Topical urea in dermatology.

For years research has been conducted to find a cutaneous moisturizer that is effective, easily available and relatively inexpensive urea, as a atopical agent, is all of this and is also a mild keratolytic. A trial was conducted over 200 patients suffering from a variety of disorders that were selected and using urea topically as urea 10 (10%), urea-20 lotion (20%), Urea-HC (with hydrocartisanl). The agent was applied twice daily for a maximum period of 8 weeks. Results indicate that urea is an effective moisturizer and in promotes the penetration of hydrocelisone into the skin.

Lichenoid tissue reaction.

Lichenoid tissue reaction (LTR) is characterised by epidermal basal cell damage which takes the form of liquefaction degeneration or cell death either apoptosis or necrosis with an associated cascade of histologic events in epidermis and dermis. LTR is found in clinical conditions with lichenoid poikilodermatous and pigmentary dermatoses. A selected group of fifty lichenoid and pigmentary dermatoses such as Lichen planus (LP) Discoid lupus erythematosus (DLE) Lichenoid melanodermatitis (LM) and Lichen nitidus (LN) were studied. In LP basal cell liquefaction degeneration was extensive in comparison to other disease with large number of Civatte bodies and colloid bodies. There were significant vasodilatation in upper dermis inside the massive band like infiltrate. PAS positive basement membrane was disrupted in reaction area. Hypergranulosis was conspicuous. Chronic DLE showed spotty lichenoid reaction in the form of basal cell liquefaction degeneration. Civatte bodies and colloid bodies were infrequent. Infiltrate was more focal but could be band like. Epidermal atrophy and thickening of PAS positive basement membrane were important differentiating features, LM or Melanodermatitis toxica revealed focal mild to moderate liquefaction degeneration of basal cells with atrophy of the epidermis. The infiltrate although band like was less dense with marked pigmentary incontinence in clumps and giant melanophages. Civatte bodies, colloid bodies were not found and vascular changes were less prominent. LN showed localised basal cell damage with claw like rete ridges clutching a dense infiltrate. The dermal infiltrate often showed multinucleated giant cell. Civatte bodies and colloid bodies were not present. In some cases of the overlap syndrome LP/LE a careful study of lichenoid tissue reaction could distinguish these two diseases.(ABSTRACT TRUNCATED AT 250 WORDS)

Orthostatic tolerance of normal Indians and those with suspected abnormal cardiovascular reflex status.

There were 143 healthy Indian males subjected to 70 degrees headup tilt (HUT) for 20 min on 166 occasions to evaluate their cardiovascular response. They responded with a significant rise in heart rate, diastolic pressure, and mean arterial pressure. Pulse pressure fell significantly. Heart rate and diastolic pressure during the tilt were significantly (p less than 0.05) and positively correlated to the height of the subjects. None of the subjects had syncope, which has been reported in normal individuals elsewhere. Of the 57 subjects with suspected abnormality of the cardiovascular reflex status, who were also investigated by the tilt test, 13 (22.8%) developed various degrees of orthostatic intolerance. Of these 13 subjects, 10 had had syncope in the past. In view of these observations, it is concluded that 70 degrees head-up tilt is an important clinicophysiological investigation in the assessment of subjects with a suspected abnormality of their cardiovascular reflex status.

Systolic time intervals in pilots and non-pilots during 70 degree head-up tilt.

Nine highly experienced Indian fighter pilots in current flying practice and 11 healthy non-pilots underwent head-up tilt (HUT) to 70 degrees for 20 min. Heart rate and blood pressure response to tilt stress was similar in the two groups as was the contractile function of the left ventricle measured by systolic time intervals. The highest PEP/LVET ratio of 0.402 seen at the end of HUT was considerably lower than the values seen in other studies (5,11). This suggests that myocardial contractility of Indians adjusts better to tilt stress than that of western subjects not adapted to heat.

Impact of lead on nucleic acids and incorporation of labelled amino acid into protein.

The effects of high administration of lead acetate (3.4 mg/100 g of body weight) for a period of 4 days were studied in albino rats. The weights of lead treated rats were not affected while the liver showed enlargement following treatment. Administration of lead reduced the protein content of liver and concentration of DNA decreased to a slight extent. Protein/DNA ratio in liver remained unaltered and this unaltered protein/DNA ratio following lead treatment suggested nonimpairment of cellular protein concentration. The RNA content of liver was increased following treatment and protein/RNA ratio was reduced. RNA per unit amount of DNA was found to be increased and this increased cellular concentration of RNA might result from the degradation of RNA. Lead administration caused a reduction in the incorporation in vivo 14C-leucine into protein of liver. It has been suggested that the diminished incorporation of amino acid into protein following lead treatment might be affected at the translational level of protein biosynthesis.

100% oxygen breathing during acute heat stress: effect on sweat composition.

Twelve male Indian Air Force subjects were exposed on two occasions to a simulated hot environment (DB 57 degrees C; WB 35.5 degrees C; RH 25%) for a period of 50 min. On one occasion the subject breathed normal atmospheric air while on the other occasion he breathed 100% oxygen at ambient atmospheric pressure. Arm sweat collected at the end of the two experiments was analysed for Na+, K+, Mg++, C1-, and lactic acid. Arm sweat Mg++ was found to be much higher than that reported in the literature. Hyperoxia during heat stress improved arm sweating and showed significantly lower concentrations of Mg++, Na+, K+, and lactic acid. However, the total loss of these cations and lactic acid through the arm were not found to differ significantly for the two experiments. Based on arm sweat concentration, when the total body loss of these cations was worked out for the two runs, only whole-body Mg++ showed a significantly lesser loss during hyperoxia. The selective retention of Mg++ during the hyperoxic heat run and its association in lowering the heat-induced physiological strain are discussed.
