[Cowden syndrome].

A patient with multiple hamartoma syndrome or Cowden's disease with multiple gastrointestinal polyps and malignant melanoma is presented. The syndrome is characterized by hamartomatous tumors of the skin, fibrocystic disease of the breasts, gastrointestinal polyps and disease of the thyroid gland such as goiter and adenoma. In addition, other abnormalities and malformations occur in the skeletal system, central nervous system and urogenital tract. Recognition of this syndrome is important because of the association with malignant tumors of the breast and thyroid gland and with malignant melanoma. Gastrointestinal hamartomatous polyps may lead to the diagnosis of Cowden's disease which must be separated from other intestinal polyposis syndromes.

[Management of acute intrathoracic aortic dissection].

At the intensive care station of the Internal Medicine Department, University Clinic Graz, all patients admitted were promptly and carefully examined in case of suspected acute intrathoracic aortic dissection and immediately treated. In 1987 a total of 4446 patients were referred to this CCU, 893 suffered from acute thoracic pain. In 21 patients of the latter a clinical suspicion of aortic dissection was observed. In 9 patients of these, diagnosis could be established within 5 hours by echocardiography, CT and angiography. 11 patients had no dissection while one positive finding could only be determined by postmortem exam. Retrospectively, these figures for 1982 were 3908 admitted patients, 790 suffering from acute thoracic pain of which only 5 showed suspected dissection. In 3 cases dissection could be confirmed using the same diagnostic procedures, but 2 were without finding. Further 3 patients out of the total series were affected with dissection which could only be determined postmortem. Out of the 9 patients with positive diagnosis, 6 showed Typ I, 1 Typ II and 2 Typ III dissection according to DeBakey (8m, 1f, range 46-78 years). 2 patients underwent surgery immediately after diagnosis, 1 after 12 hours, while 4 cases were conservatively treated. These 7 patients have survived by controlled hypotension up to now (10 to 22 months). Two patients died 2 and 10 hours, respectively, after admission with positive diagnosis. In spite of the severity of this disease prompt and efficient diagnosis and rapid therapeutic management are the key to survival.

[High-resolution computed tomography of the thorax in lymphangioleiomyomatosis and tuberous sclerosis].

We present three cases that illustrate the HRCT appearance of pulmonary lymphangioleiomyomatosis and pulmonary involvement in tuberous sclerosis. All patients had multiple cystic lesions diffusely distributed throughout the lungs. The lack of fibrotic changes was a prominent feature in all patients. HRCT correlated more closely than conventional CT with the pathologic findings.

Abdominal metastases of an ovarian cancer demonstrated on bone imaging.

Tc-99m HMDP uptake by abdominal metastases of an ovarian cancer was seen on a bone image. The findings were confirmed by computed tomography and by biopsy at surgery. The psammoma bodies throughout the tumor tissue appeared to be the substrate of this unusual extraosseous tracer uptake.

The CT findings of pulmonary sarcoidosis: analysis of 25 patients.

We analyzed the CT findings in 25 patients with biopsy-proved pulmonary sarcoidosis. In all 25 patients, 10-mm collimation scans were available. In 16 of the 25 patients, select 1.5-mm scans were obtained. These were retrospectively targeted by using a 20- to 25-cm field of view and a high-spatial-resolution algorithm. The CT and pathologic findings were compared in two patients in whom surgical specimens of the lung were available. CT findings included hilar and mediastinal adenopathy (n = 22), subpleural nodules (n = 19), and 1- to 10-mm-diameter nodules (n = 17) and irregular linear densities (n = 12), both mainly along the bronchovascular structures. High-resolution CT was superior to conventional CT in the assessment of subpleural nodules and irregular linear densities, but conventional CT was superior in the assessment of peribronchovascular nodules. The two gross pathologic specimens showed the sarcoid granulomas to be mostly along the lymphatics in the peribronchovascular sheath and, to a lesser extent, in subpleural and interlobar septal lymphatics. We conclude that the characteristic CT appearance of pulmonary sarcoidosis consists of small nodules and irregular linear densities along the bronchovascular bundles.

[Computerized tomography image of entero-enteric invagination].

A case demonstrating the computertomographic appearance of entero-enteric intussusception in an adult patient is presented. Unlike conventional barium studies CT is able to demonstrate completely the intussusception complex. Its characteristic CT-appearance allows a definite diagnosis.

[Computerized tomography in the diagnosis of allergic bronchopulmonary aspergillosis].

In four patients with immunologic and laboratory findings consistent with allergic bronchopulmonary aspergillosis a CT of the chest was performed. CT revealed central bronchiectasis in all patients. Other features of the condition revealed by CT were mucoid impaction, peripheral alveolar infiltrates, and segmental or subsegmental atelectasis. Owing to their central location the bronchiectatic bronchi always appear parallel to the image plane. This means they can generally be visualized by CT.

[Conventional and computed tomographic findings in polysplenia syndrome].

A case of polysplenia syndrome associated with abdominal situs inversus and anomalies of the inferior vena cava system is demonstrated by conventional and CT examinations. The conventional radiographic findings already suggest the diagnosis; final verification can be achieved via computed tomography.

[Pulmonary involvement in tuberous sclerosis].

The authors present a case of tuberous sclerosis with marked pulmonary involvement, confirmed by both radiological and pathological studies. The radiological manifestations and basic pathology of this rare condition are reviewed with emphasis on differential diagnosis.

[Skeletal changes in tuberous sclerosis].

This paper is based on the skeletal changes which were found in six cases with confirmed tuberous sclerosis. The bone changes of this rare condition are summarised. The differential diagnosis is discussed.

[The computed tomographic spectrum of pulmonary aspergillosis].

The authors describe the CT findings in 10 cases of pulmonary aspergillosis which include allergic bronchopulmonary aspergillosis, aspergilloma and invasive pulmonary aspergillosis. CT reveals a more distinct radiologic-pathologic correlation than plain films and shows suggestive features of pulmonary aspergillosis earlier and in a more detailed fashion. Thus the use of CT in the appropriate clinical setting should allow earlier diagnosis than conventional radiography.

[Clinical assessment and therapy of barbiturate poisoning (based on 2 case reports)].

The clinical features, complications and course of barbiturate poisoning are described and illustrated by two case histories in order to recommend therapeutic guidelines on the basis of practical experience. For the intensive care physician it is essential to get information on the composition of barbiturate-containing drugs as rapidly as possible in order to determine the correct therapeutic management. A table is presented of the components of the most commonly used barbiturates.

[A 42-year-old patient with a metastasizing insulinoma].

We report the case of a patient suffering from a rapidly metastasizing insulinoma who after a period with noncharacteristic symptoms suddenly fell into confusional states leading to initial misinterpretation.

[Cerebral manifestations in non-Hodgkin lymphoma and in lymphogranulomatosis on the CT image].

Primary or secondary cerebral non-Hodgkin-lymphoma or Hodgkin's disease are very rare. The incidence varies from 0.2 to 4%. There is a predilection for the basal ganglia, corpus callosum, thalami and subependymal regions. Hodgkin's disease involving the brain usually infiltrates from the meninges. Solitary tumors are rare, multifocal tumors are extremely unusual. The typical CT appearances of intracranial lymphomas are described in three cases. These consist of isodense or hyperdense intracerebral lesions with marked increase in density on contrast enhancement. Periventricular tumor expansion in NHL is characteristic.
