[Surgical treatment of injuries of the bile ducts].

The bile ducts were injured during cholecystectomy in 143 of 155 patients treated at the clinic. In most cases (78) the iatrogenic injuries occurred under conditions of a severe inflammatory and cicatricial process in the zone of the cystic neck and hepaticocholedochus. Mortality in correction of a fresh injury of the duct by the traditional methods was 11.1%. An alternative approach to improvement of the results of surgical treatment of injury to the bile ducts and its late-term sequelae may be the use of precision techniques with application of sutures of nonreactive and nonlithogenous monofilamentous suture materials.

[Cicatricial strictures of anastomosis of the hepatic duct and common bile duct].

The authors have observed 34 cases of scarry strictures of bilio-digestive anastomoses requiring repeated surgical correction. The causes of such complications, methods of clinical and special diagnostic are discussed. The technique of reoperations for such complications is described along with their results and recommendations for prevention of scarry strictures of anastomoses of the common hepatic duct.

[Pancreatitis in obstruction of Vater's ampulla].

Clinical observations of 478 patients operated for pancreatitis resulting from different obturating lesions of the Vater papilla (stones, cicatricial stenosis, polyps etc.) are analyzed. The questions of diagnosis, preoperative preparation, medical methods, technique of the operation and postoperative management are discussed. Recommendations are given for the improvement of results of the treatment and the decrease of postoperative lethality.
