In a study of 124 patients with systemic amyloidosis, renal involvement was the major presenting feature in 68 per cent of patients with AA and 42 per cent with AL disease. Renal failure contributed to the deaths of 72 per cent and 33 per cent of patients respectively. In 23 renal biopsies from patients with AA disease, quantitative glomerular amyloid infiltration correlated with urinary protein excretion and with serum creatinine. Seven of these patients had repeat biopsies; change in glomerular amyloid correlated with changes in both renal function and proteinuria. Four patients with end-stage renal amyloidosis have been successfully managed on CAPD for 37 patient months.
Serum antiviral antibody titers, presence of lymphocytotoxic antibodies, and lymphoproliferative responses to viral antigens were determined in 44 multiple sclerosis (MS) patients and 25 healthy subjects. These measurements were correlated with the HLA type and the clinical characteristics of the patients. The Dw2/DR2 phenotype, known to be associated with increased MS susceptibility, was also associated with a later onset of MS and more rapid progression of disease. Within the DW2/DR2 group, the disability status and rate of progression did not correlate with the relative degree of lymphoproliferative response. Patients with the Dw2/DR2 phenotype had similar lymphoproliferative responses to all viral antigens as control subjects possessing or lacking Dw2/DR2. Patients lacking Dw2/DR2 had increased lymphoproliferative responses selectively to measles virus when compared with normal subjects or Dw2/DR2 positive individuals. Absence of the B40 antigen was associated with high antibody titer to measles, and presence of the Dw7/DR7 antigens was associated with high titers against herpes simplex I virus. Both B40 and Dw7/DR7 phenotypes were associated with decreased occurrence of lymphocytotoxic antibodies. The Dw7/DR7 phenotype has been associated in previous studies with a decreased susceptibility to MS. These results, therefore, suggest that both humoral and cellular antiviral immune mechanisms in man are under the influence of HLA-linked genes. Progression of lack of progression in MS cold in turn be somehow related to the differing strength of antiviral immune responses in individuals of the appropriate HLA genotype.

Resistance to superinfection with vesicular stomatitis virus (VSV) occurred in GL-V3 monkey kidney cells infected with the CVS-11, Pitman Moore, LEP Flury, but not the ERA strain of rabies virus. Specific immunofluorescent staining of intracellular rabies antigen showed that the number and size of fluorescent foci increased after the onset of interference, and that this was paralleled by increasing yields of infectious virus. Although CVS-11 and ERA differed in their ability to induce interference, the virus yields from monolayers infected with either strain were similar. Interference apparently had no effect on the replication or dissemination of the inducing virus, and seems unrelated to the long incubation period or aberrant forms of infection in vivo.
The case of tracheal intubation using two doses of the new steroid neuromuscular blocking compound ORG NC45 (Norcuron) 0.1 and 0.15 mg/kg was compared with a standard dose of pancuronium 0.1 mg/kg at 60, 90 and 120 seconds. The results did not reveal any statistically significant benefit of the new drug within 2 minutes of administration over the control.
Rectal biopsy homogenates, faecal samples, and leucocyte rich plasma obtained from 39 patients were inoculated into tissue cultures. A non-transmissible cytopathic effect was observed in cultures inoculated with material from patients with Crohn's disease. In a blind study this cytopathic effect was observed in three specimens from three patients with Crohn's disease, in six specimens from four patients with ulcerative colitis, and six specimens from five patients with upper gastrointestinal disease. The cytopathic effect is unlikely to be associated with the bowel diseases tested. These results are also consistent with the failure of others to isolate a virus by the techniques used. With the limited range of techniques used no evidence was obtained to implicate conventional viruses in the pathogenesis of Crohn's disease.
Eleven cases of fracture or fracture/dislocation of the thoracolumbar or lumbar spine were studied to determine the effectiveness of three methods of management of the injuries. Ten of the patients had a neurological deficit as well as a spinal injury. All cases were studied by computed tomographic (CT) scanning of the injury site before and after treatment. CT scanning was found to be superior to linear tomography in determining the degree of canal compromise by bone at the injury site. Eight patients underwent operation, with the aims of achieving spinal stability in unstable injuries and of decompressing neural structures. These 8 patients had received Harrington distraction instrumentation (HDI) as an initial procedure. In those patients with fracture/dislocations from flexion-rotation injuries and canal compromise resulting from bony malalignment, reduction of the fracture/dislocation by HDI resulted in stabilization of the spine and increase of the open canal area at the injury site. However, in those patients with canal compromise resulting from bursting fractures with retropulsed bone fragments, HDI did not reduce in bone fragments. Removal of the anterior vertebral body and the bone fragments, however, did result in significant improvement of the canal dimensions. A poor correlation was found between the degree of canal compromise as measured by the CT scanner and the resulting neurological deficit. Reasons for this are presented. Six of the 10 neurologically compromised patients, including 2 patients with stable bursting injuries and retropulsed bone fragments treated conservatively, had improved one grade on Frankel's classification by 3 months after injury. The rate and degree of recovery were not related to the degree of canal decompression achieved at operation.

Blood samples from four patients at the time of surgery to relieve the compression of the carpal tunnel syndrome, which was diagnosed by clinical and electromyographic evaluation, were differentially assayed to determine the specific activities and the % deficiencies of the erythrocyte glutamic oxaloacetic transaminase (EGOT). The data from these assays revealed that these four patients had a severe deficiency of vitamin B6. These data, in conjunction with previous biochemical and clinical results over five years, underscore the desirability, and even necessity, of testing by the EGOT analysis for the presence of a severe deficiency of vitamin B6 in all such patients before surgery. Treatment with vitamin B6 (pyridoxine) for a minimum period of 12 weeks, depending upon the duration and severity of the symptoms, has been effective without exception. Surgery may relieve compression, but does not correct a deficiency of vitamin B6. Surgery in addition to therapy with vitamin B6 should be reserved for those patients who have had the deficiency for so many years that much tissue damage is irreversible by pyridoxine, and additional relief from pain can be achieved through the surgery.
We have studied the regulation of differentiation within the hemopoietic system by fusing mouse Friend cells (which can be induced to undergo red blood cell differentiation) to various mouse lymphomas and myelomas which express characteristic T and B lymphocytes surface antignes. Our results show that both erythroid and lymphoid differentiation markers can be co-expressed within the same cell. To determine whether this result applies to other differentiation states, we fused suspension Friend cells to three adherent fibroblast cell lines, and isolated both adherent and suspension hybrids. In fact, suspension hybrid clones were inducible for hemoglobin, whereas adherent clones were not. No obvious differences in overall chromosome balance were evident between the adherent and suspension hybrids. A similar correlation between suspension morphology and inducibility of hemoglobin was found in hybrids between suspension Friend cells and an adherent lymphoma line. These results show that different developmental programs can be co-expressed within the same hybrid cell; but the strongly adherent type of morphology is inconsistent with expression of the red blood cell phenotype, both in hybrid cells derived entirely from hemopoietic parental cells and in cells from widely different lineages.

Circulating interferon was found in volunteers immunized with 3.0 ml and 1.0 ml doses of human diploid cell strain (HDCS) rabies vaccines. Peak interferon titres were observed 17--25 hours after primary vaccination and the response diminished upon revaccination. The response was dose-related and did not correlate with the antigenic value of the vaccines or with the titre of infections virus in the vaccines before inactivation.
