[Urologic traumatology in childhood (closed injuries)].

Critical injuries of the kidney have to be treated surgically immediately. Here also have to consider the same rules of the surgery that go with an acute abdomen. By slight and severe injuries to the kidneys there is adaequate time for a thorrow diagnostic work up, prior to eventual operative intervention. The extreme rare ureter lesions with the symptomatic of a peritonism due to an urinoma and a retroperitoneal haematoma have to be treated operatively at the earliest possible time. Injuries to the urine bladder have always to be treated operatively with a special urgency since letality increases rapidly after six hours after the injury. Lesions of the urethra have to be treated operatively early by eliminating haematoma and urine and exact anastomosis of urethra's stumps to avoid later complications through scar-building.

[Immunocoagulopathies as a possible cause of acquired hemorrhagic diathesis].

Spontaneous factor VIII inhibitors may be responsible for severe life-threatening haemorrhages. It is reported on two patients with inhibitors directed against factor VIII: C. In one female patient a rheumatoid arthritis could be made evident as basic disease. Diagnostics, therapy and course are reported and discussed.

Renal allograft survival in patients transfused perioperatively only.

We investigated in a prospective study the effects of perioperative blood transfusions on the outcome of renal transplantation. All patients (n = 105) receiving their first cadaveric renal allograft were transfused perioperatively (i.e. 0-6 hours before transplantation) with two units of non-washed, unfiltered packed red cells. Forty-eight were transfused perioperatively only; 57 patients had received blood earlier and thus were transfused pre- and perioperatively. Graft survival one and two years post-transplant was 79 per cent at both time intervals in the group transfused perioperatively only, and 85 per cent and 74 per cent in the pre- and peroperatively transfused group. No adverse effects were observed concerning perioperative transfusions. In patients transfused perioperatively only, acceptable graft survival rates are obtainable. In these patients the risk or presensitisation is avoided, and thus their chance of successful transplantation increased.

[Emergency endoscopy in patients with hereditary hemophilia].

Haemorrhages from the upper gastrointestinal tract are a particularly threatening complication for patients with hereditary haemorrhagic diathesis. Though in the individual case under conservative therapy a temporizing attitude is possible, the source of the haemorrhage should be localized. When the peculiarities of this group of patients are taken into consideration the emergency endoscopy does not evoke a new danger of bleeding, on the contrary it is a decisive prerequisite for the individual therapy according to indication.

[Observations on polymyalgia rheumatica].

The polyalgia rheumatica, which appears in one part of the patients together with a giant cell arteritis of the region of shoulder girdle and pelvic girdle. Also intensive temporal artery, is manifest with severe pains of their headache may be dominant in the disease. Observations on 14 own patients who belong to this picture of the disease confirm the initially existing and under glucocorticoids rapidly reversible severe general humoral reactions. Up to now the incompletely tested immunological phenomena are not specific for the polyalgia rheumatica and must be supplemented by further examinations.

[Clinico-pharmacological investigation of an oral form of a depot preparation on silicone resin base with indomethacin as the active ingredient].

Indomethacin depot tablets on the basis of silicon resin were investigated in a clinical pharmacological study including pharmacokinetic data on 20 patients with rheumatoid arthritis. Effects and side-effects of the depot tablet are equal to those of the standard Metindol-capsules. After depot tablets serum indomethacin level is in the therapeutic range for 8--12 hours so that drug administration is reduced to twice daily.

[Early diagnosis of stomach neoplasms].

The primary prevention of the carcinoma of the stomach promises little success in near future. Therefore the effort is directed to early recognition and healing (secondary prevention). The optimum method for this is the combination of X-ray-examination, gastroscopy and aimed biopsy. But also by early diagnosis and therapy in the invasive carcinoma the chances of survival are to be improved. At present only 5-6% of the patients with gastric carcinoma survive longer than 5 years. The author adopts a definite attitude to the groups with risk of carcinoma. The search for further factors of persons endangered by carcinoma (habit of nutrition, kind of constitution and preparation of food, other exogenous factors) must be continued intensively in order to get selected groups which may be controlled consequently.

[Liver involvement in lymphogranulomatosis].

Of 146 patients with lymphogranulomatosis biochemical parameters were tested for their diagnostic valency concerning the recognition of a liver infiltration. In patients with histologically proved affection of the liver the AP, GGTP, AAP, LAP and LDH show a significant increase in comparison to the enzyme values of the patients without any hepatic manifestation. In an increased result of 4 enzyme values with a probability of 85% muste be reckoned with a participation of the liver. The enzyme SGOT, SGPT, GDH, LDH-isoenzymes, choline esterase, beta-GC, the De Ritis quotient and the quotient (Formula: see text), on the other hand, do not give any additional differential-diagnostic information.
