A polyphasic taxonomic study of four strains of Paenibacillus larvae and four strains of Paenibacillus pulvifaciens (including duplicates of both type strains) supported the reclassification of both former Bacillus species into one species, P. larvae. Our conclusions were based on morphological and Analytab Products (API) tests, sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) of whole-cell proteins, gas chromatography of methylated fatty acids, pyrolysis mass spectrometry, DNA-DNA binding, and the following genomic fingerprinting methods: amplified ribosomal DNA restriction analysis, random amplified polymorphic DNA analysis, and AFLP analysis. The last method is a novel high-resolution DNA fingerprinting technique based on the selective amplification of restriction fragments. Despite more than 90% DNA relatedness between the strains studied, SDS-PAGE of whole-cell proteins, biochemical tests, and DNA fingerprinting (AFLP) distinguished between the P. larvae and P. pulvifaciens strains at the subspecies level. Taking this evidence along with differences in pathogenicity, we propose to reclassify the honeybee pathogens P. larvae and P. pulvifaciens as P. larvae subsp. larvae and P. larvae subsp. pulvifaciens. An emended description of the species and descriptions of the subspecies are given. The type strains are P. larvae subsp. larvae ATCC 9545 (LMG 9820) and P. larvae subsp. pulvifaciens NRRL B-3685 (LMG 6911 and ATCC 13537).
We investigated the ability of a recently developed genomic fingerprinting technique, named AFLP, to differentiate the 14 currently defined DNA hybridization groups (HGs) in the genus Aeromonas. We also determined the taxonomic positions of the phenospecies Aeromonas allosaccharophila, Aeromonas encheleia, Aeromonas enteropelogenes, and Aeromonas ichthiosmia, which have not been assigned to HGs yet. A total of 98 Aeromonas type and reference strains were included in this study. For the AFLP analysis, the total genomic DNA of each strain was digested with restriction endonucleases ApaI and TaqI. Subsequently, restriction fragments were selectively amplified under high-stringency PCR conditions. The amplification products were electrophoretically separated on a polyacrylamide gel and visualized by autoradiography. Following high-resolution densitometric scanning of the resulting band patterns, AFLP data were further processed for a computer-assisted comparison. A numerical analysis of the digitized fingerprints revealed 13 AFLP clusters which, in general, clearly supported the current Aeromonas taxonomy derived from DNA homology data. In addition, our results indicated that there is significant genotypic heterogeneity in Aeromonas eucrenophila (HG6), which may lead to a further subdivision of this species. A. allosaccharophila and A. encheleia did not represent a separate AFLP cluster but were found to be genotypically related to HG8/10 and HG6, respectively. In addition, the results of the AFLP analysis also confirmed the phylogenetic findings that A. enteropelogenes and A. ichthiosmia are in fact identical to Aeromonas trota (HG13) and Aeromonas veronii (HG8/10), respectively. The results of this study clearly show that the AFLP technique is a valuable new high-resolution genotypic tool for classification of Aeromonas species and also emphasize that this powerful DNA fingerprinting method is important for bacterial taxonomy in general.
We investigated the usefulness of a novel DNA fingerprinting technique, AFLP, which is based on the selective amplification of genomic restriction fragments by PCR, to differentiate bacterial strains at the subgeneric level. In totals, 147 bacterial strains were subjected to AFLP fingerprinting: 36 Xanthomonas strains, including 23 pathovars of Xanthomonas axonopodis and six pathovars of Xanthomonas vasicola, one strain of Stenotrophomonas, 90 genotypically characterized strains comprising all 14 hybridization groups currently described in the genus Aeromonas, and four strains of each of the genera Clostridium, Bacillus, Acinetobacter, Pseudomonas and Vibrio. Depending on the genus, total genomic DNA of each bacterium was digested with a particular combination of two restriction endonucleases and the resulting fragments were ligated to restriction halfsite-specific adaptors. These adaptors served as primer-binding sites allowing the fragments to be amplified by selective PCR primers that extend beyond the adaptor and restriction site sequences. Following electrophoretic separation on 5% (w/v) polyacrylamide/8.3 M urea, amplified products could be visualized by autoradiography because one of the selective primers was radioactively labelled. The resulting banding patterns, containing approximately 30-50 visualized PCR products in the size range 80-550 bp, were captured by a high-resolution densitoscanner and further processed for computer-assisted analysis to determine band-based similarity coefficients. This study reveals extensive evidence for the applicability of AFLP in bacterial taxonomy through comparison of the newly obtained data with results previously obtained by well-established genotypic and chemotaxonomic methods such as DNA-DNA hybridization and cellular fatty acid analysis. In addition, this study clearly demonstrates the superior discriminative power of AFLP towards the differentiation of highly related bacterial strains that belong to the same species or even biovar (i.e. to characterize strains at the infrasubspecific level), highlighting the potential of this novel fingerprinting method in epidemiological and evolutionary studies.
Thirty-one Acinetobacter baumannii strains, comprising 14 strains from 14 outbreaks in different northwestern European cities and 17 sporadic strains, were compared by investigating various properties of the strains including biotype, antibiogram, cell envelope protein electrophoretic profile, ribotype pattern, and the band pattern generated by a novel genomic fingerprinting method, named AFLP, which is based on the selective amplification of restriction fragments. Results showed that 12 strains from unrelated outbreaks were linked together in two clusters according to their similarities by these typing methods, whereas sporadic strains were more heterogeneous. Outbreak strains appeared to be markedly more resistant to antibiotics than nonoutbreak strains. The uniformity of typing characters in two sets of outbreak strains suggests that strains in each cluster have a common clonal origin.
Between October 1992 and September 1994, 70 patients presenting with primary unilateral hernia were randomly assigned to either a transabdominal preperitoneal laparoscopic (group L) repair (n = 35) with a 3 x 5 inch polypropylene mesh or a conventional Shouldice repair (group S) (n = 35). The series consisted of 60 men and 10 women, with a median age of 56 years and a BMI of 24. We used the Nyhus classification; there were 25 type 2, 24 type 3a and 21 type 3b hernias. Preoperative characteristics of both groups were equivalent. When comparing group L to group S, we noted a similar operative time (65 +/- 20 min) and a similar postoperative hospital stay (3.8 +/- 1.3 days). The postoperative pain that was evaluated on a visual analogue scale (score from 0 to 10) was lower in group L. On the first and third postoperative day, the mean score was 3.4 +/- 1.5 and 1.3 +/- 1.4 in (group L compared) to 5.3 +/- 1.9 (p < 0.001) and 2.8 +/- 1.8 (p < 0.005) in group S. Significant morbidity occurred in 7 patients of group L: 3 seromas and 4 cases of orchitis, and in 5 patients of group S: 1 wound abcess, 2 cases of chronic neuralgia and 2 cases of orchitis. We recorded a quicker return to home activities in group L: 6 +/- 2 versus 10 +/- 4 days (p < 0.05) in group S but time off work was similar in both groups (30 +/- 9 days). All patients were evaluated in April 1996: the median follow-up was 30 months (range: 19-42 months). We recorded 1 recurrence in group L and 3 in group S. Statistical analysis revealed that the recurrence rate, for patients older than 50 and presenting with type 3 hernia, was higher in group S (3/14 = 21%) than in group L (0/13 = 0%). In conclusion, compared to Shouldice repair, laparoscopic herniorrhaphy is associated with less postoperative pain, a quicker return to everyday activities and an equivalent postoperative morbidity. In our series, the recurrence rate was even lower for patients older than 50 with type 3 hernia.
In this study we investigated changes to horizontal cells in human retinae affected by glaucoma. Glaucoma is characterized by raised intraocular pressure and is responsible for retinal ganglion cell and, possibly, photoreceptor degeneration. It was therefore assumed that horizontal cells might also be affected. The carbocyanine dye DiI was placed at discrete points on fixed, whole-mounted retinae obtained from normal and glaucomatous patients. After allowing 6-24 weeks for intramembranous diffusion within the lipid layers of the nerve cells and, therefore, fluorescent labeling, we measured horizontal cell soma and dendritic field sizes. Selected cells were then embedded in Araldite and cut at 4 microns. Horizontal cells in glaucomatous eyes appeared larger and had a granulated outline as compared with cells from normal retinae. Analysis of the mean cell soma size indicated that cells were 26% larger in the glaucomatous retinae and that this increase was significantly different from that seen in normal retinae (P < 0.05). The dendritic field size was unaffected (P > 0.05). As seen in cross section there was a clear loss of photoreceptor outer segments, and shrunken silhouettes of photoreceptor inner segments with pyknotic nuclei were observed. It is proposed that the increase in some size is indicative of horizontal cell responses that are likely to culminate in degeneration as a result of heightened intraocular pressure. In addition, this paper provides further evidence that photoreceptors are affected by advanced glaucoma.
Gas-liquid chromatography of cellular fatty acid methyl esters (FAMEs) was used to determine the phenotypic and genotypic diversity among 489 presumptive Aeromonas strains isolated from five Flemish drinking water production plants. FAME profiles were compared with the predetermined library profiles of a representative database, AER48C, which contains the mean FAME data of all 14 currently established hybridization groups (HGs) or genospecies within Aeromonas. Using AER48C, more than 93% (457 strains) of all presumptive aeromonads isolated on ampicillin-dextrin agar were unequivocally identified as belonging to this genus. Moreover, 85.5% and 73.5% of these strains could be assigned to a particular phenospecies or HG, respectively. Raw and treated surface water samples were dominated by members of the Aer. hydrophila complex (38.8%, comprising HGs 1-3), followed by the Aer. caviae complex (22.7%, comprising HGs 4-6) and the Aer. sobria complex (16.7%, comprising HGs 7-9). HGs 3, 5A/B and 8 were the most prominent genospecies in this type of water. On the other hand, it was found that raw and treated phreatic groundwater samples displayed a much more limited species diversity since these were almost entirely dominated (95.8%) by strains belonging to HGs 2 and 3 of the Aer. hydrophila complex. In general, flocculation-decantation and sand filtration were not shown to influence the overall species distribution in any of the plants examined.

This paper describes a study on the relationship between job characteristics, health and well-being of health care professionals from various sectors (N = 249). More specific, we investigated the Demand-Control-Support Model (Johnson & Hall, 1988), using covariance structure modelling (LISREL 7). Although the data did not confirm the total model, we did found evidence with regard to some predicted relationships. Based on the present data, a revised model was presented. Finally, some practical implications were discussed.
Striated muscle fibres, both skeletal and cardiac of different species including human, skinned by freeze-drying, were activated in solutions strongly buffered for Ca2+. The single fibres were immersed in solutions with different [Ca2+]. Sarcomere length was set and controlled by laser diffraction. Fibre type was determined by Sr2+ activation. The relation between the negative logarithm of the Ca2+ concentration and the normalized tension, the Ca2+ sensitivity curve, was investigated. The effect on the contractile machinery of three different Ca2+ channel antagonists (verapamil, diltiazem and nifedipine) in a therapeutic concentration (10(-6) M) was investigated. The possible effects on the Ca2+ sensitivity curve were quantified by: (1) the change in maximal tension developed at pCa2+ = 4.4; (2) the change in pCa2+ value at which 50% of the tension induced at pCa2+ = 4.4; (3) the steepness of the Ca2+ sensitivity curve in this point. The three drugs tested, at a therapeutic concentration of 1 microM, all enhanced maximal induced tension by respectively 25, 20 and 7%. The sarcomere length dependency of the effect proved to be dependent upon the drug, but also slightly on fibre type (skeletal or cardiac), or on species. It is concluded that the drug influences the cooperativity of the two different types of binding sites on troponin-C (low- and high-affinity sites). Tension enhancement was due to increased stiffness of the actin-myosin interaction site.
Three important issues must be addressed in any attempt to determine whether combination painkillers play a role in analgesic nephropathy. The first issue, namely that of a causal link between the combination itself and nephrotoxicity, has never been adequately documented. On the contrary, there is much evidence that the combination as such has no influence whatsoever. The cause of the nephrotoxicity is most likely the painkilling mechanism, i.e. the antagonism to prostaglandins; the most potent prostaglandin-antagonists, the non-steroidal anti-inflammatory drugs, whether used in combination or singly, also most frequently cause renal pathology. The second issue, i.e. the safety of combination painkilling drugs in comparison with that of single substances, is intimately bound up with the advantages of the former with respect to both activity and the activity-side-effects ratio. The third issue, abuse, should be recast in a broader context. The central element here is not the painkilling drug but rather the labile personality of the user in conjunction with a more or less stressful environment in which a wide variety of drugs and stimulants are available and taken for better 'coping'. To a great extent analgesics abuse can be prevented by information (i.e. social medicine). In a broader perspective, man experiences considerable difficulty adapting to the sweeping social, technological and ideological changes of recent decades, and this transition contributes in no small measure to the analgesics problem. It should be a priority of government to find remedies for this state of affairs.

Social anxiety among 110 stutterers was compared with the measures of two control groups (110 social phobic patients and 110 normal persons). Results do not support the notion of social anxiety as an essential part of stuttering.

Elastase inhibiting activity (EIA) was demonstrated in the epidermis from lesions and in psoriatic scale, whereas normal epidermis did not contain significant EIA. Two new elastase inhibitors were partially purified and characterized using psoriatic scale as a source. The two species (approximate molecular weights 10 and 20 kDa) were shown to be stable, and high-affinity inhibitors of human leucocyte elastase (Ki less than 10(-10) M). No activity against human cathepsin G could be demonstrated. Cultured human keratinocytes were shown to contain EIA activity similar to that found in psoriatic scale. EIA could also be demonstrated in human epidermis following the induction of an experimental inflammatory response by sellotape-stripping. We propose the acronym SKALP (skin-derived antileucoprotease) as a name for these new proteinase inhibitors.

Intravenous alfentanil was administered as a constant 1 h infusion to 6 dogs. Before, during and up to 3 h after infusion, the effects of 3 doses (0.001, 0.004 and 0.016 mg/kg/min) on 6 quantitative EEG measures (zero-crossing frequency, root mean square (rms) amplitude, spectral edge, relative delta, alpha and beta power) were assessed in relation to plasma concentrations of alfentanil. All EEG measures, except zero-crossing frequency and rms amplitude, showed statistically significant dose-dependent changes in peak effect and duration. In addition, times-to-peak effect and return-to-baseline were sensitive to dose. The EEG effects of the low dose were smaller than those of the middle and high doses, whose peak effects did not statistically differ; but the high dose produced more persistent effects, which outlasted the infusion period for a longer time. Alfentanil-induced changes in rms amplitude and relative delta power showed the widest dynamic ranges. Measurable EEG changes occurred at low plasma concentrations, but EEG responses saturated at the middle dose. Significant correlations between plasma concentration and EEG effect were obtained for only the subperiod ranging from onset of infusion to peak EEG effect, indicating very short concentration-effect equilibration delays. On the other hand, clockwise concentration-effect loops were suggestive of acute tolerance: EEG responses peaked before peak plasma levels and they returned to baseline at dose-dependently higher plasma concentrations of alfentanil.
