Comparison between two different assays for measurements of allergen-specific IgE and skin prick test in the diagnosis of insect venom allergy.

Forty-one patients stung by wasp and 29 by bee were evaluated by comparing the correlation between skin prick test (SPT), Phadebas radioallergosorbent test (P-RAST) and Magic Lite SQ Specific IgE assay (LIA). All patients had had a systemic reaction to insect sting. In the patients stung by bee we found agreement between SPT and P-RAST, and SPT and LIA. Similarly, there were no significant differences in sensitivity between P-RAST and LIA (p > 0.05). In patients stung by wasp, SPT was found to be more sensitive than P-RAST. There were no differences between SPT and LIA or between P-RAST and LIA.

Incidence and progression of myringo-incudo-pexy after secretory otitis.

A cohort of originally 327 healthy children born in 1975 was followed by repeated otomicroscopies and tympanometries from age 5 to age 16 years, in total 9 otomicroscopies. Prevalence of drum pathology, especially myringo-incudo-pexy was investigated. Prevalence of myringo-incudo-pexy was 0.5% at age 5 years, 2.5% at age 7 and 4.2% at age 16. Forty percent of pexies were stable; 36% fluctuated and 26% disappeared. Hearing was surprisingly good at age 16, but the pexy represents a risk for progression.

Emergency preparedness: the best defense for major disasters.

This article discusses how Silver Cross Hospital in Joliet, Illinois developed its disaster plan and how that plan was successfully tested when a tornado plowed through its service area, causing mass casualties who required both inpatient and outpatient care.

[An ambulance manned by physicians--a different model].

An arrangement is described in which the general practitioners on duty participate in the ambulance service and the prehospital treatment of acutely ill patients. This arrangement has been followed prospectively for one year and was utilized in 141 cases. Approximately 40% of the cases involved cardiac patients and the remainder consisted of a varied assortment of other diagnoses. Attempts were made to resuscitate 11 patients with cardiac arrest and two of these could be discharged alive from hospital after admission. It is concluded that this arrangement is feasable as a means of improving the prehospital treatment of acutely ill patients but that it should be supplemented by increased efforts in training the population in resuscitation. In addition, it is concluded that the function of the alarm central was not optimal as half of the alarms came via the doctor on duty instead.

Tattoo removal by overtattooing with tannic acid.

Removal of tattoos was studied in the guinea pig. Professional tattoos were made, and two different solutions of tannic acid were injected intradermally into the tattoos four times. The macroscopic end result was formation of a slight fibrosis with little visible pigment. Histologic examination after treatment showed a moderate fibrosis with little visible pigment situated deeper in the dermis than in the untreated tattoo.

SS-B/La antigen purification, and ELISA detection of anti-SS-B/La antibodies in sera from patients with inflammatory connective tissue diseases.

SS-B/La antigen was purified by immunoadsorbent columns with immunoglobulin from a patient with primary Sjögren's syndrome. Monitoring of the purification was facilitated by the fused rocket-immunoelectrophoresis technique. Technical ELISA variables for the detection of serum antibodies against the SS-B/La antigen were evaluated, and a recommended procedure is described. Prospective investigation of anti-SS-B/La antibodies in 103 blood donors and 131 patients with chronic inflammatory connective tissue diseases, including 43 patients with primary Sjögren's syndrome was performed. Anti-SS-B/La antibody concentrations were above normal in 65% of the patients with primary Sjögren's syndrome (verified by the strictly objective Copenhagen criteria for keratoconjunctivitis sicca and xerostomia) and 9% of patients with other chronic connective tissue diseases. The predictive value for primary Sjögren's syndrome among patients with increased levels of the anti-SS-B/La antibodies attending a rheumatology clinic was 78%.

Dynamics of eardrum changes following secretory otitis. A prospective study.

Children with secretory otitis who, during the period from 1970 through 1975, had been treated with adenoidectomy and insertion of ventilating tubes were reexamined three to eight years after treatment and, again, ten to 16 years after treatment. The changes of the tympanic membranes were analyzed with regard to the occurrence of attic retractions and changes of the pars tensa of the membranae tympani. At the first follow-up examination, varying degrees of attic retractions were found in 37% of ears and, at the second follow-up examination, in 27% of ears. This general improvement was mainly due to normalization of the slight retractions, whereas the frequency of severe retractions increased. Pathology of the pars tensa of the membranae tympani was found in 57% of ears at the first examination and comprised the following: tympanosclerosis (19%), tympanosclerosis and atrophy (10%), atrophy (22%), atrophy and pexi (3%), and adhesive otitis (3%). At the second follow-up examination, pathology of the pars tensa of the membranae tympani occurred in 58.05% of ears and was distributed as follows: tympanosclerosis (26%), atrophy and tympanosclerosis (11%), atrophy (14%), atrophy and pexi (5%), adhesive otitis (2%), and sinus cholesteatoma (0.5%).

Clinical results of early and late operative correction of undescended testes.

Operative treatment of undescended testes was performed in 556 boys (695 testes). The histopathological changes in the testicular tissue were more severe in older patients. However, post-operative clinical results were found to be significantly better in boys treated immediately before puberty than in boys treated at an earlier age.

Na+-dependent uptake of 4-azidophenylalanine by pig intestinal microvillus vesicles. Interaction with neutral amino acid uptake and labelling pattern.

A procedure for the synthesis of tritiated 4-azidophenylalanine in a radioactively homogeneous form is described. The characteristics of the 4-azidophenylalanine uptake by pig intestinal microvillar vesicles and its interaction with the neutral amino acid transport have been studied. 4-Azidophenylalanine is transported into an osmotically sensitive space and the transport is sodium dependent. It is demonstrated that 4-azidophenylalanine transport is inhibited by L-alanine and L-phenylalanine but not by L-lysine and D-glucose. Likewise 4-azidophenylalanine inhibited the initial transport of L-alanine and L-phenylalanine but the initial transport of L-lysine and D-glucose was unaffected. Treatment of microvillar vesicles with 4-azidophenylalanine in combination with photolysis irreversibly inactivated the L-alanine transport. Photolabelling in the presence of sodium using tritiated 4-azidophenylalanine resulted in labelling of 10 peptides, all found in the fraction insoluble in Triton X-100. Omission of sodium from the reaction mixture or addition of L-alanine did not specifically abolish labelling in any of the labelled components.

Development of the ciliary epithelium in human nose.

In 15 foetuses, ranging in age from the 8th to the 23rd menstrual week, the development of ciliated cells in the nose was analysed quantitatively by counting ciliated cells in high power fields. The formation of ciliated cells is the first step in the differentiation from embryonic to respiratory epithelium, which starts in the 9th menstrual week. The density of ciliated cells increases strongly and reaches a maximum already in the 16th week. It does not further increase in older foetuses and prematures. Based on the rapid increase in density it is assumed that the differentiation of ciliated cells takes about 2-3 days which is considerably shorter than for goblet cells.

Isolation and characterization of three new polymyxins in polymyxins B and E by high-performance liquid chromatography.

Two polymyxin antibiotics, polymyxins B and E (colistin), have been separated analytically into ten to thirteen components on a commercial reversed-phase material by isocratic elution with a mixture of acetonitrile, phosphate/formate and acetate buffer containing sodium sulphate and triethylamine. The analytical system was transferred to a preparative system, using a C18-bonded stationary phase, without extensive impairing of the selectivity. The major components of each product were isolated and characterized by high-performance liquid chromatography, amino acid analysis and identification of the fatty acid. Three components were isolated and characterized for the first time. The fatty acid was also identified in some of the minor components.
