Guidelines for the management of women with abnormal cervical smears 1998.

Four important changes have been introduced in the revised guidelines for the management of women with abnormal cervical smears, 1998. 1. For the purposes of management, atypical squamous cells of undetermined significance (ASCUS) and atypical glandular cells of undetermined significance (AGUS) have been included with low grade squamous intraepithelial lesions (LSIL/CIN1/HPV) except where the smear raises the possibility of a high grade squamous intraepthelial lesion (HSIL) or favours glandular dysplasia. 2. Women with low grade cytological and histological intraepithelial abnormalities will, after initial follow-up, revert to three yearly screening. 3. Requirements for those practising colposcopy have been defined. 4. A formal review of all cases of invasive cervical cancer is advocated.

Sarcoplasmic reticulum Ca(2+)-ATPase of sea cucumber smooth muscle: regulation by K(+) and ATP.

Although several Ca(2+)-ATPase isoforms have been described in vertebrates, little is known about Ca(2+)-transport in the muscle of invertebrates. In the microsomal fraction obtained from the sea cucumber (Ludwigothurea grisea) longitudinal body wall smooth muscle, we identified a Ca(2+)-transport ATPase that is able to transport Ca(2+) at the expense of ATP hydrolysis. This enzyme has a high affinity for both Ca(2+) and ATP, an optimum pH around 7.0, and - different from the vertebrate sarcoplasmic reticulum Ca(2+)-ATPases isoforms so far described - is activated 3- to 5-fold by K(+) but not by Li(+), at all temperatures, Ca(2+) and ATP concentrations tested. Calcium accumulation by the sea cucumber microsomes is inhibited by Mg/ATP concentrations >1 mM and the accumulated Ca(2+) is released to the medium when the ATP concentration is raised from 0.1 to 4.0 mM.

Postnatal development of the calcium-dependency of glutamate release from rat cortical synaptosomes: comparison with 5-hydroxytryptamine release.

The ontogeny of the Ca(2+)-dependency of the depolarisation-induced release of preloaded [3H]glutamate and [3H]5-hydroxytryptamine [5-HT] from rat cortical synaptosomes was examined. 5-HT release was found to be exclusively Ca(2+)-dependent at all ages studied. In contrast, glutamate release only showed a significant Ca(2+)-dependent component from postnatal day 10 [PND 10] onwards. This correlated with the ontogeny of the glutamate accumulating activity of synaptic vesicles, a finding consistent with vesicles being the site of Ca(2+)-dependent release. The effectiveness of K(+)-depolarisation in inducing the Ca(2+)-dependent release of both transmitters increased during the early neonatal period, reaching near adult levels at PND20 for 5-HT and PND30 for glutamate.

Epilepsy and children's social and psychological adjustment.

This research investigates the effects of epilepsy on the social and psychological adjustment of the children studied in the National Health Interview Survey of 1988. Analyses examine the effect of epilepsy on four measures of adjustment--home behavior problems, school behavior problems, depressed mood, and impulsiveness. For each outcome, we address five questions: (1) Do children with currently active epilepsy have poorer adjustment than children with inactive epilepsy? (2) Do children with epilepsy fare worse than other children? (3) Do demographic background and family structure moderate the apparent effects of epilepsy? (4) Do family processes mediate the apparent effects of epilepsy? and (5) Do cooccurring conditions produce the apparent effects of epilepsy? Generally, we find that: (1) Children with active and inactive epilepsy fare about equally; (2) Children with any history of epilepsy fare worse than children without epilepsy; (3) Demographic and family background moderate only a small part of epilepsy's effect; and (4) A combination of family processes and cooccurring conditions appears to produce epilepsy's apparent effects.

Oxidative stress is present in atherosclerotic peripheral arterial disease and further increased by diabetes mellitus.

OBJECTIVE: Atherosclerosis, which is associated with increased oxidative stress, may be widespread and more advanced in both complicated and uncomplicated diabetics than in healthy, age-matched non diabetic controls. The development of microangiopathy and the early development of microangiopathy and the early development of macroangiopathy which occurs in diabetes have unclear aetiology. We have previously shown an increase in the products of free radical activity in conventional atherosclerosis and this study has considered whether there us a further increase in this oxidative stress in diabetics. EXPERIMENTAL DESIGN: We measured oxidation by free radicals as lipid peroxides (malondialdehyde) and plasma thiols in 19 insulin dependent diabetic patients with retinopathy or microproteinuria but no clinical evidence of large vessel disease, 19 non diabetic patients with obstructive peripheral arterial disease confirmed by angiography and 19 asymptomatic matched healthy control subjects. PATIENTS: The patients were recruited from either the local diabetic or vascular surgery clinics. RESULTS: There were significant increases in oxidative stress in diabetic patients compared to control subjects with increased malondialdehyde 9.2 mumol/l (7.1-11.2) versus 7.65 mumol/l (5.4-9.6, p < 0.01 Mann Whitney test) and reduced plasma thiols 458 mumol/l (294-595) versus 506 mumol/l (433-589, p < 0.05). Those non-diabetic patients with obstructive peripheral arterial disease also had significantly increased malondialdehyde levels at 8.4 mumol/l (6.5-10.1, p < 0.05) and reduced plasma thiols 435 mumol/l (361-462, p < 0.05) compared to controls. The difference between patients with conventional atherosclerotic peripheral arterial disease and those with additional diabetes was significant for malondialdehyde (p < 0.05). CONCLUSIONS: These data confirm the oxidative stress that exists in atherosclerotic peripheral arterial disease and suggest that diabetic patients with microangiopathy have an increase in this oxidative stress which may contribute to their early development of atherosclerosis.

Learning through experience: an evaluation of 'Hospital at Home'.

A Hospital at Home service for children was begun in April 1991 in a rural health authority in England. After 18 months a mainly qualitative evaluation has shown that the children and parents concerned have been very satisfied with the service and would choose the same type of care again. General practitioners and health visitors felt it was to the benefit of the whole family for the child to be at home while ill. They acknowledge a high standard of care received by the children but were critical of poor liaison between hospital and community and the loose arrangements for medical responsibility. These areas of strength and weakness are known to have been experienced in other districts establishing paediatric home care. Here, proposals have been made to improve the weaker aspects of the service as a direct response to the evaluation. These are now being put into practice, with one new appointee already in post. It is suggested that the proposals are applicable to other districts planning similar services and could help them to avoid the same pitfalls and establish a more credible service with good community integration from the start.

Phonological behavior in toddlers with slow expressive language development.

Toddlers with slow expressive language development were compared to normally speaking age-mates on three global measures of phonological behavior: the average level of complexity of their syllable structures, the number of different consonant phonemes produced, and the percentage of consonants correctly produced in intelligible utterances. The groups were found to differ significantly on all three variables. Further analyses were done, breaking the groups down into narrower age ranges. These comparisons also revealed differences between late-talking and normal youngsters. Detailed analyses of the range of phonemes and syllable structures produced, as well as the appearance of phoneme classes within syllable structures and positions, revealed that late talkers showed a delayed rather than a deviant pattern of phonological development. The implications of these findings for identifying and monitoring expressive delay in toddlers are discussed.

Portable chest radiography in intensive care: a comparison of computed and conventional radiography.

A prospective study was performed comparing computed and conventional radiography for the detection and visibility of cardiovascular devices in intensive care unit patients. Computed images were obtained using a commercially available 2K x 2K, 12-bit storage phosphor plate system. Image sets from 50 patients were assessed independently by three observers. A significant difference between the types of image was found for the detection of mediastinal drainage tubes and prosthetic valves. Computed images allowed greater confidence in the identification of courses and tips of lines. This advantage was most marked with edge-enhanced computed images.

Na+ effects on intracellular pH of isolated, perfused rabbit gastric mucosal surface cells.

The effects of extracellular Na+ on intracellular pH were studied by perfusing BCECF loaded gastric mucosal surface cells adherent to glass coverslips held in a spectrophotofluorometer. Removal of Na+ from a NaCl Ringer perfusate (pH 7.4) resulted in progressive intracellular acidification, which was partially blocked by amiloride. An H+ conductance did not appear to be present. Acidification induced either by Na+ removal or by a NH4 prepulse was reversed by extracellular Na+, but this effect was not completely prevented by amiloride. Amiloride significantly, but not completely, inhibited Na22 uptake by gastric mucosal surface cells. The data suggest that extracellular Na+ maintains intracellular pH of gastric mucosal surface cells through amiloride-sensitive and -insensitive pathways. In the absence of extracellular Na+, cellular acidification seemed to be partially due to Na+/H+ exchange.

Comparative accuracy of high resolution computed tomography and chest radiography in the diagnosis of chronic diffuse infiltrative lung disease.

One hundred individuals who had undergone both high resolution computed tomography (HRCT) and chest radiography were studied to determine the accuracy of each technique in establishing the diagnosis of diffuse lung disease. The group consisted of 86 patients with a diagnosis of a chronic diffuse infiltrative lung disease and 14 normal subjects. Two independent observers assessed the HRCT examinations and chest radiographs and recorded the three most likely diagnoses. Overall a confident diagnosis was reached more often with HRCT (49%) than with chest radiography (41%). The diagnoses were correct in 82% of HRCT examinations and 69% of chest radiographs. Diagnoses made on HRCT, irrespective of the degree of certainty, were accurate more often than diagnoses made on chest radiography (56% and 47% respectively). Of the patients thought to have a normal chest radiograph, 42% had diffuse infiltrative lung disease (DILD). Of the patients thought to be normal on HRCT, 18% had DILD. Conversely, normal subjects were correctly identified as such in 82% of chest radiographs and in 96% of HRCT examinations. This study emphasizes the important role of CT in helping to confirm or refute the presence of abnormality when the chest radiograph is normal or questionably abnormal, and underlines the superior diagnostic accuracy of HRCT compared with conventional chest radiography in DILD.

The effect of intermittent local heating on fracture healing in the distal tibia of the rabbit.

Two series of controlled experiments were carried out to investigate the effect of intermittent local heating on fracture healing in the distal tibia of the rabbit. In the first series, the distal tibia was fractured in a reproducible manner and then treated by compression plate fixation. Nine animals received intermittent local heating around the fracture site for 6 weeks postoperatively in comparison with 5 control animals. The mechanical strength of the healed tibia was then measured in a standardized 3-point bend test. Intermittent local heating produced stronger bones than the untreated controls in 6 of the 9 cases, a result which was marginally significant (P = 0.055). In the second series of experiments, a reproducible bone defect was created by drilling a 2 mm diameter hole through the distal tibia; compression plate stabilization was not required. Fourteen animals received intermittent local heating around the fracture site for 3 weeks postoperatively in comparison with 7 control animals. Intermittent local heating produced stronger bones than the untreated controls in 8 of the 14 cases, a result which was not significant (P greater than 0.1). It is concluded that intermittent local heating of the type studied has too small an effect on accelerating the rate of fracture repair to be clinically useful.
