[Bacillary peliosis in AIDS. Anatomo-clinical study of 2 cases].

The authors report two cases of peliosis hepatis, occurring in patients with AIDS, who presented a persistent fever and an hepatomegaly. The liver biopsies showed areas of peliosis, where bacilli were observed by Warthin-Starry stain. In one case, techniques of molecular biology allowed the identification of Rochalimaea henselae, pathogen involved in bacillary angiomatosis. This rickettsia has been newly recognized in the United-States, where 17 cases of bacillary peliosis have been published in immunocompromised hosts and mainly in patients with AIDS. These observations illustrate the clinical and histological features of this new opportunistic infection, as it is described in the literature. The clinical signs include an unexplained fever, an hepatomegaly, and in 75% of the cases, a splenomegaly. The cutaneous lesions of bacillary angiomatosis are associated in 40% of the cases. An antibiotic treatment by erythromycin ensures a complete recovery.

[Leiomyosarcoma of the liver: a case treated by transplantation].

A 68 year old woman with primary hepatic leiomyosarcoma, impairement of general status and right upper quadrant pain was diagnosed with liver tumor by sonography, CT scan and nuclear magnetic resonance imaging. Angiograms showed a hypervascular tumor. The patient was treated by hepatic transplantation but died 15 days after operation. The diagnosis of leiomyosarcoma was established by immunohistochemical and ultrastructural studies; the primary nature was confirmed by necropsy. Diagnostic and therapeutic features of primary hepatic leiomyosarcoma--the 19th case reported in the literature--are discussed.

[Primary lymphoma of the cranial vault. Apropos of a case].

Primary lymphomas of the cranial vault are most unusual lesions. Only six cases have been described in the literature. We report a case of a 71 year old female who was admitted with an occipital mass. A C.T. scan revealed a mainly extra cranial tumor with an osteolysis of the cranial vault and a small intracranial component. It was diagnosed as a phenotype B centroblastic lymphoma. Half of the patients reported in the literature had a subcutaneous mass as the main symptom. These neoplasms are usually highly malignant tumors. The treatment realized is neurosurgical excision followed by chemotherapy and radiotherapy.

[Cytological study of the duodenal mucosa during chronic diarrhea in HIV-positive patients].

The cytologic findings of duodenal smears in diarrheic HIV-positive patients were compared with results of histologic and ultrastructural studies. This study included 50 diarrheic patients undergoing upper gastrointestinal endoscopy. Duodenal biopsies were taken with touch preparations. Smears were stained with Giemsa and then with Ziehl-Neelsen or the PAS method. The biopsy specimens were then processed in a standard fashion for histologic and electron microscopy examination. Cytologic findings included 8 cases of intestinal microsporidiosis, 4 cases of cryptosporidiosis, and 3 cases of atypical mycobacteriosis. Histologic and ultrastructural studies confirmed these diagnoses and showed 8 cases of CMV enteritis, of which 4 were associated with other enteric pathogens. Duodenal cytology complements endoscopic biopsy in the work-up of diarrhea in AIDS patients and may provide a rapid diagnosis especially in the case of intestinal protozoonosis or atypical intestinal mycobacteriosis.

[Rhino-orbito-cerebral mucormycosis caused by Rhizopus oryzae.A typical case in a cirrhotic patient].

The rhino-orbito-cerebral mucormycosis is an acute rapidly progressive fungal infection. This disease is caused by a zygomycetes fungus, most often from the Rhizopus genus. This fungus is saprophitic of the nasal cavity and paranasal sinuses. It becomes pathogenic in some particular conditions, specially during diabetes mellitus. Histopathological study is the only method allowing the diagnosis, by revealing the tissue invasion by characteristic hyphae. Mycologic study allows a definitive identification of the fungus. The authors report a case of rhino-orbito-cerebral mucormycosis in a 44 year-old woman with cirrhosis. She presented an acute blindness and ophthalmoplegia. Despite of a rapid histologic diagnosis from the nasal and ethmoidal biopsies, the patient died 3 days after.

[Cutaneous hemangioma associated with eosinophilia].

A patient with a cutaneous angioma developed an intravascular coagulation syndrome with a eosinophilic leukocytosis and multiple arterial and venous thromboses affecting the vital prognosis. The possible role of the eosinophilia in the mechanism of complications is discussed. Local modifications in the angioma are suggestive signs preceding the onset of general thrombotic complications, their severity contrasting with the simplicity of the secondary surgical excision of the angioma.

[Pseudo-tumoral circumscribed myositis ossificans of the thoracic wall. Contribution of ultrasonography, x-ray computed tomography and MRI].

The authors report the main clinical and radiographic aspects of a circumscribed myositis ossificans in a 5 year old child located in the thoracic wall, near the sternum, stimulating a malignant tumor. CT scan was more contributive than MRI, both for diagnosis and survey, because it shows better the progressive peripheral calcification typical of the lesion.

Primary gastrointestinal malignant lymphomas associated with Epstein-Barr virus after heart transplantation.

Gastrointestinal involvement has been reported in 12-21% of post-transplant lymphoproliferative disorders but is unusual in the setting of heart transplantation. We report four post-transplant lymphoproliferative disorders observed among the 174 heart transplant recipients of our series, all of which were primary malignant lymphomas of and confined to the digestive tract. The mean onset time from transplantation was 22 months. Small intestine lesions were present in all four patients, with gastric involvement in one. Histologically, the tumour was monomorphic of immunoblastic type in one case and polymorphic in the three other cases. Analysis of cytoplasmic immunoglobulins demonstrated the presence of a major monoclonal subset in all patients. Epstein-Barr virus genome was found in numerous tumour cells by in situ hybridization. The exclusive localization to the digestive tract and the lymphoepithelial lesions observed in two cases suggest that these lymphoproliferations might originate from mucosa-associated lymphoid tissue.

Primary uveal B immunoblastic lymphoma in a patient with AIDS.

A case of primary intraocular malignant lymphoma without cerebral involvement is reported in a 30-year-old man with acquired immunodeficiency syndrome. The study of the enucleation specimen showed a B immunoblastic lymphoma with a CD30 positive anaplastic large cell component. There was no involvement of the adnexal structures of the orbit. The patient subsequently completed non-surgical staging showing no extension of the tumour. The clinical course was rapidly fatal with dissemination to the pericardium and pleura.

A standardized multifield irradiation technique for breast tumours using asymmetrical collimators and beam angulation.

Combined angulation and asymmetrical collimator matching techniques are used for field alignment in breast irradiation. A local regional irradiation technique has been developed that allows a uniform dose distribution to the entire target volume. The localization is characterized by simple measurement of a few patient co-ordinates. The equipment settings to produce a theoretical exact field alignment are calculated by a special computer program. These settings guide the simulation. We have designed the method such that the technique is standardized for all patients receiving breast and lymph node irradiation. This allows an easy, accurate and fast patient throughput at the simulator and accelerators.

[Rabies encephalomyelitis with myocarditis and pancreatitis. Report on a case recently imported into France].

We report the case of a 29 year-old man who died from rabies in France, following a dog-bite during a trip in Mexico. Although it was clinically suspected, the diagnosis was uncertain until he died because of digestive, cardiac and psychiatric misleading symptoms associated to the neurologic disorders. Post mortem diagnosis was based upon virological study in immunofluorescence on cerebral smears, viral isolation on cell-culture, and ELISA. It was confirmed by light microscopy examination which showed numerous Negri bodies, and ultrastructural study of the rhabdovirus in the central nervous system. Extranervous lesions, especially myocarditis and pancreatitis, were observed and their meaning is discussed. The physician is exceptionally confronted to the diagnosis of human rabies in France. Nevertheless, the lack of compulsory antirabic vaccination and the increase of touring in enzootic countries increase the risk of infection. As an intra vitam diagnosis in frequently lacking, the diagnosis of rabies infection needs a complete post mortem virological study as well as an histological and ultrastructural examination of the central nervous system.

[Orbital apex syndrome disclosing naso-oculo-cerebral zygomycosis].

In a 70-year-old male patient with untreated diabetes a febrile orbital apex syndrome of rapid onset revealed a rhino-orbito-cerebral zygomycosis. Biopsies of the ethmoidal mucosa showed numerous colonies of the Rhizopus genus. Despite medical treatment (amphotericin B) and surgery (ethmoidectomy), the patient died within one week. Rhino-cerebral zygomycosis is a rare disease which occurs in diabetic and immunocompromised patients.

[Cholesterol emboli with neurologic manifestation in the spinal cord].

A 71-year old man presented with a progressive chronic paraparesis combined with inflammatory biological features including hypereosinophilia and an aneurysm of the abdominal aorta. Disseminated cholesterol embolization of arterioles was evidenced by the identification of cholesterol crystals in biopsies of the quadriceps muscle and of an iliac lymph node. Despite the lack of post mortem study of the spinal cord, the presentation was highly suggestive of cholesterol emboli in the spinal arteries. Only ten documented cases have been reported.

[Prevention of scar formation by polyglactin 910 (Vicryl) mesh after lumbar laminectomy in the rat].

Laminectomy in lumbar spine surgery often produces disabling adhesive arachnoiditis. We have tested, on the rat, polyglactin 910 mesh with collagen or not, with a mail 0.9 mm large. We have used 34 Wistar male rats. L5 laminectomy were realised on all the rats. Rats were ranged in four groups: group 1: control, L5 laminectomy alone, 6 months follow up--group 2: one month follow up, L5 laminectomy, vicryl mesh with collagen or not--group 3: two months follow up, L5 laminectomy, vicryl mesh with collagen or not--group 4: six months follow up, L5 laminectomy, vicryl mesh with collagen or not. On 28 rats, the histologic study have demonstrated: group 1 control: important and adhesive fibrosis scar--group 2: no or minimal reaction--group 3: fibroblastic or few cellular, non or little adhesive fibrosis--group 4: moderate adhesive fibrosis in half of the cases with vicryl mesh without collagen, minimal non adhesive fibrosis with collagen vicryl mesh.

[Histopathologic features of opportunistic infections of the small intestine in acquired immunodeficiency syndrome].

The frequency of the opportunistic infections of the duodenum in AIDS patients was determined by way of histologic study in 207 patients between January 1987 and June 1991. All cases had serial paraffin sections, run through HES, PAS, Giemsa, Brown-Brenn, and Zieh-Neelsen stains, and 20 cases had in addition cytologic and electron microscopic study. 63 patients had opportunistic infections (10 cryptosporidiosis and 2 isosporiasis; 12 mycobacterial enteritis; 15 CMV enteritis; 7 candidosis; 7 intestinal microsporidiosis confirmed by electron microscopic examination; 12 Giardiasis; 3 duodenal leishmaniasis; 1 intestinal cryptococcosis). Multiple concurrent infections were noted in 6 cases. A mild to severe villous atrophy was observed in 28 cases, associated with opportunistic infection. A patchy distribution of pathogen agent was noted in 34 cases, and 37 cases were associated with oesophagal candidosis. This study points out the value of histologic examination of intestinal biopsy for the diagnosis of systemic infections as well as of unusual parasitosis, and the necessity for multiple endoscopic biopsies because of the frequent patchy distribution of pathogens.
