[Iliac arteriopathy and Ehlers-Danlos disease (author's transl)].

A man of 40 years of age with the Ehlers-Danlos syndrome was noted to have an arteriopathy characterized by a rectilinear aspect of the iliac trunks with stenosis of the external iliac. Macroscopic examination showed thickening of the tunica of the arterial walls, while an increase in the elastic fibers in the intima was observed on histological examination. This type of arteriopathy appears to be extremely rare in the Ehlers-Danlos syndrome but these findings show similarities between this case and certain arterial affections of systematized elastorrhexis. The possible relationships between arteriopathy and elastic dysplasia are discussed as well as the classification of this very particular type of arteriopathy reported.

[Etiologic diversity of digital arteritis].

The etiological study of a series of 30 cases of digital arteritis, shows that there may be various causes at the origin of a digital arteritis. In elderly patients, atherosclerosis appears as the most frequent cause. At a middle age, digital arteritis frequently occurs in a context of vasomotor pathology: in this case, sclerodermis is a frequent cause to examine systematically but some cases are secondary to a Raynaud's phenomenon, apparently idiopathic. Other etiologies: traumatic, hematologic, iatrogenic... may be pointed out; but in some cases, digital arteritis occurs alone without any apparent reason.

[Indications and place of surgery in acrosyndromes of the hand with arteriolitis].

In acrosyndromes of the hand with lesions of the digital arteries, the surgical indication must first be based on an accurate etiological diagnosis. The identification of a high lesion, usually subclavian, with the potential of embolus will be an indication for a direct approach of the lesion at the subclavian level. In case of isolated digital arteritis, secondary to a Raynaud's phenomenon or apparently primary, an operative indication must be kept in mind in case of trophic disorders and severe pain: thoracic sympathectomy gives usually in these cases very satisfactory results.

[Arteritis of the lower limbs after the age of 70. Various practical problems].

Arteritis in old age has certain features which make its treatment difficult. In this paper we underline the following points: the seriousness of amputation, not only from a functional point of view but also with regard to life itself; because of this, the justification for trying to save the limb whenever this is possible; the phenolization of the sympathetic nervous system may be indicated in an interesting way in these cases; the intermediate results of restorative arterial surgery show that good results can be achieved even when the patient is very old. So there are often real therapeutic possibilities for these patients.

[Post-phlebitis varicose veins: pathogenetic role and treatment principles].

Many clinical types can be distinguished among the post-phlebitis varicose veins : Substitution varicose veins : pre and supra pubic varicose veins that should be left intact-superficial post-phlebitis venous insufficiency which is characterized by a saphenous insufficiency that is not significantly different as a whole in its physiopathology from the essential saphenous insufficiency, and requires therefore the same forms of therapy. Superficial venous insufficiency combined with an insufficiency of communicating veins of the leg are quite frequent in post-phlebitis situations and can necessitate a special procedure on the insufficient communicating veins. Finally an isolated insufficiency of the communicating veins.

[Venous physiopathology. The major pathogenic processes in venous pathology and their consequences].

Various pathological circumstances, and especially: acute venous thrombosis, superficial venous insufficiency and post-phlebitic disease, may upset the conditions of venous circumlation. Among the major pathological processes that may thus be involved, one can underline: -- Venous obstruction, whose physiopathology is different in the acute thrombosis and in the post-phlebitic phase. Especially studied in this work are: the phases of the enry into the acute venous stasis, which may lead to venous gangrene -- the offset and non offset obstructive syndromes. The offsetting of a venous obstruction depending on the development of a sufficient and effective substitute circulation. -- The orthostatic reflux may involve the superficial venous network (superficial venous insufficiency) or the deep one (deep venous insufficiency). In either case, the insufficiency of the communicating veins may complicate the primary venous insufficiency. -- On the tissue level, various pathogenic processes induce a peripheral venous stasis, generating problems that may be classed under three headings: lymphatic repercussions, arterial problems and dysfunctions of the arterio-venous anastomoses. These various processes and their consequences on the tissue level, are studied.
