Comparison of dextranomer and streptokinase-streptodornase in the treatment of venous leg ulcers and other infected wounds.

The clinical efficacy of dextranomer (Debrisan) and streptokinase-streptodornase (Varidase) was compared in a controlled randomized in-patient study. There were two patient groups: Group A consisting of 28 patients with a total of 31 venous leg ulcers and Group B consisting of 56 patients with other infected wounds (posttraumatic and postoperative wounds, amputation stumps, burn wounds, arteriosclerotic ulcers, decubital ulcers, ulcers due to bone disease, rheumatic ulcers, ulcers due to erysipelas and wounds of mixed aetiology). Both agents have a good cleansing effect as well as a good effect on infection and wet necrosis. Dextranomer stimulates the formation of granulation tissue faster than streptokinase-streptodornase.

Trapped air in extrinsic allergic alveolitis.

Non-ventilated lung volume (trapped air) during tidal breathing and several other lung function parameters were determined in 19 patients with extrinsic allergic alveolitis, most of them with farmer's lung, and in 28 non-smoking healthy subjects. The trapped air was significantly larger (p < 0.01) in the patients (mean 0.73 litres) than in normal subjects (mean 0.32 litres); in 7 patients (37%) it was definitely enlarged. The results suggest that the frequent increase of trapped air may be caused by patchy small airways occlusion due to bronchiolitis.

The effect of ICI 74,917 on asthma and bronchial provocation tests.

A study was made of the protective action of the anti-allergic drug ICI 74,917, a phenanthroline derivative, in specific allergen provocation tests and in combating clinical symptoms in thirteen asthmatic out-patients. It was found that ICI 74,917 was able to diminish allergen-induced bronchoconstriction significantly (P less than 0.01) during both the placebo period and during chronic treatment with ICI 74,917 for 4 weeks. The results obtained during these two periods did not differ significantly. There was not, however, any statistically significant difference in the symptom scores or PEF rates during the treatment with ICI 74,917 or placebo for 1 month. This may indicate that although ICI 74,197 is able to prevent bronchoconstriction in an allergen provocation test, it failed to improve asthma clinically. No signs of the phenomenon of tachyphylaxis were observed in the present study. The Student's t-test was used in the statistical analysis.

Serological cross-reactions against Yersinia enterocolitica in patients infected with other arthritis-associated microbes.

In order to investigate arthritis-triggering, serologically cross-reactive antigens, sera from patients infected with arthritis-associated microbes, Salmonellae, Chlamydia trachomatis, and Sindbis-like alphavirus, were reacted against SDS-PAGE separated and immunoblotted Yersinia enterocolitica 0:3 antigens. These sera reacted with Yersinia to the same extent as did the control sera taken from patients with streptococcal, staphylococcal and Bordetella pertussis infections or from healthy blood donors. Moreover, the various sera produced different reactivity patterns, directed against several different antigens. Although sera from test subjects, as well as from controls including healthy individuals, recognized some Yersinia antigens, these patterns differed markedly from those recognized by sera taken from patients with Yersinia infection. Significantly, analysis of the reactivities against the different molecular weight antigen components of Yersinia revealed no dominant band or pattern which could thus have been defined as arthritis-associated.

Lymphocyte subpopulations, cytokine release and specific immunoglobulin G in reactive and nonreactive beekeepers.

Although bee venom sensitization and systemic sting reactions are common among beekeepers, the prediction of the severity of reactions has not yet been possible with laboratory tests. The present study was designed to evaluate parameters that might be clinically useful in estimation of systemic reactivity, and parameters that could differentiate allergic beekeepers from sensitized subjects. Thirty-two beekeepers were selected and placed into the following three groups: anergic (n = 10), asymptomatic sensitized (n = 11), and allergic (n = 11). Peripheral blood lymphocyte subpopulations, venom-specific immunoglobulin (Ig) E and IgG and cytokine release by peripheral blood mononuclear cells were measured. The ratio of stimulated interleukin-4 to stimulated interferon-gamma was significantly higher in sensitized beekeepers than in allergic or anergic subjects. Venom-specific IgG correlated significantly with the number of annual stings (r = 0.575) and the years spent in beekeeping (r = 0.471). No significant differences in the subpopulations of peripheral blood lymphocytes were found between the study groups. We conclude that differences in the subpopulations of peripheral blood lymphocytes are not associated with sensitization or systemic reactivity. In asymptomatic sensitized beekeepers, T helper 2 T-cell dominance is more pronounced than in allergic subjects. Bee venom specific IgG correlates directly with the degree of exposure to bee venom.
