[Analysis and criticism of hemodynamic investigations during respiratory analeptic dispensing (author's transl)].

Almitrine is a respiratory analeptic acting on peripherical chemoreceptors. The hemodynamic effects have been studied with chronic respiratory diseases. The works have shown that almitrine produces an increase of pulmonary arterial pressure due to the raising of pulmonary vascular resistances by stimulation of peripherical chemoreceptors. Studying again the hemodynamic investigations with chronical respiratory failure it has been interesting to consider the effects of the produce with patients whose gazometric parameters have been normalized by artificial ventilation at FiO2 = 1, so as to produce a saturation of peripherical chemoreceptors. The results show that infused almitrine keeps hypertensive properties on pulmonary arterial with raising of pulmonary arterial resistances; the pulmonary vasoconstrictor effect is kept in spite of a normalization of blood gases. So it is necessary to look after pulmonary arterial pressure of patients receiving almitrine, that whatever the pre-existing pulmonary arterial pressure may be.

[Changes in erythrocyte lipids in major septic states].

The authors studied changes in erythrocyte lipids in severe infectious syndromes, separating healthy individuals (n = 10) from those with hepatic insufficiency (n = 7). No variations in sphingomyelins were seen. By contrast, the fall in cholesterol and other phospholipids studied (phosphatidylcholine and phosphatidylethanolamine) was highly significant. The difference between the two groups remained clear on the fourth day. No relationship could be established with prognosis and the organism responsible. In conclusion, the authors stress the need for study of the relationships between these changes and erythrocyte functions.

[Respiratory failure during Ogilvie's syndrom (author's transl)].

About two cases of "Ogilvie's syndrom" so called colonic ileus or acute idiopathic dilatation of the colon, in patients with chronic bronchitis, the authors discuss the possibility of this etiology like respiratory failure. A rapid colonic decompression is necessary to prevent cecal perforation.

[Changes in erythrocyte lipids in severe septic states].

In cases of major sepsis the erythrocyte lipids undergo significant change especially when there is concomitant hepatic insufficiency and this in turn lead to alteration in the blood viscosity and very probably to the genesis of disorders of coagulation.

[Study of erythrocyte lipids after perfusion of modified fluid gelatin].

Study of changes in erythrocyte lipids after the infusion of 1,000 ml of modified fluid gelatin (Plasmion) demonstrated disturbances at the level of the lipid double layer with, in particular, a decrease in phosphatidyl-ethanolamines found in the serum. No erythrocyte deformation was seen.

[Validity of a teaching method of arrhythmias with a simulating machine (author's transl)].

The authors have tested a method of teaching the arrhythmias with a simulating machine. Two groups of students have been drawn by lot and have set for a first test on their knowledge about dysrythmias. One group was instructed in a classical way, the second one was instructed with the simulating machine which reproduces the majority of rhythm modifications on electrocardioscopic screen. At the end of the teaching, a post-test took place, then a last one a month later. The comparison of the two methods by measuring the residual knowledge enables to prove the interest of using simulating machine to teach disorders of cardiac rhythm.

Immune activation during cerebellar dysfunction following Plasmodium falciparum malaria.

Evidence for immune activation was investigated in 12 patients with a rare syndrome of self-limiting, delayed onset cerebellar dysfunction following an attack of falciparum malaria which occurred 18-26 d previously. Concentrations of tumour necrosis factor, interleukin 6 and interleukin 2 were all significantly higher in serum samples of patients during cerebellar ataxia than in recovery sera and in the sera of 8 patients who did not develop delayed cerebellar dysfunction following an attack of falciparum malaria. Cytokine concentrations in the cerebrospinal fluid were also significantly higher in ataxic patients than in controls. These findings suggest that immunological mechanisms may play a role in delayed cerebellar dysfunction following falciparum malaria.

MRI of complicated anal fistulae: comparison with digital examination.

OBJECTIVES: The assessment of complicated anal fistulae is based on digital examination. Our objective was to study the potential role of MRI in this setting by comparing preoperative digital examination with MRI. MATERIALS AND METHODS: Digital examination and MRI were prospectively and blindly interpreted in 13 patients with complicated anal fistulae. Surgical findings were used as standard. RESULTS: Complete agreement between the results of digital examination and the surgical findings was observed in 8 patients and between the results of MRI and the surgical findings in 12 patients. Digital examination misclassified two low transsphincteric fistulae as high transsphincteric and missed high secondary extensions or abscesses in three other patients. Magnetic resonance misclassified only one suprasphincteric fistula as low transsphincteric and did not overlook high secondary extensions or abscesses. CONCLUSION: These findings suggest that MRI may show abnormalities not detected by preoperative digital examination.

Endoscopic treatment of the carpal tunnel syndrome: review of the recent literature.

The carpal tunnel syndrome is a common pathology, recognized since one and half century. Its surgical treatment, median nerve neurolysis, has only been popularized 30 years ago. Despite improvement of the surgical technique, two main problems remain unsolved: lack of grip strength and "pillar" pain, which can last several weeks after surgery. Endoscopy, in a same way as knee meniscectomy or cholecystectomy, has contributed to minimize drawbacks of conventional surgery. Ten years of experience have improved the material and the technique, the most recent series reporting very low complication rates. In experimented hands, and with correct indications, endoscopic treatment of carpal tunnel syndrome offers a clinical benefit for the patient and an economical one for the Society.

[Post-operative necrotising enterocolitis. One case cured by hyperbaric oxygenotherapy (author's transl)].

About one case of necrotising enterocolitis following an extra-peritoneal and aseptic operation, it seems that a post-operative ileus, by the ischemia of mucous membrane and the swarming of anerobics that it involves, seems to be responsable of this dreadful complication. The macroscopic and deceiver aspect of intestine to an early re-operation can impeed this diagnosis. The hyperbaric oxygenotherapy seems to be a perceptible adjuvant treatment.
