Undifferentiated leukemia following treatment of gastric cancer with 5 fluorouracil, doxorubicin, mitomycin C chemotherapy.

Secondary leukemias are an increasingly well recognized complication of cancer chemotherapy. We report on the development of undifferentiated leukemia in a forty one year lady, eighteen months after treatment of gastric cancer with 5-fluorouracil (5 FU), doxorubicin and mitomycin C. She developed progressive weakness and pancytopenia over a period of few months. Leukemic blasts which were undifferentiated by immunophenotyping were seen in her peripheral blood. She died of intracranial haemorrhage during induction chemotherapy for the leukemia. Undifferentiated leukemia is a rare complication of FAM chemotherapy for gastric cancer.

Effect of dentin desensitizers and dentin bonding agents on dentin permeability.

PURPOSE: To determine the effect of dentin desensitizing agents (oxalates and glutaraldehyde) and dentin bonding agents, with and without resin-based composite (RBC) on dentin permeability and morphology of the dentin surface. The influence of saliva and toothbrushing on these agents was also evaluated. MATERIALS AND METHODS: Radicular dentin discs +/-1 mm thick were obtained from extracted human teeth. The thickness of the discs was standardized by grinding them with 600 grit silicon carbide powder. Both pulpal and periodontal surfaces of the discs were treated with 6% citric acid for 2 min prior to the baseline measurement of dentin permeability. Permeability was measured for the discs, using a split-chamber apparatus, at four different time points: at baseline, after treatment with each desensitizing modality, after 24 hrs of saliva immersion and after simulated toothbrushing equivalent to 3 weeks of normal brushing. The treatment modalities were: Sensodyne Dentin Desensitizer, Gluma Dentin Desensitizer, All-Bond DS (primers without etching), Etch+Primers (All-Bond 2 system), Etch+Primers+Adhesive and Etch+Primers+Adhesive+RBC (AElite Flo). Radicular dentin discs were prepared separately for scanning electron microscopy (SEM). One group was left untreated as the control group. Discs in the other groups were treated with the modalities listed above. One disc from each group was viewed under the SEM after treatment with the desensitizing modality. The second disc in each group was viewed after 24 hrs of saliva immersion following desensitizing treatment and the third disc after 3 weeks of simulated toothbrushing following desensitizing treatment and saliva immersion. RESULTS: The greatest reduction in permeability was caused by All-Bond DS followed by Sensodyne Dentin Desensitizer and Etch+Primers+Adhesive+RBC. Gluma Dentin Desensitizer, Etch+Primers, and Etch+Primers+Adhesive did not reduce permeability by a significant amount. Saliva immersion increased permeability significantly only for the Gluma and Etch+Primers+Adhesive groups. There was a trend towards increase in permeability after toothbrushing; however, this increase was not statistically significant for any group (P > 0.05). Thus, placement of primers without etching (All-Bond DS) reduced permeability more than any other treatment modality. These data indicate that etching a sensitive dentin area may be appropriate only if RBC is to be placed in the area. Exposure to saliva and toothbrushing countered the decrease in permeability caused by dedicated desensitizers and dentin bonding agents. The SEM micrographs revealed that in most cases, form corroborated function and the morphology of the dentin surface corresponded to the dentin permeability values.

Microleakage of Class II resin-based composite restorations with flowable composite in the proximal box.

PURPOSE: To compare microleakage of Class II hybrid resin-based composite (RBC) restorations with and without low viscosity (flowable) RBC in the proximal box. MATERIALS AND METHODS: Conservative Class II cavity preparations were made in 50 extracted premolars randomly assigned to five groups (n = 10). OptiBond FL dentin bonding agent, and Prodigy hybrid RBC were used to restore all cavity preparations. Preparations in four of the five groups were restored with flowable RBC as the initial increment in the proximal box. The four flowable RBCs tested were: AEliteFlo, Revolution, UltraSeal XT and Flow-It. In the control group, the entire preparation was restored only with hybrid RBC. Specimens were stored in water at 37 degrees C for 24 hrs after restoration, thermocycled 1,000x, immersed in 2% basic fuchsin, sectioned and evaluated for microleakage. Microleakage was scored on a 0-4 scale. RESULTS: Non-parametric Kruskal-Wallis one-way ANOVA followed by Wilcoxon signed ranks test was used for statistical analysis. The use of any of the four flowable RBCs tested did not influence microleakage significantly (P > 0.05) in Class II RBC restorations.

Cardiac involvement in limited systemic sclerosis: non-invasive assessment in asymptomatic patients.

Nineteen patients with limited systemic sclerosis (SSc) and without any cardiac symptoms were evaluated non-invasively for silent cardiac involvement using electrocardiography; M-mode, two-dimensional and Doppler echocardiography, and resting and post exercise radionuclide ventriculography. Left anterior hemiblock and mild pericardial effusion were seen in two patients. The interventricular septal thickness and left ventricular posterior wall thickness were greater in patients compared with controls. Patients with limited SSc also exhibited low early diastolic filling velocities and a low early diastolic atrial filling ratio. However, these values did not correlate with the age of the patient or disease duration. Eight patients (42.1%) were found to have cardiac dysfunction, of which four had combined systolic and diastolic dysfunction. Isolated systolic and diastolic dysfunction was seen in three patients and one patient, respectively. The age and disease duration in patients with cardiac dysfunction did not differ from patients without cardiac dysfunction. Functional cardiac involvement occurs in a large proportion (42.1%) of patients with limited SSc, and can be easily picked up by non-invasive methods such as echocardiography and radionuclide ventriculography. The prognostic significance of these findings requires further long term studies.

Radiation safety for health care workers in the bronchoscopy suite.

Increased use of fluoroscopy during flexible bronchoscopy has raised concerns about radiation safety of health care workers in the bronchoscopy suite. We review the potential health risks associated with occupational radiation exposure, the monitoring devices available, and discuss the measures to reduce radiation exposure during flexible bronchoscopy.

Occurrence and significance of decahydroquinolines from dendrobatid poison frogs and a myrmicine ant: use of 1H and 13C NMR in their conformational analysis

Structures for 2,5-disubstituted decahydroquinolines (DHQs) are reported for the two diastereomeric pairs cis-275B (14) and cis-275B' (15) and 5-epi-trans-269AB (18) and trans-269AB (19), all isolated from skin extracts of dendrobatid frogs, and for 5-epi-cis-275B' (16) and 5-epi-trans-275B (17) found in the extracts of virgin queens of a myrmicine ant [Solenopsis (Diplorhoptrum) azteca]. Detection of such DHQs in an ant, their first reported occurrence, strengthens a dietary hypothesis for the origin of the approximately 30 DHQs that have been detected in extracts of frog skin. NMR data on the two conformers of cis-decahydroquinoline permit assignment of ring conformations and stereochemistry to cis-DHQs of the "N-endo" type or the "N-exo" type. These conformations are also assigned on whether H-8a is equatorial or axial as determined with E-COSY or 1D-HOHAHA spectra.

Extensive facial damage caused by a blast injury arising from a 6 volt lead accumulator.

Low-voltage electrical injuries are relatively uncommon. Injury caused by flow of heavy current due to short-circuiting a low-voltage battery has not been described in the English literature. A 9-year-old boy connected two thin household electrical wires to the two terminals of a 6 volt (lead accumulator) battery and pressed the other two ends between his teeth. This resulted in a blast causing a compound comminuted fracture of the mandible and extensive tissue damage in the oral cavity. The low internal resistance of a lead accumulator (approximately 0.03 ohms) permits the flow of a heavy current (approximately 200 amps) when short-circuited. This instantaneously vaporises a minuscule portion of wire at approximately 2000 K resulting in a sudden rise of intraoral pressure to 30 kg cm-2 leading to tissue damage.

Utility of transbronchial needle aspiration in the diagnosis of endobronchial lesions.

BACKGROUND: The utility of transbronchial needle aspiration (TBNA) in visible endobronchial lesions presenting as either an exophytic mass lesion (EML) or submucosal and peribronchial disease (SPD) is not well established. OBJECTIVE: To compare the yield of conventional diagnostic procedures (CDP) (bronchial washing, bronchial brushing, and endobronchial forceps biopsy) with that obtained from a combination of CDP and TBNA (CDP + TBNA). DESIGN: Prospective study of 55 patients. SETTING: Tertiary-care referral hospital. RESULTS: Of the 55 patients in whom malignancy was confirmed, CDP + TBNA identified 53 (96%) vs 42 (76%) identified by CDP (p = 0.001). The highest yield from any individual procedure was obtained by TBNA. Of the 23 patients with SPD, 22 (96%) were diagnosed using CDP + TBNA compared with 15 (65%) by CDP (p = 0.016); the yield from TBNA alone (22 of 23) in this group surpassed the combined yield from all other procedures. Although no statistically significant difference in yield was observed for EML, the use of TBNA identified four additional patients compared with CDP. CONCLUSION: We conclude that the addition of TBNA to CDP increases diagnostic yield in patients with visible endobronchial lesions.

Renal lesions associated with AIDS--an autopsy study.

Kidneys from 55 cases (20 with HIV infection and 35 with AIDS) were studied by routine Haematoxylin and Eosin stains and special stains (PAS, PASM GMS, ZN, Mucicarmine and Congo red) to evaluate, glomerular, interstitial and vascular pathology. Twenty-four of the 35 (68.6%) cases of AIDS showed infective aetiology which included 17 cases (48.5%) of tuberculosis, 5 cases (14.4%) of fungal infection (3 cryptococcus neoformans and 2 candida species) and 2 cases (5.7%) of CMV infection. Other lesions noted were amyloidosis and tubular calcinosis. HIV associated nephropathy (HIVAN) was not detected in any of the cases. Intravenous drug abuse was not a risk factor in our cases which probably explains the absence of HIV associated nephropathy in the present study.

Alterations in cardiac gene expression during ventricular remodeling following experimental myocardial infarction.

Following myocardial Infarction (MI) the heart undergoes a process of remodeling characterized by considerable hypertrophy of the non-infarcted myocardium. We have recently characterized the molecular basis of key electrophysiologic alterations that may provide insight into the arrhythmogenecity of post-MI remodeled hypertrophied myocardium. To further characterize other key alterations in the pattern of cardiac gene expression in a time-dependent manner, we have measured mRNA and immunoreactive protein levels of selective cardiac genes in the remodeled hypertrophied left-ventricular (LV) myocardium of rats, 3 and 21 days after left-coronary ligation and compared the results with sham-operated rats. RNase protection assay was performed to assess the expression of c-fos, atrial natriuretic factor (ANF), brain natriuretic factor (BNF), alpha2/3 isoform of Na-K ATPase, cardiac alpha/beta isoform of myosin heavy chain (MHC). Compared to the sham group, the expression of c-fos was increased 10-fold (P<0.02) in the MI group on day 3, but unlike other overload hypertrophy models, the expression remained elevated by three-fold on day 21. Similar to other overload models, the ANF and BNF expression increased significantly. No alterations were observed in the expression of cardiac alpha-actin. There was reexpression of the fetal isogene form of MHC and Na-K ATPase after MI. The beta-MHC mRNA levels, the fetal isoform of MHC, returned to basal levels after 21 days. After an initial five-fold decrease the adult isoform of alphaNa-K ATPase, alpha2 Na-K ATPase mRNA, returned to control levels and similar changes were seen in the corresponding protein levels. These findings indicate that during LV remodeling and hypertrophy following MI, there is an upregulation of early response genes and fetal isogene expression. The pattern of activation, however, is distinct from that observed in other overload models, indicating the possible involvement of alternate signal transduction pathways.

Managing solitary pulmonary nodules.

A solitary pulmonary nodule (SPN) on a chest radiograph represents a major diagnostic dilemma. The goals of management are to resect malignant tumors without delay and to avoid unnecessary thoracotomy if the nodule is benign. But because of the difficulty distinguishing benign from malignant nodules, even with advances in imaging techniques, these goals cannot be met in all cases.
