Relationship between perceived problem-solving and career maturity: implications for minority and disadvantaged premedical students.

This study examined the relationship between career maturity of minority and disadvantaged premedical students and their self-appraised problem-solving. 83 students enrolled in a special program took the Medical Career Development Inventory and the Problem-solving Inventory one week apart. Pearson product-moment correlations and a stepwise multiple regression analysis indicated a significant relationship between scores for the students' appraisal of their problem-solving and their career maturity. Implications for intervention strategies for the present population are discussed.

[Diabetic coronary disease and risk of myocardial infarction].

The raised incidence of myocardial infarction and sudden death in diabetics was explained over 20 years ago by the increase in prevalence and severity of coronary atherosclerosis. Coronary angiographies of 820 consecutive patients admitted to hospital for coronary artery disease were analysed. One hundred and fifteen of these patients (14%) were diabetic. In 77.4% of diabetics and 72.6% of non-diabetics, coronary angiography showed coronary lesions which were usually distal and three vessel in diabetics. When the three coronary arteries were analysed globally, the percentage of patients with at least one mild stenosis was much higher in diabetics than in non-diabetics (50.6% vs 30.3% respectively, p < 0.001), but there was no statistical difference in the number of severe stenoses or occlusions. The increase in mild stenosis in the diabetic population could indicate an anatomical predisposition to future rupture of a plaque. This result could explain the increased frequency of myocardial infarction and sudden death in diabetes, not necessarily preceded clinically by angina pectoris.

[Importance and limitations of the validation of functional MRI of motor function and language using preoperative cortical stimulation].

UNLABELLED: This chapter describes and discusses the value of the localization of functional areas obtained from functional MRI in brain tumor cases. Correlation method is cortical brain mapping by intraoperative stimulation. The experience reported here is focused on the study of motricity and language. METHODS: Twenty two patients with tumors of the rolandic region (n = 16) or in the temporal lobe (n = 6) underwent functional MR mapping and subsequently cortical mapping before tumor resection. The tasks chosen were a flexion and extension of the fingers or a naming task. We used 3D reconstructed images of the surface of the brain to assess intra and post operatively the functional MRI and stimulation data. RESULTS: For the motor correlation, in each case, the results of direct cortical mapping matched those obtained with functional MRI, both positively and negatively, although the extent of the functional activations was larger than the area required to elicit the corresponding movement during intraoperative brain mapping. For the language correlation and for the task chosen, only the results of the precentral areas matched those of functional MRI. CONCLUSIONS: Functional MRI can be used preoperatively to assess motor functional area in patients with rolandic tumors. More studies are needed to validate intraoperatively the language areas and the real extent of functional MRI activations. Finally, the observed discrepancy between functional MRI and cortical stimulation is likely due to the rather profound differences between both techniques, in terms of neurophysiology, practical applications and statistical analysis.

[Somatosensory evoked potentials during surgery of scoliosis: significance of epidural recording].

PURPOSE OF THE STUDY: To evaluate the usefulness of the epidural recording in scoliosis surgery in children or young adults. MATERIAL AND METHOD: We used somatosensory-evoked potentials in 60 children or young adults in surgical treatment of scoliosis. Somatosensory-evoked potentials were elicited and recorded using a standard method recording of cortical P40 and subcortical P27, simultaneously with the monitoring of spinal cord function using epidural electrode to record the ascending somatosensory volley (by tibial nerve stimulation) at a high thoracic level. RESULTS: The use of subcortical and epidural recording sites for the somatosensory-evoked potentials indicated that a reliable response could be obtained in 96 p. 100 of the patients. Cortical somatosensory-evoked demonstrated a 48 p. 100 false positive rate. DISCUSSION: Epidural recording in scoliosis surgery is safe and reliable. Combined subcortical and epidural recordings are especially useful in patients with neuromuscular scoliosis or with cerebral palsy that may not have reliable cortical potentials.

Use of anti-GP IIb-IIIa in acute thrombosis after intracoronary stent implantation.

Acute occlusion of coronary stents still occurs in 0.5-2% of patients. The usefulness of GP IIb-IIIa receptor inhibitors has never been evaluated in this indication. After 1,454 stent implantations, acute occlusion occurred in 16 patients. Direct percutaneous transluminal coronary angioplasty (PTCA) was immediately performed. In eight patients, no recurrent thrombosis occurred during the 15 min following PTCA, and abciximab infusion was started after this period. In six patients, immediate recurrent thrombosis occurred in the stent. In these cases, an intravenous bolus of abciximab followed by a new inflation at low pressure was performed. Fifteen min after the bolus, stable TIMI 3 flow was restored in all six cases, and no thrombus or haziness remained. In two patients, a TIMI 0 flow persisted despite PTCA and the use of a bolus of abciximab. No recurrent ischemic symptoms were observed before hospital discharge. Abciximab in combination with balloon angioplasty can be used safely to control acute thrombosis after stent deployment.

Percutaneous transluminal angioplasty of radial artery grafts.

The radial artery is being used with increasing frequency to replace the saphenous vein as a coronary artery bypass graft, in the belief that it will provide improved long-term patency. Several centers have confirmed that the early results of surgery using the radial artery seem to be better than those obtained with saphenous grafts. Despite these apparent gains, early failure of the radial artery graft can occur and is frequently associated with symptomatic myocardial ischemia. Percutaneous angioplasty is an alternative to reoperation to treat lesions occurring on radial artery grafts. We report on 4 patients who underwent angioplasty of radial artery grafts.

Comparative view of the socioeconomic impact of migraine versus low back pain.

The burden of migraine in terms of cost and impact on socioeconomic indicators is still controversial. In a recent comparative study between migraineurs and controls from the French general population, we show that only general practitioner (GP) consultations and complementary examinations are more frequent in migraineurs. In this paper, we compare the socioeconomic impact of migraine versus another common neurological disease, low back pain, which has similar consequences in term of deficiencies, disabilities, and handicaps. Our study is a subproject of the Gazel cohort study, conducted on 20,000 volunteers working in the "Electricité et Gaz de France" company. The socioeconomic impact was evaluated prospectively by the number of workdays missed between 1989 and 1992 in 436 subjects with migraine but without low back pain (M group), 590 subjects with low back pain but without migraine (L group), 555 subjects with migraine and low back pain (ML group), and in 1005 subjects without headache or low back pain (C group). Moreover, in 1993 all subjects completed a mailed questionnaire on their 6-months' history of use of medical services. The number of workdays missed during this 4-year period was statistically greater in the ML group (58.1 days), followed by the L group (38.4 days), the C group (35.1 days), and the M group (31.8 days) (p = 0.0001). For the use of medical services, the results were different according to the different indicators: GP consultations were more frequent in the ML and M groups, specialist consultations and complementary examinations in the L and C groups. In conclusion, migraine and low back pain seem to have a similar socioeconomic impact. Absenteeism is particularly high when both neurological disorders are present.

[Left atrial myxoma and coronary embolism].

The authors report the case of a 49 year old man referred for a preoperative evaluation of a left atrial myxoma. During the hospital admission, the patient had an episode of sudden anginal chest pain associated with electrocardiographic changes of anteroseptoapical myocardial infarction. Emergency coronary angiography showed occlusion of the middle segment of the left anterior descending artery whereas it had been absolutely normal on the coronary angiogramme performed a few days beforehand. The diagnosis of coronary embolism from the left atrial myxoma was made and an emergency coronary angioplasty was performed followed by surgical ablation of the tumour.

[Study of cortical atrophy with magnetic resonance imaging in corticobasal degeneration].

Corticobasal degeneration (C.B.D.) is a neurodegenerative disorder characterized mainly by an asymmetrical a kineto-rigid syndrome associated with fronto-parietal cortical signs, particularly apraxia. Conventional imaging even magnetic resonance imaging (M.R.I.) has often been considered as poorly contributive for the diagnosis of C.B.D. We retrospectively studied routinely performed M.R.I. scans of 15 patients presenting a clinical and metabolic (P.E.T/S.P.E.C.T.) syndrome characteristic of probable C.B.D. M.R.I. scans were assessed by 3 investigators, not aware of the clinically most affected side, taking into account M.R.I. technical parameters. We quantified, on each side, the cortical atrophy (frontal, parietal and temporal) and the white matter changes, by using the semi-quantified method of Victoroff et al. (1994). Abnormalities were considered if observed by at least 2 of the 3 investigators. Abnormalities were then correlated with the side initially and most severely affected. The most contributive findings were the asymmetric parietal atrophy (clinically correlated in 93 p. 100 of cases), asymmetric frontal atrophy (clinically correlated in 60 p. 100) and asymmetric dilatation of the lateral ventricles (clinically correlated in 60 p. 100). 80 p. 100 of affected subjects displayed at least 2 of these M.R.I. abnormalities. These results are in accordance with the metabolic and pathologic features of C.B.D. This study demonstrates that M.R.I. evaluation of the cortical atrophy asymmetry may contribute to the diagnosis of C.B.D.

[Fulminant orthochromatic leukodystrophy in an adult].

A 27-year-old man from Marocco developed a progressive dementia leading within a year to a mutic akinetic state. The course of the disease was also marked by epileptic seizures. MRI revealed diffuse white matter involvement. A frontal white matter brain biopsy was consistent with the diagnosis of orthochromatic leucodystrophy, i.e: presence of sudanophilic lipids and pigmented cells associated with myelin loss. Adult forms of orthochromatic leucodystrophy are very rare. Our case was characterized by a fulminant course.

[Propriospinal myoclonus induced by relaxation and drowsiness].

Propriospinal myoclonus is a subtype of spinal myoclonus characterized by axial flexion or extension jerks, arrhythmic and prolonged muscle bursts with a pattern of activation consistent with a slow conduction within the propriospinal pathways. We describe a new idiopathic case of flexion propriospinal myoclonus occurring upon relaxation and drowsiness. This syndrome is to be added to the spectrum of movement disorders occurring during drowsiness.

[Management of arrhythmias in patients with heart failure].

Cardiac failure is a common cause of arrhythmia. Many factors predispose to the genesis of arrhythmias in these patients. A number of non-invasive methods allow stratification of the risk of arrhythmia in cardiac failure. Approximately half the deaths of these patients are due to arrhythmia. Unfortunately, most of the investigations for risk evaluation have a high negative predictive value but a lower positive predictive value. The treatment of supraventricular arrhythmias, mainly atrial fibrillation, is complex in cardiac failure. Class I antiarrhythmics are contraindicated. The only remaining options are Class II, especially Sotalol, and Class III drugs, especially Amiodarone. In some cases, non-pharmacological methods such as ablation, pacing or an implantable atrial defibrillator must be considered. The treatment of ventricular arrhythmias is also difficult. In this indication, Class I antiarrhythmic agents must also be avoided. Non-sustained ventricular tachycardia may be treated by betablockers or amiodarone. The use of an implantable defibrillator is increasingly recommended after the results of several controlled large scale trials. The indication is obvious in patients resuscitated from sudden death and these devices are also beneficial in sustained ventricular tachycardia in patients with cardiac failure. Many studies are currently under way to determine the value of this therapeutic modality in indications now considered to be "prophylactic".

Dependency in Parkinson's disease: a population-based survey in nondemented elderly subjects.

Little epidemiological data are available on the dependency status of elderly patients with Parkinson's disease (PD) living in the community. This study assessed the activities of daily living (ADL), the instrumental activities of daily living (IADL), and mobility in a representative sample of elderly nondemented PD subjects (n = 20), compared to a control population (n = 2,697). We found a significantly higher level of dependency in the PD sample based on ADL, IADL, and mobility scales. Half of the PD subjects were dependent for ADL (versus 13.2% for controls), 80% were dependent for IADL (versus 28% for controls), and 20% had their mobility restricted to their home (versus 1.5% for controls). The proportion of PD patients tended to be higher in those more depressed or with more severe motor symptoms. PD patients were not found to be more isolated socially or from family than was the control group and, in any case, dependency seemed not to be associated with isolation. When adjusting for age, sex, depressive symptoms, Mini Mental Status examination score, and dyspnea, PD remained significantly associated with dependency. PD thus constitutes a significant factor of dependency in elderly subjects living at home. Institutionalization occurred over four times more frequently in the PD group than in the general population, but no specific factor of institutionalization was noted.

Isolated corticotroph insufficiency associated to myasthenia gravis.

Myasthenia gravis, a neuromuscular junction autoimmune disease, may be associated to glandular autoimmune diseases such as adrenalitis. We report here the association of myasthenia gravis with a rare cause of adrenocortical deficiency, i.e. isolated corticotroph deficiency. The patient suffered from myasthenia gravis for 5 yr when she reported increased asthenia. Basal cortisol and ACTH levels were low. Metyrapone failed to increase ACTH and 11-desoxycortisol. Corticotropin-releasing hormone or tetracosactide failed to increase normally the cortisol levels. No other endocrine dysfunction or autoimmune disease could be detected. This observation enlarges the spectrum of endocrine diseases associated to myasthenia gravis and strengthens the need for careful endocrine examination.

Increased incidence of moderate stenosis among patients with diabetes: substrate for myocardial infarction?

Persons with diabetes are at higher risk for myocardial infarction and sudden death than are persons without diabetes. It has been demonstrated that the artery that occludes during acute myocardial infarction generally had less than 75% stenosis on a previous angiogram. The extent of coronary artery stenosis was analyzed for 820 consecutively examined patients who underwent coronary angiography at our institution. The patients were categorized according to the presence or absence of diabetes mellitus. The severity of stenosis was taken into consideration. Patients with diabetes had moderate (50% to 75% narrowing) stenosis much more frequently than patients without diabetes (50.6 versus 30.3%, p < 0.001). Moreover diabetes mellitus was an independent risk factor for moderate stenosis. The lesions were more frequently located on distal arteries, more frequently had a pattern of three-vessel disease, and had a trend toward more diffuse disease than described 25 years ago. This greater amount of moderate stenosis may be considered a substrate for future acute plaque rupture. It may explain the high prevalence of acute myocardial infarction and sudden death among patients with diabetes without an increase in the incidence of angina pectoris.

Frequency of severe attacks in migraine sufferers of the Gazel cohort.

The concept of severe migraine was raised to define migraineurs most in need of care and for use in clinical practice. We aimed to measure the frequency of severe attacks in a working sample of 276 migraine sufferers using a diary over a 3-month period. Migraine sufferers recorded each attack's clinical features, the degree of their disability, their use of drugs and the effectiveness of the drugs. Since the definition of severe attack is not standardized, we studied the impact of different definitions on the frequency. The frequency of severe attacks was 0.9% and appeared to be very sensitive to the definitions, ranging between 0.4 and 13%. In France, the extrapolated number of severe attacks is nearly one million out of a total of 115 million. In the migraineurs who had had at least one severe attack, the individual variability of intensity, duration or disability was very high, so the proportion of severe attacks in a given sufferer was low--between 15% and 50%. We conclude that the global concept of severe migraine is not relevant and should be split into two components--severe attack and severe migraine sufferer. The goals are different, too. Regarding treatment, for example, the severe attack concept is more valid for acute treatment strategies, whereas the severe migraine sufferer concept should be preferred to determine the need for prophylactic treatment. Since much work is being done nowadays to define acute treatment strategies, definition of the criteria of severe attack and validation of a measurement tool should be a priority.

Loss of productivity and quality of life in migraine sufferers among French workers: results from the GAZEL cohort.

To assess the burden of migraine, we compared the productivity and the perceived health of 991 migraine sufferers (IHS criteria), 1,004 other-headache sufferers, and 1,757 nonheadache sufferers randomly chosen among volunteers from employees of the French national gas and electricity company. The extent of absenteeism was collected independently from the study subjects during a 4-year period. Performance at work was self-assessed at the end of the followup. The study subject also completed the Short-Form 36 Health Survey Questionnaire (SF36) and the Spielberger anxiety scale. The number of workdays lost due to migraine was not statistically different between the migraine and control groups, after adjustment for age, sex, and number of health impairments other than headaches. Performance was found to be greatly reduced in migraineurs. The quality of life was poorer and the anxiety level was higher in the migraine group than in the nonheadache group. There was no difference between migraineurs and other-headache sufferers for any variable. The lack of difference in absenteeism between migraineurs and nonheadache subjects calls into question prior research based on self-report. Moreover, it also calls into question the economic impact on migraine due to its direct costs found in previous studies without a control group. We found that the burden of migraine may be considered mainly in terms of reduced quality of life.

[A comparative study of 4 diagnostic strategies in coronary artery disease. A theoretical approach].

The prognosis of patients with coronary artery disease is constantly improving mainly due to better control of risk factors, to improved methods of myocardial revascularisation and better follow-up. For effective management, the diagnosis of coronary disease has to be established. The authors used a statistical model to study the efficacy of 4 diagnostic strategies used in daily practice: (i) three investigations in two stages: exercise stress testing, stress echocardiography or stress scintigraphy followed by coronary angiography when positive (EE, ECHO, SCINTI): (ii) one stage investigation: coronary angiography of first indication (CORO). When the a priori probability is low or moderate (less than 0.5) the efficacy of EE, ECHO and SCINTI is excellent as there are few false negative results. When the a priori probability of coronary disease is high (0.7 or more), the percentage of false negative results (patients with undetected significant coronary disease) becomes very high. At this level of risk the CORO strategy avoids the false negatives without increasing the costs with respect to the other strategies. In conclusion, the choice of diagnostic strategy of coronary artery disease should take into account the "a priori" risk of the patient. When high (> 0.7), coronary angiography of first intention seems to be statistically justified.
