[Surgical treatment of colonic diverticulitis--personal observations].

The aim of paper is presentation of our experience in surgical treatment of diverticular disease. In the period of 11 years we operated on 22 patients aged from 38 to 85 years because of complicated diverticulitis. We admitted to hospital 11 patients as emergency cases; operation was performed urgently in 9 of them due to symptoms and signs of acute peritonitis. The other patients were admitted to hospital because of stenosing inflammatory pericolic mass, pericolic abscess or at least two episodes of acute diverticulitis in the past. Resection of affected part of colon was performed in all patients. Primary anastomosis was performed in 13 patients, Hartmann's operation was performed in 8 patients and suture of caecum in site of resected diverticulum was performed in one patient. There was no mortality. Perioperative complications occurred in 31.8% of patients; anastomotic leak occurred in 1 (7.7%) of 13 patients with primary bowel anastomosis. Results were discussed on the base of recent literature.

[The results of GnRH analog treatment of endometriosis].

Our study consisted of 146 patients with endometriosis diagnosed during laparoscopy. The age of those women varied from 19 to 43. Pathological changes were classified according to Revised American Fertility Society scale. Numeric scale was also used to evaluate clinical symptoms characteristic to this disease. During the initial laparoscopy biopsies were taken, endometrial implants were coagulated, pelvic adhesions deliberated and endometriomas were enucleated or their wall cut out and coagulated. When endometriosis was histopathologically confirmed the hormonal treatment was undertaken during a period of time from 3 to 6 months depending on the severity of the disease. The patients were treated with 3.6 mg gosereline and 3.75 mg triptorelin monthly or with 400 mcg of naphareline daily. The hormonal therapy was monitored by the concentration of estradiol in blood serum. After full cycle of GnRH analogues treatment laparoscopy was repeated. The mean of The Symptom Severity Scores decreased from 7.1 to 2.1 after the treatment which is a 70% decrease. In the group of women with pain complains 96% of patients noticed improvement, in the group suffering from infertility there were 26.3% of patients who got pregnant. GnRH analogues were good tolerated by patients during the treatment.

[VEGF concentration in peritoneal fluid of patients with endometriosis].

The theory of Sampson that endometrial cells and fragments desquamated during the menstrual period are transported through fallopian tubes into the peritoneal cavity where they implant, proliferate and develop into endometriotic lesions is generally accepted. There is increasing evidence that immunological mechanisms play a role in the pathogenesis and pathophysiology of endometriosis. Excessive endometrial angiogenesis is proposed as an important mechanism in the pathogenesis of endometriosis. Evidence is reviewed for the hypothesis that the endometrium of women with endometriosis has an increased capacity to proliferate, implant and grow in the peritoneal cavity. From the known angiogenic factors, vascular endothelial growth factor (VEGF) has emerged as a pivotally important regulator of normal angiogenesis and pathological neovascularization. In present study we evaluated the concentrations of VEGF in peritoneal fluid of patients with endometriosis and showed no correlation between AFS score and VEGF concentration in peritoneal and in ovarian endometriosis. Above results do not confirm former observations indicating the role of VEGF in endometriosis pathogenesis.

Thymus and lymph node cell CD4(+) and CD8(+) marker expression and their angiogenic activity of offspring of salbutamol-treated pregnant C3H mice.

Salbutamol, beta(2)-adrenoceptor agonist is a first choice drug in preterm delivery treatment. The aim of the present study was to determine whether salbutamol can cause any alterations in neonatal immune systems and therefore should be considered in newborn care. The experiments were performed on 4-5- and 6-7-week-old female and male offspring of salbutamol-treated C3H inbred mice. Thymus and lymph node weight, cellularity and lymphatic organs, lymphocytes phenotypes and their angiogenic activity were evaluated. We observed lower thymus weight in 6-7-week-old progeny of salbutamol-treated mothers and in the same time lower thymus cell number in both age groups. Lower lymph node weight was developed in older progeny while cellularity was diminished both in 4-5- and 6-7-week-old offspring of salbutamol-treated mothers. We have not detected any changes in percentage of CD4, CD8, CD3 and CD4CD8 positive lymphocytes in progeny of salbutamol-treated mothers. As far as lymph node lymphocytes phenotype is considered we showed in both age groups lowering of CD4 and CD3 positive cells in experimental groups. In the LIA test (lymphocyte-induced angiogenesis) we showed lower lymph node cell angiogenic activity of salbutamol-treated mothers' progeny in both age groups. In the case of thymus lymphocytes we have not observed any alterations in their angiogenic activity. The differences in histological examination of thymus and lymph nodes were not detected in experimental and control groups.

[Serum VEGF (vascular endothelial growth factor) concentration in patients with endometriosis].

The theory of Sampson that endometrial cells and fragments desquamated during the menstrual period are transported through fallopian tubes into the peritoneal cavity where they implant, proliferate and develop into endometriotic lesions is generally accepted. Accumulating data suggest that deficient immunity against retrograde endometrium during menstruation may be involved in the pathophysiology of endometriosis. Recent studies in women with endometriosis demonstrated functional changes in several immunologic components in the peritoneal fluid as well as in sera of those patients. Among others it was shown that a wild pattern of cytokines take part in events occurring during endometrial cells implantation, proliferation and forming of endometriotic lesions. One of them VEGF seems to play a very important role in neovascularisation and implantation of ectopic endometrial lesions. In present study we evaluated the concentrations of VEGF in serum of patients with endometriosis and showed negative correlation between AFS score and VEGF concentration in peritoneal endometriosis. Above results do not confirm former observations indicating the role of VEGF in endometriosis pathogenesis.

[Electroresectoscopy in submucous fibroids, intrauterine adhesions and uterine malformation treatment].

182 hysteroscopic operations with electroresectoscope use performed in Ist Department of Obstetrics and Gynaecology (Warsaw) between 1996-1999 were analyzed. Myomectomy was performed in 117 patients, resection of uterine septum in 25 and deliberation of intrauterine adhesions in 40 patients. Abnormal bleeding and infertility were the main indications for hysteroscopy. In 70% of patients normalization of menstrual bleeding was obtained and 24% of patients got pregnant. In 3 cases uterine wall perforation was noted, increased uterine bleeding was observed in 7 patient and there was only one case of fluid overload.

Hartmann procedure: place in surgery and what after?

The results of 65 consecutive cases of Hartmann's procedure carried out between 1982/1993 inclusive, were reviewed. There were 41 emergency procedures including 27 cases with complicated carcinoma and 24 elective procedures including 20 with carcinoma, 3 with diverticular disease and one with perirectal haemangioma. Postoperative complications occurred in 25 (38.5%) patients, including 13 (19%) with wound infection. Seven patients had to be reoperated because of intraperitoneal abscess (3 cases) or wound dehiscence (4 cases). There were five (7.7%) postoperative deaths. The mean hospital stay of operation survivors was 16 days (range 8-54). Intestinal continuity has been reestablished in 13 (81.2%) of 16 survivors in the benign disease group and in 4.5% of patients of the malignant group. The mean time to of restoration of continuity was 240 days (range 40-630) after the primary procedure. No one died after these procedures. In five (31.2%) patients postoperative complications were observed: anastomotic leak (2 cases) and wound infection (3 cases). All complications were treated conservatively.

[Results of treating endometriosis with nafarelin].

50 women with endometriosis diagnosed during laparoscopy or laparotomy were given six months treatment of nafarelin 400 mcg/day. Before the treatment and after its completion endometriosis was classified according to the AFS scale. Symptom Severity Score numerical scale was also used. After the completion of nafarelin treatment a decrease in AFS scores from 19.6 to 10.2 was seen. The complete withdrawal of clinical symptoms was seen in 52% of patients, and partial withdrawal in 42% of patients. The mean of the Symptom Severity Scores (dysmenorrhoea, dyspareunia, lower abdominal pain and pelvic exam abnormalities) has decreased from 7.1 to 2.1 points which is a 70% decrease. These results show the high efficacy of nafarelin in endometriosis treatment.

[Relationship between decreased activity of NK cells in umbilical cord blood and infection incidence in children].

In our study the natural cytotoxic activity of the umbilical blood of healthy newborns born in 1987 was examined. All mothers of the examined children were healthy and pregnancy was normal. Blood was drawn from the umbilical vein immediately after the delivery to the heparinized tubes. Mononuclear cells of the blood were isolated using the modified Boyums method. The mean activity of the NK cells was 8.6 +/- 1.5 (SE), and was lower than in the studies by other authors. Three years after the delivery all mothers of the examined children got the questionnaire about the health of their babies. After the analysis of the results, the correlation between lowered NK cells activity in the umbilical blood and the morbidity of the children was shown.

Experimental assessment of canine pancreatic segment function.

The purpose of the study was to examine the variability of pancreatic arterial vascularization and to evaluate a vascular model of canine pancreatic segment for transplantation, providing preservation of normal endocrine and exocrine functions. The study included the assessment of pancreatic segment function on the basis of glycemia and post mortem corrosion-casts dissection.

[Usefulness of hysteroscopy in the diagnosis and treatment of intrauterine adhesions].

During hysteroscopy intrauterine adhesions were found in 96 patients in a group of women with disturbances of fertility and menstruation. The diagnosis and treatment were carried out with a Wolf hysteroscope using CO2 as the expanding gas. The purpose of the study was evaluation of the effectiveness of treatment of these adhesions by means of hysteroscopy. Normal menstrual bleeding after this treatment was achieved in 75% of the treated patients, and 51% of the patients conceived. Hysteroscopy was useful in the diagnosis, treatment and posttreatment control in cases of Asherman adhesions.

[Usefulness of hysteroscopy in diagnosing sterility].

Sterility is a severe social problem. Using hysteroscopy, which is possible thanks to development of medical technology, seems to be an important improvement in diagnostics and therapy of sterility. The aim of this study is evaluation of the usefulness of hysteroscopy in diagnostics of sterility. There was a group of 106 women with diagnosed primary sterility and 82 women with secondary sterility. Wolf and Storz hysteroscopes were used during examinations, whereas the uterine cavity was dilated by CO2 using Wolf Metrometer 2121. During this examination a normal uterine cavity was found in most cases (32.9%). The next findings were: endometrial polyps (27.6%), intrauterine adhesions (23.4%), sub-serous myomas (14.3%), and congenital uterine abnormalities (1.5%). Comparing outcomes of hysteroscopy in both groups of sterile women, the high rate of intrauterine adhesions in the secondary sterility (43.9%) was detected. Performing hysteroscopy in patients with sterility increases considerably the range of informations about reproductive organs. Also the possibility of eye-controlled biopsy, deliberation of intrauterine adhesions or removing of endometrial polyps during endoscopic procedure increases the value of hysteroscopy.
