhabitants. The F.A.P. characterize it self besides the adenoma, of the large bowel also for the extracolic diseases (adenoma of the small bowel and of the stomach, epidermoid cysts, osteomas, dermoid tumors, congenital hypertrophy of the pigmentary epithelium of the retina). The Authors report their experience concerning a patient with F.A.P.: a man of 40 years old. The patient's genealogic tree was so composed: a brother, a sister and 2 children (a boy of 9 years old and a girl of 6 years old). They have been exposed to the following screening examinations: hemoccult test, rectal exploration, coloscopy and oculist visit). These investigations have given negative results. Because of the youth of the children it has been advised a yearly check-up. At last the Authors underline that each time a patient with F.A.P. is discovered (proband) it is necessary subject the 1 degree grade familiar to the screening above mentioned; because the life expectation in the secondary case (calls-up) discovered.

[The importance of pre-operative ultrasonography in assessing the technical difficulty and potential complications of laparoscopic cholecystectomy].

In this work thirty patients with symphtomatic not complicated gallbladder disease were studied by ultrasound (US) and by oral-cholecystography, to value the correlation between the results and the surgical judgement of laparoscopic cholecystectomy (LC) difficulty level (by Visual Analogue Scale). Wall thickening and diminished gallbladder function were significantly (P < 0.001) associated with increased technical difficulty of LC. Moreover the US evaluation of Veress's needle insertion seat is helpful. There was no significantly association between LC operative difficulty and volume and other US parameters (P > 0.05). Basing on results and on literature review, US and oral-cholecystography can be considered as predictors of LC operative difficulties and it can be used as a screening.

Water supplementation enhances the effect of high-fiber diet on stool frequency and laxative consumption in adult patients with functional constipation.

BACKGROUND/AIMS: The purpose of this study was to determine the effects of a high-fiber diet and fluid supplementation in patients with functional chronic constipation. METHODOLOGY: One hundred and seventeen patients with chronic functional constipation (aged 18-50 years) were randomly divided into two treatment groups. For two months both groups consumed a standard diet providing approximately 25 g fiber per day. Group 1 (58 patients) was allowed ad libitum fluid intake, while Group 2 was instructed to drink 2 liters of mineral water per day. Compliance was monitored throughout the study and results were assessed in terms of bowel-movement frequency and laxative use. RESULTS: Fiber intake was similar in the two groups, while total daily fluid intake in Group 2 (mean 2.1 liters) was significantly greater than that of Group 1 (1.1 liters)(p < 0.001). In both groups, there were statistically significant increases in stool frequency and decreases in laxative use during the two-month trial, but both changes were greater in Group 2 (stool frequency: p < 0.001 vs. Group 1; laxative use: p < 0.001 vs Group 1). CONCLUSIONS: A daily fiber intake of 25 g can increase stool frequency in patients with chronic functional constipation, and this effect can be significantly enhanced by increasing fluid intake to 1.5-2.0 liters/day.
