Postmastectomy rehabilitation in a community hospital.

The need for, operation, and outcomes of an in-hospital postmastectomy rehabilitation program are described as operating at a general community hospital, the Mt. Sinai Hospital of Cleveland. Patient outcomes in terms of range of motion (94 and 98 percent normal 90 days after discharge, right and left arm respectively); resumption of normal activities and return to work (94 percent had resumed normal activities within 90 days and 85 percent had returned to work); and emotional stress (fewer than one in ten reported moderate to severe emotional stress) are demonstrated to be indicative of good rehabilitation. These results are comparable to those reported achieved in a specialized oncology setting. This type of program can be successfully conducted in a community hospital with the limited use of regular hospital personnel, although problems related to physician resistance and assuring the regular availability of appropriate staff members for even a short time each day must be overcome.

Scoliosis and hydrocephalus in myelocele patients. The effects of ventricular shunting.

Developmental scoliosis is a common cause of increasing disability and deformity in long-term myelocele survivors, and is believed to result from a paralytic collapsing spine. The possible etiological role of compensated hydrocephalus and hydromyelia was assessed by determining the effect of ventricular shunting on 11 myelocele patients with developmental scoliosis. After successful shunting, one patient with a 47 degrees curve continued to deteriorate. Three cases with curves greater than 60 degrees were stabilized for short periods, but eventually required spinal fusion. Seven cases with curves less than 55 degrees were improved from a mean scoliosis of 29 degrees to 13 degrees during a 20-month follow-up period. Several patients had pre-existing shunts that were found to be non-functional on shuntogram. These findings suggest that the spinal complications of hydrocephalus may be more common than previously recognized in myelocele patients and that advanced developmental scoliosis may be avoided by early recognition and ventricular shunting.

Nasality and precision of articulation.

Two reading passages, one with nasal consonants and one without, were tape-recorded for 72 subjects: 34 selected as having precise articulation and 38 selected as having imprecise articulation. These speech samples were evaluated for degree of precision and also for degree of nasality by the psychological scaling method of equal-appearing intervals employing a 7-point scale. Analysis indicated a relationship between degree of precision and degree of unpleasant nasality, that is, a tendency for decreases in precision to be accompanied by increased nasality and a tendency for males to be more imprecise in articulation and more nasal than females.

Nitrogen inhalation as a method of euthanasia in dogs.

Thirty-four dogs were euthanatized in a chamber flushed with pure nitrogen. Electroencephalograms and arterial blood pressure, recorded during euthanasia, showed the dogs to be unconscious in an average time of 40 s and to be dead in 204 s. The dogs displayed no detectable signs of pain before unconsciousness occurred. Hypoxia produced by nitrogen inhalation was found to be effective, humane, safe, and economically feasible as a method of euthanasia.

Cyclic hormonal changes in patients with congenital abscence of the uterus.

Our studies have shown normal cyclical patterns of ovarian steroids and pituitary gonadotropines in patients with congenital absence of the uterus. It appears that the life-long abscence of a functioning uterus has no effect on the functional life span of the corpus luterum, but may have some effect in the level of estrogenes in the second half of the cycle.

Bleeding mucosal lesions in the upper gastrointestinal tracts of patients with oesophageal varices and hepatic cirrhosis.

Thirty panendoscopic examinations were carried out on 24 patients with oesophageal varices and hepatic cirrhosis who presented with gastrointestinal hemorrhage. A mucosal lesion, focal or diffuse, was found in the upper gastrointestinal tract in all but three instances. The findings confirmed previous reports that hemorrhage in patients with oesophageal varices frequently occurs from a source other than variceal. This study has also demonstrated that where hemorrhage occurs from oesophageal varices, erosions of the oesophageal mucosa may play a major role in its pathogenesis.

Cyclandelate in the treatment of cerebral arteriosclerosis.

Twenty-one patients with cerebral arteriosclerosis were treated for twelve months with placebo or cyclandelate (Cyclospasmol), 400 mg four times daily, in a double-blind study with medication cross-over after six months. The group included 8 men and 13 women, with a mean age of 69 years. Each patient showed at least 5 of 9 signs or symptoms adopted as "inclusion criteria" for the study. Concomitant psychotropic or other drug therapy was standardized or matched during the trial, and periodic assessments were made of the patients' behavioral, physical, neurologic and psychiatric status. No serious side effects were observed. There was no significant difference between the cyclandelate and placebo phases in measurements of physical state. Changes on the gross behavior scales were insufficient for analysis. Tests of memory, control of manual dexterity and comprehension of everyday situations showed statistically significant improvement during the cyclandelate phase. In contrast to placebo, no measurable intellectual decline occurred during cyclandelate therapy.

Tissue distribution of 203 Pb-acetate: comparison with 67 Ga-citrate as an abscess-localizing agent.

Since 203Pb-acetate accumulates in necrotic tumor tissue, the possibility was raised that it might also accumulate in other necrotic tissue such as abscess. We first studied the tissue distribution and excretion of 203Pb-acetate in control rats at 4, 24, 48, 72, and 96 hr. An enterohepatic circulation for lead is suggested. We then compared the uptakes of 203Pb-acetate and 67Ga-citrate in experimental abscesses in rats. The mean gallium accumulation in the abscess was 10 times that of lead at 24 hr and 12 times that of lead at 72 hr. The abscess-to-tissue ratios were greater for gallium for every tissue examined, although the abscessed areas were clearly visualized by scanning at 24 and 72 hr with both agents. With the exception of blood, abscess-to-tissue ratios for 67Ga at 24 hr were higher than or equal to those at 72 hr. However, the 67Ga ratios for the inflamed tissue surrounding the abscess to muscle and blood were higher at 72 hr than at 24 hr, which suggests that inflammation without abscess might be better identified by gallium scanning at 72 hr.
