Malar bone fractures and their sequelae. A statistical study of 1.393 cases covering a period of 20 years.

The authors analyse the results of 1.393 cases of malar fractures treated in the maxillo-facial Departments of Montpellier and Perpignan. A clinical and therapeutic classification is given. Study of the sequelae demonstrated that the most frequent were infra-orbital nerve lesions, residual displacement of the malar bone, diplopia, and enophthalmos. A point is made about the use of Franchebois's inflatable balloon as a means of retention. Its indications as well as its contra-indications are clearly defined. The reduction in the number of bone sequelae, when compared to cases treated without the balloon, demonstrates the efficacy and simplicity of this method.

[The value of Cottle's technic for esthetic and functional correction of the nose].

The "maxilla-premaxilla approach" as well as the surgical respect of the dorsum nasi are what make the Cottle technique doubly interesting: aesthetically and functionally. In relation to the nasal septum cartilage, it allows nasal septum deviation to be considered as the consequence of the disturbance of the surrounding bony structure and constitutes satisfactory treatment by replacing a simple septoplasty with a septorhinoplasty. From an aesthetic point of view, Cottle's technique allows a more natural correction by respecting the osteocartilaginous nasal bridge. Moreover, this technique offers the advantage of correcting Saddle nose with or without bone grafting. The analysis of a group of 87 patients over a period of 4 to 10 years confirms its value and defines the limitations and indications.

Effect of cimetidine on the pharmacokinetics of the new beta-blocker betaxolol.

The effect of cimetidine on the new beta-blocker betaxolol (Kerlone) was investigated in 7 healthy volunteers. Propranolol was used as a reference beta-blocking drug. In the control study, a single oral dose of propranolol (80 mg) was given (D0) followed by 5 daily doses of cimetidine (1 g) (D1-D6) and a second oral dose of propranolol (D7). The study was repeated with 20 mg of betaxolol replacing the propranolol dose. The oral clearance of propranolol was significantly decreased (21%). No significant effect of cimetidine on the kinetics of betaxolol was observed.

[Various cases of ossifying fibroma and cranio-maxillofacial fibrous dysplasia].

Rare and of difficult diagnosis, osteofibroma and fibrous dysplasia can occur in different areas of the face and skull. Differential diagnosis is difficult particularly in the isolated forms and need histological confirmation. This will guide their different treatment depending of their nature and localisation. In case of base of skull involvement eyesight must be looked into particularly well.

[Coral in infra-osseous lesions. Evaluation after 7 years' use].

The coral or madreporaria skeleton is morphologically and chemically very similar to mineral bone. The authors began to experiment with the implantation of selected pieces of coral in the cranial and facial bones of dogs, pigs and monkeys. The obtained true reossification with the wide porous varieties, and a very stable implant with a very good tolerance with the other varieties. Having collected and analysed the first two years results on the animal, we started, in 1981, to use coral implants in corrective surgery of the face by extrapolating in other specific areas, such as periodontology which we believe to be a very promising area of investigation. The filling of the interior bone cavities remains our chief concern; a quick look taken at the available literature on the subject has convinced us of the strong need for grafts in the treatment of bone lesions. We do not propose here a critical or a comparative study of the different materials mentioned: Our goal has been to transmit our finding, based on the importance of madrepore: its wide availability; easy use; economical price; biological qualities; consistent result.

[Acute hematogenic interstitial nephritis of urinary origin: an unrecognized factor in the exacerbation of chronic kidney failure].

Hypertension, phosphate retention, hyperfiltration hyalinosis and the natural course of the underlying are well known factors leading to progression of chronic renal failure. Acute bacterial interstitial nephritis occurring in a previously diseased kidney, although well documented in experimental animals, has not been shown to aggravate chronic renal failure in man. We report on 3 cases of acute suppurative interstitial nephritis, due to E. coli urinary infection complicated by septicemia. All had rapid aggravation of previously mild renal failure secondary to chronic interstitial nephritis. Sepsis originated from the urinary tract which in 2 instances had been temporarily obstructed. Renal biopsy disclosed a diffuse interstitial infiltrate containing numerous polymorphonuclear leukocytes. This was superimposed on chronic tubular and interstitial lesions. In 1 case there were glomerular lesions with crescents and mesangial C3 deposits. A 2nd biopsy performed in 2 cases was of prognostic interest. In one case it showed active lesions and the necessity of continuing the treatment and in the other a satisfactory healing allowing cessation of therapy. Treatment was guided by antibiograms, the clinical and urinary signs of activity, renal biopsy findings and antibiotics known to be concentrated in renal tissue. The duration of treatment seemed important for the regression of acute renal lesions. Hematogenous bacterial interstitial nephritis should be considered as a possible cause of aggravation in chronic renal failure.

[Comparative effects of dopamine and dobutaine in subendocardial perfusion in the acute phase of myocardial infarct].

Dobutamine, a selective agonist of beta I adrenergic receptors was proposed as an inotropic support of the failing left ventricle in the acute phase of myocardial infarction. We studied the action of Dobutamine (10 micrograms/kg/min) on subendocardial perfusion, compared to the effects of an equipotent dose of Dopamine, in 10 patients with acute transmural infarction. Subendocardial perfusion was assessed by subendocardial viability ratio (EVR), ratio of the diastolic pressure-time index to the systolic pressure-time index. A mean 35 p. cent increase in cardiac index was observed with 10 micrograms/kg/min Dopamine, and with 6,75 micrograms/kg/min Dobutamine; heart rate increased by 13 p. cent with both drugs; with Dobutamine, systolic arterial pressure increased by 9,6 p. cent while pulmonary wedge pressure decreased by 34 p. cent; Dopamine increased systolic arterial pressure by 19 p. cent while pulmonary wedge pressure remained unchanged. Subendocardial viability ratio decreased by 25 p. cent with Dopamine (p less than 0,01) but only by 14 p. cent with Dobutamine (p less than 0,01). Thus, there was significantly less impairment of subendocardial perfusion with Dobutamine and its use is preferable when inotropic support of the failing left ventricle is necessary in the acute stage of myocardial infarction.

[Primary sclerosing cholangitis (author's transl)].

Two case of Primary Sclerosing Cholangitis (PSC) associated with inflammatory bowel disease are reported. In one of these cases, inflammatory bowel disease was asymptomatic and discovered only by performing colonoscopy. Endoscopic retrograde cholangiography is of particular value, showing the specific lesions of PSC. Treatment is only symptomatic.
