[Clinical features and surgical treatment of carcinoma of the pancreas (author's transl)].

From 1964 to 1975 a total of 338 patients were operated on for pancreatic cancer. Duodenopancreatectomy was performed in 69 (20%), at a postoperative death rate of 16%. Second look operations proved to be worthwhile. Thus the resection rate after first operations elsewhere was 27%. Five patients lived for more than five and 19 for more than two years after operation. Since 1971 30 patients have survived so far after radiacal operation.

[Massive gastrointestinal hemorrhage following internal drainage of pancreatic pseudocysts].

In operative treatment of pancreatic pseudocysts by inner drainage there is a risk of massive gastrointestinal bleeding particularly following an anastomosis to the stomach. In 10 patients in whom cystogastrostomy or cystoduodenostomy had been performed elsewhere a second laparotomy was necessary because of acute bleeding. In one patient a cystadenoma of the pancreas had been anastomosed to the duodenum by mistake at the previous operation. The leak of obliteration of the cyst is suggested to be the most important factor in the pathophysiology of bleeding. Inner pseudocyst drainage in a disconnected small bowel loop therefore principally should be performed at the lowest point of the cyst. The indication for an inner cyst drainage, however, must be closely examined since simultaneous pathologic changes in the pancreas often justify a resection to remove the origin of the cyst and, further, averting the complications of an inadequate inner pancreatic cyst drainage.

[Differential diagnosis of benign liver tumours (two cases of focal nodular hyperplasia)].

Two patients (33-year-old man and 24-year-old woman) with focal nodular hyperplasia of the liver were treated by surgery in 1973. This is a rare neoplasm of the liver which, according to past experience, is a benign one. The clinical signs and symptoms in the two patients were uncharacteristic, but on angiography there was marked tumour vascularization. Histologically the tumour is closely related to a hamartoma but its classification is diffucult because of the confused nomenclature.

[Echinococcus multilocularis causing extensive biliary obstruction (author's transl)].

Infection of man by Echinococcus multilocularis occurs relatively seldom, most commonly localizing in the liver in the manner of a malignant tumour. A case of alveolar hydatid disease is reported presenting with extensive biliary obstruction, and a large cavitation in the liver due to necrosis. A review of epidemiology, clinical and roentgenological findings is given.

[Results of cement-free St. Nabor hip prosthesis implantation and clinical and radiological results after 8 to 10 years].

PURPOSE OF THE STUDY: The aim of the retrospective study was to analyse the medium-term results of the cementless hip prosthesis St. Nabor. METHOD: 46 patients with complete postoperative and radiographic documentation were available for clinical review. The average follow-up was eight years and five months (range from eight years, one month to ten years). The average age at the last follow-up was 60 years. Ten patients showed severe coxarthritis and 32 patients a dysplasia of the acetabulum before surgery. Two patients suffered from a severe osteonecrosis of the femoral head and two patients had fractures of the acetabulum or femoral neck, respectively. RESULTS: The medium-term follow-up for the acetabular component showed a survival rate of 97.8%. The radiographic data visualized one loosening as a result of a low grade infection, 18 months after surgery. There was no migration of the acetabular component according to Sutherland et al. and no protrusion was found. None of the acetabular components showed radiolucent lines or was loose according to radiographic criteria. In comparison to the first post-operative radiograph, 26 of the hips were found with a significant reduction of lacunae, twelve of them showed a complete absence at last follow-up. The overall Harris hip score averaged 92 points (range 58 to 100 points) for the entire group. 41 (92%) of all hips were rated as excellent or good. Two hips (4%) of all hips were rated as fair and two hips (4%) were rated as poor. CONCLUSION: The high rate of good and satisfying results in medium-term follow-up proves this St. Nabor acetabular component system to be an excellent implant in total hip arthroplasty for younger patients with or without dysplasia.

Pulmonary tumorlets in cases of "tuberculoma" of the lung with malignant cells in brush biopsy.

Two cases of pulmonary nodules associated with Mycobacterium intracellulare infection were initially interpreted as carcinomas of the lung after brush cytology specimens were reported to contain malignant epithelial cells. In both instances, the cells observed in the cytology specimens exhibited marked cytologic atypia. These cells were identical to those present in tumorlets adjacent to the granulomatous lesion. Tumorlets may thus represent a source of false-positive cytologic findings or constitute a type of microscopic neoplasms of the lung.
