[Plasminogen activator inhibitor (PAI-1) and diabetes mellitus. I. Regulation of PAI-1 levels].

Elevated PAI-1 levels were described in different insulin resistant conditions, incl. diabetes mellitus type II. In its formation probably several stimulating factors participate. Hitherto accomplished studies suggest not only the important effect of insulin acting synergically with very low density. lipoproteins (VLDL) on hepatocytes but also the importance of the endothelial pool where the action of proinsulin, VLDL and cytokines in synergy with Cai-dependent stimuli (oxidation stress, thrombin) is involved. Under certain circumstances also another adipocyte compartment may play a role, and a significant role of thrombocytes in raising the PAI-1 level cannot be ruled out either. A more detailed analysis of the raised PAI-1 level in different patients could extend the possibilities of therapeutic reduction of levels of this important risk factor of atherosclerosis.

[Plasminogen activator inhibitor (PAI-1) and markers of endothelial dysfunction in patients with diabetes mellitus].

The authors examined 25 patients with diabetes mellitus type 2 (NIDDM) without vascular complications, treated by sulphonyl urea preparations, 12 hyperinsulinaemic (HI) non-diabetic subjects and 11 normoinsulinaemic healthy subject s. Patients with NIDDM and HI non-diabetics had significantly elevated PAI-1 levels which correlated with the C-peptide level (r = 0.519, p < 0.001), triacylglycerols (TG) (r = 0.685, p < 0.001), BMI (r = 0.607, p < 0.001) and levels of endothelial markers such as von Willebrand s factor and thrombomodulin (TM). In the group of patients with NIDDM no relationship of PAI-1 and C-peptide was found and a significant correlation was found with TM levels (r = 0.609, p = 0.001) and TG levels (r = 0.476, p = 0.046). The results suggest that the endothelial department has an effect on the regulation of PAI-1 levels in patients with NIDDM.

[Hemostasis in patients with diabetes mellitus. I. Markers of endothelial dysfunction].

In the submitted pilot study the authors examined 47 diabetic patients without vascular complications and 15 healthy blood donors. In an aged-matched sub-group the authors confirmed significantly elevated levels of Willebrand factor (vWF) in patients with non-insulin dependent diabetes mellitus (NIDDM), as compared with healthy blood donors, while the thrombomodulin (TM) levels did not differ significantly. The mutual correlation of parameters with calcium-dependent release (vWF, platelet factor PF4 and C-peptide) was confirmed in the group of patients with NIDDM with normal TM values and in the group of blood donors. These findings could be explained by the hypothesis that raised intracellular calcium levels, described already in early stages of diabetes could in diabetic patients participate also in the activation of haemostasis.

[Hemostasis in patients with diabetes mellitus. II. Tissue factor and the extrinsic blood coagulation inhibitor].

In the submitted pilot study we examined 37 patients suffering from diabetes mellitus without vascular complications and 15 healthy blood donors. The diabetic patients had not, as compared with the blood donors, significantly elevated values of the tissue factor (TF) and tissue factor pathway inhibitor (TFPI). The TFPI levels correlated with other markers of endothelial dysfunction, in particular thrombomodulin (r = 0.452, p < 0.01) and with triacylglycerol and cholesterol levels. They did not correlate with age, the C-peptide level and BMI.

Influence of teniposide on platelet functions in vitro.

Teniposide added to citrated platelet-rich plasma reduced platelet aggregation induced by collagen, but did not interfere with ADP-induced aggregation. The availability of platelet factor 3 was decreased irrespective of inducer. Reptilase clot retraction induced by ADP or collagen was reduced. Teniposide did not interfere with platelet adhesion to glass. It did not release lactic dehydrogenase from the cytoplasm of platelets. Coagulation factors were not affected.

Thrombomodulin and von Willebrand factor in smokers and during smoking.

Circulating thrombomodulin (TM) and von Willebrand factor (vWF) were determined in smokers before and after smoking of two filter cigarettes and in control subjects. The basal levels of TM and vWF were significantly increased in smokers relative to controls (p < 0.001). However, levels of these two factors remained unchanged immediately after smoking of two filter cigarettes, while a statistically significant correlation was observed only between plasma TM and number of years of smoking (p < 0.05).

von Willebrand factor, soluble P-selectin, tissue plasminogen activator and plasminogen activator inhibitor in atherosclerosis.

Tissue plasminogen activator antigen (tPA), plasminogen activator inhibitor antigen (PAI-1), soluble P-selectin and von Willebrand factor antigen (vWf) were measured by ELISA in 41 patients with peripheral vascular disease (PVD), 41 with ischaemic heart disease (HD) and in 46 age and sex matched asymptomatic controls. Increased vWf was found in patients with IHD (p = 0.0002) and in patients with PVD (p = 0.0011) relative to the controls but levels did not differ between the two patients groups. Raised tPA found in both PVD (p = 0.0006) and IHD (p = 0.0061) compared to the controls also failed to differentiate the two groups of patients. Soluble P-selectin was also raised in both groups (p = 0.003 in IHD and p = 0.0102 in PVD) with no difference between the groups. There were no differences in levels of PAI-1 between the groups. In the subjects taken as a whole, there were significant Spearman's correlations between tPA and vWf (r = 0.37, p < 0.001), tPA and triglycerides (r = 0.38, p < 0.001), tPA and P-selectin (r = 0.19, p = 0.032), vWf and age (r = 0.25, p = 0.005) and inversely between vWf and HDL (r = -0.25, p = 0.006). These data support the concept that increased levels of tPA may be important in atherosclerosis, and indicate that soluble P-selectin may be useful in further analysis of the role of platelets and the endothelial cell in this disease.

[Elevated levels of plasma von Willebrand factor: a new risk factor for atherosclerosis?].

The authors assessed the plasma level of von Willebrand's factor in a total of 104 patients aged 32-60 years. Of these 10 patients are free from confirmed ischaemic heart disease with no history of acute myocardial infarction and 94 patients after acute myocardial infarction. Samplings for von Willebrand's factor in patients after acute myocardial infarction were made at least two months after the infarction. The patients were subjected to: A) ergometric examination, b) to ergometric and coronarographic examination. The patients after acute myocardial infarction with minimal atherosclerotic changes or without atherosclerotic changes on the coronaries had a normal or slightly elevated plasma level of von Willebrand's factor. With the number of coronary arteries affected with atherosclerosis the plasma level of von Willebrand's factor increased. In patients after infarction with evidence of atherosclerotic changes on the coronary arteries without other risk factors the plasma level of von Willebrand's factor was also high. The results stimulate the hypothesis that high plasma levels of von Willebrand's factor could be a risk factor of atherosclerosis.

[Resistance to activated protein C, so-called APC resistance: the first two cases in Slovakia].

For the anticoagulant action of activated protein C (APC) in addition to protein S another newly detected cofactor is essential. Its deficiency is described as APC-resistance. This defect which is closely associated with the molecule of the factor V seems to be the most frequent cause of thrombophilia. The authors describe two patients where APC-resistance was diagnosed for the first time in Slovakia.

[The incidence of anemia in the Slovak population].

The authors examined in two diagnostic preventive systems a total of 26,620 asymptomatic people in the district. The total number comprised 15,511 women (58.3%) and 12,109 men (41.7%) above 15 years of age. In the group of women anaemia was recorded in 4.1% and in men only in 0.19%. The highest incidence was in women and men in the age bracket from 15 to 19 years and above 70 years (girls 11.5% and boys 1.27%). Sideropenic anaemias accounted for 91.9% in women and 60.8% in men. Macroerythrocytic anaemia in men (13.1%) is associated with chronic alcohol intake.

[Large blood platelets--risk factors in myocardial infarction].

Thrombocytes participate in the pathogenesis of myocardial infarction. Larger platelets are more active than small ones. In patients after an acute coronary a larger number of platelets was found, a medium volume of půatelets and a similar thrombocytocrite as compared with the healthy population of comparable structure and age. The MPV in patients of advanced age is statistically significantly larger. There are no significant correlations between MPV and the risk factors investigated by the authors.

Prognostic value of the epithelial membrane antigen (EMA) in the case of Hodgkin's disease.

The study deals with the results obtained from 155 lymph node biopsies of patients suffering from Hodgkin's disease (HD) who were treated by MOPP and radiotherapy in the same establishment. The specimens in paraffin have been examined for the presence of antigens using several monoclonal antibodies, particularly epithelial membrane antigen (EMA). There exists no correlation between the immunophenotypes towards EMA and histological types. Thirty-three patients whose responses were EMA (+) have the assurance survival more than 10 years on the level 32.4% while those 122 patients with EMA(-) had the same survival on 90% level (p less than 0.001). Thus a new prognostic tool has been found which enables to detect the likely therapeutic failures in the case of Hodgkin's disease.

Advanced Hodgkin disease (clinical stages IIIB and IV): low relapse rate after brief chemotherapy followed by high-dose total lymphoid irradiation.

From January 1980 to September 1986, 50 patients with Hodgkin disease, clinical stages (CS) IIIB (26 cases) and IVB (24 cases) were treated by three cycles of mechlorethamine, vincristine, procarbazine, and prednisone (MOPP) chemotherapy, followed by high-dose (40 Gy) (sub)total lymphoid irradiation, including the spleen. Ten patients (2 CS IIIB, 8 CS IVB) were in failure, and seven (4 CS IIIB, 3 CS IVB) died during their first complete remission (2 from treatment-related complications, 1 from unknown cause, 4 from insufficient supportive care and/or a shortage of health supplies); three patients (CS IIIB) relapsed (2 alive in second complete remission, 1 deceased). After 7 years, actuarial survival and relapse-free duration were, respectively, 64% for the 50 patients and 89% for the 40 patients in complete remission. Unfavourable outcome was observed in patients with pelvic nodal involvement. The low relapse rate (none in CS IVB) was the most striking result after brief chemotherapy followed by total lymphoid irradiation.

Clinical significance of protein C.

The coagulation inhibitors are important components of control mechanisms which ensure the haemocoagulation equilibrium. One of them is protein C, whose level was followed in patients with DIC. and IM. In DIC. cases a significant decrease was noted with regard to the MI groups and healthy donors. EID and ELISA methods were used for testing. The patients exhibiting the protein C defect require a corresponding treatment which should suppress the thromboembolic complications.

Platelet reactivity: a contribution to age dependence studies.

The authors examined 230 healthy donors (20-90 years) as to basic biochemical and haematological parameters and those parameters characterizing the function of blood platelets. Besides well known age dependences of fibrinogen concentration, AT III, erythrocyte sedimentation, glucose, cholesterol, alpha-2-macroglobulin and circulating immunocomplexes the results are presented showing a pronounced dependence of primary haemostatic system upon age. Within the age groups 65-75 years and those above 75 years there was a significantly higher amount of circulating aggregates (increase up to 152%), elevated concentration of beta-thromboglobulin and F VIII R:Ag. The increased reactivity of platelets is detectable even by applying relatively simple tests such as the determination of heparin-neutralizing activity, aggregation response to lower concentrations of induces or the determination of the amount of circulating aggregates.
