[Continent ileostomy - improvement of life quality].

Fifteen patients who had undergone proctocolectomy and conversion of conventional ileostomy into continent ileostomy as devised by KOCK were investigated an average of 4 1/2 years after continent ileostomy. The aspects investigated were skin problems, frequency of ileal pouch emptying, continence, and quality of life. It is concluded that successful conversion to continent ileostomy has eliminated most of the drawbacks of conventional ileostomy. The technically complex method of continent ileostomy should be employed only by surgeons experienced in this type of surgery and at centers where more experience with the method can be accumulated.

[Isolated perforations of the small intestine following blunt trauma of the abdomen].

Ten cases of isolated perforation of the small intestine after blunt abdominal injury are presented. The clinical signs are initially poor, while X-ray and laboratory findings are of little value (only 1 patient with free air on abdominal plain X-ray; only 1 case with more than 10 000 leukocytes on admission). Progressive peritonitis is the only reliable indication for immediate surgery.

[Medical and surgical treatment of 209 bronchial cancers].

Of 209 unselected patients treated for bronchial carcinoma between 1976 and 1980, only 67 underwent surgery: 34 lobectomy (1 death), 19 pneumonectomy (5 deaths) and 14 exploratory thoracotomy without fatal outcome. Average survival time in lobectomy patients who died during the control period was 10 months and in those still alive 19.8 months. The corresponding figures for pneumonectomy patients were 3.9 and 17 months. Out of 142 patients considered unfit for surgery, 125 died. In the group who did not undergo radiotherapy, average survival time was 2.9 to 6.8 months depending on the histologic cell type of the carcinoma. In patients undergoing irradiation the corresponding figures were 4.7 and 7.0 months. In conclusion, radiotherapy was only of limited value in non-operated patients. Diagnosis of bronchial carcinoma is still established far too late, and therefore surgery in an unselected patient group was performed only in 32%. Lobectomy, if practicable, should be advocated as treatment of choice, whereas pneumonectomy, especially of the right side, involves marked operative mortality, provides results scarcely better than conservative treatment, and is recommended only in selected cases.

Adenocarcinoma↗

[Papillary mesothelioma of the left spermatic cord and tunica vaginalis].

The primary mesothelioma of the spermatic cord is a very rate tumor, histologically benign. Its evolution, because of the possibility of recurrence, is uncertain. We have found only six cases in the literature. Our observation relate the very quick apparition by a young Italian, 19 years old, of a left hydrocele under tension ascending into the inguinal fold. At operation, we discovered a mesothelioma in the spermatic cord and in the tunica vaginalis, covering both testicle and epididymis. Ten months after a left enlarged hemicastration, without scrotectomy, no recurrence has been reported.

[Perforation of cancer or stomach ulcer: a 1- or 2-stage operation].

The results of simple closure were compared with those of partial resection in the treatment of acute gastric perforation in 13 patients. Perforation was due to ulcer in 10 cases and to cancer in 3 cases. Differential diagnosis between cancer and ulcer during emergency surgery is difficult to establish and could be made in 2 cases only postoperatively. In the 8 patients undergoing gastric resection there was one fatal outcome by pulmonary embolism. In 5 patients where the perforation was only sutured, one patient aged 97 died of pneumonia. 2 patients developed severe hemorrhage and needed an emergency gastric resection, that was survived by only one patient. - Our results and those of the literature suggest that in acute gastric perforation due to ulcer or carcinoma, emergency gastric resection gives far better results than simple closure by suture.

[Scanning electron microscopy study of autologous vein and fascia lata before and one year after implantation into the arterial blood stream].

The present communication deals with scanning electronmicroscopical changes of autologous vein and autologous fascia lata, which were used as arterial implants. We examined material, which could be removed during reoperation for vascular occlusion. After one year, the surface exposed to the blood stream shows a homogenous layer of fibrin. Whilst the elastic fibers of fascia lata-transplants have nearly completely disappeared, we can still find them in the vein. This might be the essential reason for the longer survival of venous implants into the arterial blood stream.

[Extraction of ureteral stones using a catheter with 2 inflatable balloons. Preliminary report].

In a preliminary communication a new method for endoluminal extraction of ureteral stones is described by using a catheter with two inflatable balloons. The length of the catheter is about 100 cm and its diameter corresponds to the usual ureteral catheters. It has four openings: the central opening is located at the end of the catheters. Two other lumina open into the two balloons located at a distance of 3-5 cm. An other opening is located between the two balloons. By insufflating and emptying of the distal balloon, the concrement in the ureter is mobilized and caught. By insufflating the two balloons and injection of contrast media through the opening situated between the two balloons, the ureter is ovistended, the stone is mobilized can be extrac "swimming" ie contrast media between the two balloons. Tillenow, o concrements have been extracted from the distal ureter by this method.

[The risk of lung emolism in isolated thrombosis of the V. saphena magna].

Within 1 1/2 years we have seen 4 pulmonary emboli, one of which was fatal. The origin was an isolated thrombophlebitis of the great saphenous vein. An autopsy, with dissection of the veins of the legs and of the pelvis, and phlebographies where there is any doubt of deep venous thrombosis, allow us to speak of isolated thrombosis of the great saphenous vein. The clinical level is always lower than the real level and the thrombus can grow through the saphenofemoral junction. The conservative treatment (anticoagulation and mobilisation) do not prevent a cranial ascension of the thrombus nor a pulmonary embolism. So, when the thrombosis is higher than the knee, we do a crossectomy of the great saphenous vein. We did this operation 8 times without complications.

[Replacement of Bauhin's valve with an end-to-end telescope anastomosis between the ileum and transverse colon. Experimental study in dogs].

A method of a telescope anastomosis for prevention of reflux was developed experimentally in dogs. After the resection of 40 cm of ileum and colon ascendens the ileal part was intussuscepted into the colon and fixed by seromuscular stiches. The vessels were left untouched. The intussuscepted part measured 3-4 cm. No ileus was observed in the 24 weeks. Glass balls of 1.2 cm diameter could pass through. Barium enema as well as air insufflation of different pressures showed no reflux. The serosa was covered pretty soon by a granular tissue and later on by normal mucosa.

[Reconstruction of the diaphragm with a pericardial flap after radical pleuropneumonectomy for malignant pleural mesothelioma (author's transl)].

Pleuropneunomectomy with resection of the diaphragm only offers acceptable therapeutic results in patients with multifocal malignant mesothelioma of the pleura. Diaphragmatic replacement on the right side can be satisfactorily achieved by fixation of the liver to the thoracic wall. On the left side many homologous and heterologous tissues and prosthetic adjungts have been suggested with varying success. This report describes technique and results in two patients where the broad pedicled left hemipericard was successfully used to replace the resected left diaphragm.

[Treatment of spontaneous pneumothorax and prophylaxis of relapses (author's transl)].

This study reports the treatment of 192 consecutive cases of spontaneous pneumothorax observed in 99 patients. Relapses are frequent, but it could be shown that their number can be reduced by more aggressive therapy. Conservative treatment led to 99 % of relapses, whereas they were reduced to 32 % by continuous pleural suction, as shown in 76 cases. Pleurodesis with glucose 40 % or 50 % solution followed by continuous suction for 7 days, is advertised by the authors for every first episode of spontaneous pneumothorax. Under this treatment relapses were observed in only 19 % of the cases. If however after this treatment a recurrent pneumothorax occurs, thoracotomy has to be considered for parietal pleurectomy and occasional resection of emphysematous bullae. This procedure was successfull in all 13 cases where it had been applied.
