Sedatives used in pediatric imaging: comparison of IV pentobarbital with IV pentobarbital with midazolam added.

OBJECTIVE: This study was designed to evaluate safety, efficacy, and success of adding IV midazolam to an established IV pentobarbital protocol for pediatric sedation for radiologic imaging. Outcomes included sedation and discharge times as well as adverse events SUBJECTS AND METHODS: This prospective study compared two different sedation protocols developed by the radiology sedation committee and approved by the hospital sedation committee at our institution. Patients in the pentobarbital group received IV pentobarbital alone, and patients in the pentobarbital--midazolam group received a combination of IV pentobarbital and midazolam. A total of 1070 infants and children were enrolled, and sedation data were entered into a computer database and reviewed at bimonthly radiology sedation committee meetings for safety, efficacy, efficiency, failed sedations, and adverse outcomes. RESULTS: Mean age distribution, sex, American Society of Anesthesiologists physical status classification, fasting status, weight, and types of examinations were similarly distributed between the two study groups. Analysis of variance indicated longer times were required to sedate and to discharge patients who had received pentobarbital--midazolam (p < 0.001 for both times), even after adjusting for differences in the patients' ages and weights. The pentobarbital--midazolam group required more time to be successfully sedated and more time to discharge from the recovery room. The rates of adverse events and failed sedations were similar for both groups. CONCLUSION: Midazolam does not have a beneficial effect on pentobarbital sedation and has no effect on the rate of adverse events. The prolonged time needed both to sedate and to discharge (timed from the initial dose of sedation) pediatric patients who have received midazolam should discourage physicians from combining it with pentobarbital for pediatric sedation.

[Indications for individual child psychotherapy in a Guidance Center].

The aim of this article is to describe how we conceptualise in practical indications of individual psychotherapy in the frame of the consultation of a guidance center. We will not review the literature concerned with this subject. We shall start with the study of the following factors : parents, family, reality features (distance, time, money...). Then, we study the positive and negative features in the child. We explain first all we consider to be the main obstacles in the psychic structure of the child. Then we describe in detail the favorable elements to the psychotherapeutic work. We also recall cases who, without being impossible to treat, require a big experience from the therapist, because of complexity and demand. They are sometimes treated in our unit, according to the disponibility of the therapists. In conclusion, we indicate that the choice of psychotherapy depends not only from the sick child and his familial milieu, but also from the therapist who will involve himself in this type of treatment.

[Team and education and the Centre Universitaire de Guidance infantile at Woluwe (Brussels)].

Description of the center for child guidance at the University of Louvain where take place supervision and training role games, as described in the next three articles. It is a new center, specialised in postgraduated training for doctors and psychologists in the area of child guidance, analytical individual psychotherapy for children and family therapy. The training programmes are being described.

[Structuralizing role playing in training].

The role playing utilised in the training of members of a guidance team, especially during technical seminars, may require a structure which protects participants and liberates their personality. A first part describes the spirit and technics most used, of the type "laboratory of human relations". A second part analyses by point differences between structured role playing and therapeutical psychodrama.

Performance of a new softening expanding midline catheter in home intravenous therapy patients.

This pilot study was performed to evaluate the performance of a midline catheter as an alternative to the conventional short peripheral catheters and central venous lines used to administer intravenous therapy to the patients at Caremark Homecare before this study. The midline catheters were inserted in home care patients primarily diagnosed with osteomyelitis and infections, for the delivery of approximately 16 days of I.V. antibiotic therapy. Midline catheter performance was compared with the published data on peripheral catheters and central venous lines because there are no other midline catheters available for clinical use that could be used as a study control. Through this pilot study, the importance of adequate dilution of medications and treatment of early aseptic mechanical phlebitis were realized. Also, a variety of patient types was found to be suited to the use of a midline catheter (wide range of age, weight, venous access, and diagnoses). These findings served as a basis for the ongoing formal study. The low complication rate, insertion ease, and suitability of a wide variety of patient types makes the midline catheter a better alternative to peripheral catheters or central venous lines for intermediate-length I.V. therapies of 1 to 6 weeks.
