[Malignant sacrococcygeal teratoma in an adult].

Malignant sacrococcygeal teratoma is rare in adults. diagnosis is difficult when the tumour is presacral located. A 42-year-old female patient underwent rectum extirpation with resection of a malignant sacrococcygeal teratoma. Histological biopsy and clinical staging did not show whether the teratomas were malignant or not. These tumours have to be resected totally, when necessary together organs of the pelvic region.

[Reimplantation of splenic tissue after neonatal abdominal trauma (author's transl)].

Loss of the spleen in children frequently results in overwhelming infection caused by pneumococci. In order to preserve splenic function the reimplantation of splenic tissue into the abdominal wall and the greater omentum was performed in a newborn child and six other children up to the age of thirteen. Postoperatively the complete absence of Jolly's bodies could be observed, while a continuous increase of IgM was noted. The otherwise mandatory antibiotic prophylaxis over 2 years could be reduced to 5 months in the newborn.

[Complications following thoracic surgery (author's transl)].

From 1968--1977 a total of 2,670 patients with thoracic injury or disease were treated in the Department of General and Thoracic Surgery at the University of Giessen. Of these, 1,185 patients were operated on: 701 lung resections, 371 thoracotomies, and 113 operations on the diaphragm and thoracic wall. Postoperative complications were observed in 16% and 8% died. Frequency and severity of the complications were dependent on the type of surgery and the underlying disease.

[Investigations of stability with the fixateur externe during strain of bending. Vertical and eccentric strain during compression of the fracture line and while the distance of support is changed (author's transl)].

The Fixateur externe was investigated under vertical and eccentric load when the fracture line was compressed and the distance of support was changed. Three kinds of mounting were used: R (Fixateur externe as frame), Z (three dimensional Fixateur externe) and Z2S (three dimensional Fixateur externe with two screws according to Schanz). If the distance of outer support was increased, the stability became greatly reduced. A compression increased the stability around the fracture line and on the free end of the system. A compression force of 10 kp at the fracture line yielded the greatest increase in stability. If in case of the methods R and Z, the compression was increased beyond 70 kp, there resulted even an instability, which depended on the distance of support. The most stable method of mounting was the three-dimensional Fixateur externe with two screws according to Schanz. The stability was compared if there was either a defect or a compression at the line of the fracture: there resulted a strikingly increased instability, if the distance of support was increased. The compression made it possible to compensate partly for the loss of stability.

[Somatostatin in small intestine fistulas].

Somatostatin (GIF) was administered to a patient with persisting intestinal fistulas. Somatostatin reduced the tryptical activity of the secretions which subsequently subsided.

[Surgical treatment of lung abnormalities].

Four cases of lung malformations are presented. In this cases symptoms, diagnostic procedures, indications for operation and surgical treatment are discussed. Agenesis and aplasia of lungs do not require surgical treatment. Hypoplasia of the lung, an accessory lung and sequestrations leading to purulent-recurrent infections can be treated by resections.

Isolated extremity perfusion with DTIC. An experimental and clinical study.

Dacarbazine (DTIC) was used for isolated perfusion of extremities in dogs and man. In the animal experiment perfusions with DTIC at dosages up to 100 mg per kg of extremity weight were well tolerated. The concentration of DTIC in the perfusate ranged from 70 to 400 micrograms/ml without evidence for formation of metabolites. Electron microscopy, performed 14 days later, revealed a decrease of glycogen in striated muscle cells. Vascular damage was not observed. Five patients with advanced malignant melanoma or soft tissue sarcoma of the extremities were treated by isolation perfusion with 75 to 133 mg DTIC per kg of extremity at 40 degrees C for 60 minutes. A tumor regression of at least 30% was observed.

Levels of Cis-Platinum in hyperthermic isolated perfusion.

The Cis-platinum levels in isolated extremity perfusion were measured in dogs and man. We found that prolonged presence of Cis-platinum in the tissue is attributed to its strong and rapid binding to serum and tissue proteins. Protein binding follows the concentration gradient. Subsequent re-diffusion into the blood followed by urinary excretion was also noted.
