Continuation and effectiveness of programme and non-programme methods of family planning in Sri Lanka.

"The objective of this paper is to examine continuation and effectiveness of contraception based on a 1986 survey of more than 3,200 women in rural Sri Lanka. Data on both programme and non-programme methods of contraception are analyzed. Programme methods are defined as including all those methods that are provided through the public or private family planning programmes (namely, oral pills, IUDs, injectables, condoms, female barrier methods and sterilization). Non-programme methods refer to those that are not provided through any structured programme; these methods include calendar rhythm (commonly referred to as 'safe period' in Sri Lanka), withdrawal, abstinence and other traditional methods."

Assessing potential demand for Norplant implants in Sri Lanka.

This study assesses potential demand for Norplant implants using data from a population-based survey conducted in rural Sri Lanka. Overall, 35% of currently married non-sterilized women of childbearing age expressed interest in using the implants, 52% did not want to use the method, and 13% were unsure about using it. The demand was substantially higher (44% vs 28%) among younger age groups (less than 30 years) than among older age groups (30-44). The results showed that life-cycle variables (age, parity and marital duration) and fertility preference variables (desire for another child and desired birth-interval) were generally more important than socioeconomic variables in determining variations in potential demand for Norplant in Sri Lanka. Desired birth spacing was found to be a powerful indicator of the potential demand. The majority (85%) of respondents interested in using Norplant were also willing to absorb at least some of the cost of the implants. Among older age groups, those who were using modern temporary methods of contraception either exclusively or in combination with traditional methods had a higher demand for Norplant than non-users or exclusive users of traditional methods. For the younger women, however, potential demand did not vary significantly according to whether they were currently using a method or the type of method used.

[Malignant pleural mesothelioma: diagnosis, therapy and prognosis].

Clinical and radiological signs in 44 patients with a malignant mesothelioma of the pleura are described. 18 patients underwent radical pleuropneumonectomy with total resection of the ipsilateral diaphragma. In addition to mediastinal nodes were resected in eight cases, in three patients parts of the thoracic wall had to be removed, and in six patients partial resection of the pericardium was performed. In 15 patients (34%) palliative tumor resection was performed and in ten other persons an exploratory thoracotomy and palliative tumor resection. One patient is alive 13 years after radical pleuropneumonectomy. The mean survival time of the patients who died is after radical pleuropneumonectomy 20 months (0.2 to 54 months), after radical pleuropneumonectomy with pericardial resection 8.5 months (3 to 16 months), after palliative resection 9.7 months (0.5 to 29 months) and after exploratory thoracotomy 6.5 months (0.5 to 19 months). Three patients died within a month (6.9%), one after a radical pleuropneumonectomy, one after a palliative resection and one after an exploratory thoracotomy. The first patient died in the late sixties due to a postoperative aspiration, the other two due to tumor progression. We conclude that the radical pleuropneumonectomy can contribute to a better prognosis of the malignant pleural mesothelioma.

Modulatory effect of the gastrointestinal tract (gut) on fibrinolysis and fibrinogen derivatives.

We have investigated some components of the fibrinolytic system and fibrinogen derivatives in blood samples taken simultaneously from the human portal and cubital veins. In the first series the blood was drawn during laparotomy from 12 cholecystectomized, otherwise healthy patients. The mean value of euglobulin lysis time was significantly lower in the portal than in the cubital vein. The values of the fibrinogen, plasminogen, serial dilution protamine sulfate, and ethanol gelation tests in both samples were quite similar. In the second series blood was taken from eight patients with cancer of the gut and from six cholecystectomized patients. The staphylococcal clumping test (SCT) in serum was performed for fibrin degradation products (FDP) in samples from both veins. In patients with cancer the mean FDP level was significantly higher in the portal than in the cubital vein. In cholecystectomized patients the SCT was negative in samples from both veins. We conclude that fibrinogen pathway metabolism does not differ in the portal and cubital veins under normal conditions, whereas in neoplastic disease fibrinogen degradation is greater in the portal vein. We also suggest that the gut probably modulates fibrinolytic activity in normal conditions.
