[Peripheral venous DSA of the calf arteries in arterial obstructive disease patients. A comparison with conventional angiography].

The ability of venous DSA to demonstrate the calf vessels distal to the trifurcation in patients with proximal blocks was compared with conventional angiography in 33 patients (44 limbs) in a study carried out by four observers. In 11.1% cases, the vessels were demonstrated better by DSA and in 2.5% by conventional angiography. In 86.4% the quality was the same. The reasons for this are analysed.

Antipyretic effect and tolerability of suprofen suppositories. Controlled clinical double-blind study with placebo.

The aim of the present study was to investigate the antipyretic effect of alpha-methyl-4-(2-thienylcarbonyl)-phenyl acetic acid (suprofen, Suprol) 300 mg suppositories compared to placebo in patients with fever of various etiology. The trial was designed as a randomized double-blind study including 30 patients per treatment group. After insertion of the suppository, temperatures were measured after 30 min, 1, 2, 3, 31/2, 4, 5, and 6 h. The study was performed at four investigational centers. At the end of the study, a total of 61 patients could be evaluated: 27 on suprofen, 34 on placebo. There were no statistically significant differences in the anamnestic data. The mean age of the subjects on suprofen was 66.5 years, and on placebo 61.3 years. Prior to treatment, the mean temperatures in the two treatment groups were 38.8 and 38.9 degrees C, respectively. In the suprofen group, the temperature dropped to a mean of 37.9 degrees C within 3 h and to 37.6 degrees C within 6 h. The decrease in temperature was less marked in the placebo group; here the mean temperature was 38.4 degrees C after 3 h and 38.2 degrees C after 6 h. Suprofen was found to be superior to placebo throughout the study, the differences being statistically significant after 3 h up to 6 h. The only side effect experienced by a subject on suprofen was vomiting.

[Digital subtraction angiography for the follow-up control of percutaneous transluminal angioplasty using a scoring system].

Seventy-two patients were examined by venous DSA following angioplasty, 28 of these within three weeks and 44 six months to three years after the procedure. The resolution of the DVI 2 V is adequate for showing reparative changes after dissection in the short-term follow-up. Demonstration was sufficient to enable one to use a coding system which classified the type and severity of the vascular changes. Clinical and angiographic follow-up showed good agreement regarding the amount of improvement obtained; in these patients, clinical follow-up alone is adequate. Where there has been no change in the clinical state or if there has been deterioration, a DSA examination should be carried out in order to ascertain the reason for the lack of improvement.

Thoracic epidural analgesia in the treatment of rib fractures.

Rib fractures and flail chest could be fatal if gas exchange is impaired. Efficient pain relief with continuous thoracic epidural analgesia allows a good physiotherapy management without central sedation and impairment of cough reflex, this prevents pulmonary atelectasis and infection. Eighteen patients/19 were treated with success in spite of flail chest, chronic obstructive pulmonary disease and minor pulmonary contusion. Intermittent positive pressure ventilation must be reserved to severe pulmonary contusion and other crushing injuries of the chest as bronchial or great vessels ruptures.

Implantable injection port for epidural opiates self-administration.

We organize the self administration of opiates by the epidural route on an out patients basis to relieve cancer pain. After the insertion of a percutaneous epidural catheter, we inject 3 mg of morphine sulfate and observe the patient for a duration of 24 hours in the intensive care unit. Then, an implantable, subcutaneous device is placed, connected to the epidural space by a silastic catheter placed through a 14 G Tuohy needle. After a careful training programme given at the hospital, the patient is allowed to go home. His progress is followed in the oncologic consultation unit every 15 days. The results are analysed in view of the rare side effects and of the good pain relief. The implantable port seems to be a valuable method to relieve cancer pain, but needs a rigourous technique and training.

Anesthesia, Epidural↗

Pediatric regional anesthesia. Its use in the pre-, per- and postoperative period.

Accidents in children can occur at any moment of the day. A child has rarely an empty stomach and it is very difficult to determine when he ate or drank for the last time. The stress of the accident can slow the digestion and even stop completely. Waiting for 6 hours will not reduce the risk of inhalation of gastric contents during general anesthesia. Pain and anxiety often present with anatomical deformation may be the cause of panic that should be promptly appeased. For these reasons, there has been an increase in interest in the use of nerve block techniques in children as has been the case in adults. We carry on Brachial Plexus Blocks and Epidural Blocks.

[Computer tomographic densitometry of primary liver tumors].

The dynamic computed tomography (CT) densitometry is defined as the combination of rapid bolus injection of urographic contrast medium and timed sequential computed tomography scan permitting recognition of different patterns of enhancement. The results of this method on fifty patients with histologically proofed hepatic tumors (focal nodular hyperplasia n = 12, cavernous hemangioma n = 13, liver cell carcinoma in cirrhotic liver n = 10, primary liver cell carcinoma n = 6, bile duct carcinoma n = 4, others n = 5) are demonstrated. CT-densitometry offers valuable aid for the differential diagnosis of focal liver lesions, i.e. cavernous hemangioma, focal nodular hyperplasia of the liver.

Color and conventional image-directed Doppler ultrasonography: accuracy and sources of error in quantitative blood flow measurements.

Accuracy of two systems--conventional (DRF 400, Diasonics) and color-coded (Angiodynograph, Quantum/Phillips) image-directed Doppler ultrasonography--was investigated using an in vitro model that generated both monophasic and triphasic pulsatile flow patterns. Estimated and actual blood volume flow rates showed good correlations, but the sampling with a hand-held transducer led to wide variations in measurement error for the conventional (-69.2% to 50%) and the color-coded (-79.3% to 265.7%) systems. By performing multiple measurements, one could improve accuracy considering only the maximal values of a series instead of the mean values. Accuracy was impaired by interposed muscular or fatty tissue due to false low time-average velocity measurements caused by a loss of Doppler signal. Comparison of both systems revealed significant differences between pulsatility index values (p less than 0.001), blood flow velocities (p less than 0.001), and blood volume flow rates (p less than 0.05 for program flow, p less than 0.001 for manual and automatic flow program of the color-coded system).

Thymic atrophy and rebound enlargement following chemotherapy for testicular cancer.

Serial thoracic CT scans of 50 consecutive patients with testicular cancer were reviewed. In 30 of them with metastatic disease treated according to the Einhorn Scheme changes in thymic size caused by cytostatic agents were studied. During chemotherapy 78.6 per cent of all patients examined developed thymic atrophy, followed by rebound enlargement that reached its culminating point 13 months after initiation of treatment. After that a slow (approximately 2 years) involution process began, until the initial size or somewhat smaller size was re-achieved. The other 20 patients with no evidence of metastases, and therefore not treated with chemotherapy, showed no changes in thymic size. Rebound thymic enlargement following cytostatic therapy for metastatic testicular cancer should not be mistaken for lymphadenopathy.

[Contribution of epidural anesthesia and general anesthesia to colon surgery].

The authors, having clinically observed favourable post operative effects on patients operated under combined general and epidural anaesthesia in digestive surgery, have proceeded to a retrospective and comparative study of the last 50 colic resections. They have observed: a +/- 50% reduction of the quantity of morphine used; a +/- 50% reduction of the time required for peristaltic resumption; a +/- 50% reduction of the time required for gastric intubation thereby contributing to carefree post-operation effects in colic surgery where a series of known complications are usually to be expected. They are of opinion that the benefit of epidural anaesthesia is due essentially to the preponderance of the para sympathetic nervous system allowing the release of some hormones with intestinal tropism as well as the disappearance of the negative action of the splanchnic vaso-constriction.
