Design and synthesis of effective antagonists of substance P.

The agonist/antagonist activities of four background analogs of substance P (SP) facilitated design and synthesis of 12 new analogs to achieve effective antagonists. (D-Pro2, D-Phe7, D-Trp9)-SP, (D-Pro2, D-Trp7,9)-SP and (D-Arg1, D-Phe7, D-Trp9)-SP showed no agonist activity; 9 analogs showed weak agonist activity of SP. (D-Pro2, D-Trp7,9)-SP was the most potent antagonist which at a concentration of 10(-5) required a 3-fold increase in SP to allow a 50% response by SP. (D-Pro2, Lys6, D-Phe7)-SP and (D-Pro2, D-pClPhe7, D-Trp9)-SP were also potent, and the antagonism was competitive. For specific pairs of peptides, Lys6 is a promising substituent. D-Trp7,9 was as effective as Lys6, D-Phe7. D-pClPhe7 was three times as effective as D-Phe7. D-Dln6 was 1.33-fold better than D-Gln5. D-Pro2 and D-Pro4 were equally effective. D-Pro2 was 1.5 times as effective as D-Lys3. D-Pro2 may not be important. D-pClPhe9 and D-Trp9 were equally effective.

Quality of care in hospital emergency departments and family physicians' offices.

Indicator conditions were used to evaluate the quality of 686 episodes of care provided in two emergency departments and in five family physicians' offices. Overall, the care was considered adequate in 53% of the emergency department cases and in 40% of the cases dealt with in family physicians' offices, the difference being significant (P less than 0.01). Referrals were very common in both settings, and when quality was assessed solely on the basis of the care actually given by the primary-care providers the difference between the two settings disappeared. Half the observed deficiencies in care related to failure to document the findings from history-taking and physical examination. From these and earlier findings we conclude that the emergency department can be an appropriate setting for the care of nontraumatic illness.

Immunosuppressive alpha globulin from bovine thymus and serum: mode of action upon afferent and efferent arcs of the mouse immune response.

Immunosuppressive bovine alpha globulins of both serum and thymus origin were analyzed for in vitro effects upon the afferent and efferent arcs of the immune system. Although considerable variation in potency occurs between preparations, the functional identity of thymus and serum fractions is apparent. Antigenic recognition and effector capability are not altered whereas the proliferative lymphocyte response is either augmented or suppressed, depending upon the concentration of alpha globulin. A rather selective effect is noted on DNA synthesis and is followed by parallel changes in the development of effector capability, as measured by the development of alloimmune cytotoxic lymphocytes.

Low molecular weight dextran infusions in systemic sclerosis with Raynaud's phenomenon: a report of nine cases.

Two litres of low molecular weight dextran in normal saline were given intravenously over 48 hours to 10 patients with systemic sclerosis and Raynaud's phenomenon. The finger temperatures of nine were recorded under standardized conditions. One patient had lowered temperatures of the fingers, two had a rise in temperature, and six had no change. There were no side effects of the treatment. None of the 10 patients showed any clinical improvement.

Treatment of acne vulgaris with tetracycline hydrochloride: a double-blind trial with 51 patients.

Fifty-one patients with acne vulgaris were included in a double-blind trial to assess the therapeutic effect of 250 mg. tetracycline hydrochloride twice daily for three months. The results, assessed clinically and photographically, showed that tetracycline hydrochloride had a statistically significant beneficial effect. Hence, since it is cheap and rarely has side-effects in healthy young adults, its use is suggested, as well as local therapy, in the more severe forms of acne.

Structural-functional relationships in type I insulin-dependent diabetes mellitus in humans.

The major renal pathologic changes of diabetes include thickening of all renal extracellular basement membranes and mesangial matrix and, to a lesser extent, mesangial cell expansion. Two renal lesions appear critical in diabetic nephropathy. Mesangial expansion out of proportion to the size of the glomerulus is closely and inversely related to measures of peripheral capillary wall filtration surface and to clinical features of proteinuria, hypertension, and decreasing glomerular filtration rate (GFR). Arteriolar hyalinosis is related to global glomerulosclerosis, and both are correlated with the clinical features of nephropathy. These lesions are markedly advanced by the time renal dysfunction is clinically detectable. Relationships of structure and function early in the course of the diabetes have not been examined satisfactorily.

Gastrografin versus dilute barium for colonic CT examination: a blind, randomized study.

Fifty patients receiving oral contrast medium the day of their CT examinations were studied to evaluate colonic opacification. Two groups were given small volumes (20 versus 30 ml) of nondilute diatrizoate meglumine/diatrizoate sodium (Gastrografin) the night prior to examination and were compared with groups receiving no contrast medium (control) or a large volume (600 ml) of dilute barium the night prior to examination. Frequency of total colonic opacification was as follows: 30 ml Gastrografin, 92%; 20 ml Gastrografin, 71%; dilute barium, 46%; and control, 9%. Significant artifact was infrequent in all groups. Administration of small volumes of nondilute Gastrografin is a well tolerated, inexpensive, and effective method for opacifying the colon for CT.

Dermatologic disorders in agriculture.

The risk and types of occupational skin disease among agricultural workers depend on the crops, livestock, farming practices, and climate of the area involved. This article emphasizes those conditions prevalent in the midwestern section of North America. Contact dermatitis, zoonoses, and skin cancer are reviewed.

Microangiopathic glomerulopathy in children with sickle cell anemia.

We studied kidney biopsy specimens from three children with sickle cell anemia and microangiopathic glomerulopathy. One child also had cyanotic congenital heart disease. Laboratory evaluation revealed proteinuria and normal serum creatinine in all and normal serum complement in two of the three children at the time of biopsy. In all biopsies, glomeruli were enlarged with diffuse hypercellularity and focal segmental mesangial interposition; capillary loop lumens were congested with sickled erythrocytes. Immune labeling identified segmental immunoglobulin G, C3, and properdin over the glomerular capillary loop walls in each case. Ultrastructurally, the subendothelial zone of the glomerular basement membrane was widened with new lamina densa formation with focal mesangial interposition. The glomerular lesion we describe in these children may be due to endothelial injury related to the altered erythrocytes, glomerular hemodynamics, and the hypercoagulable state characteristic of sickle cell disease.
