Intracanalicular lipoma.

We present a patient with unilateral progressive painless loss of vision leading to optic atrophy and blindness. High-resolution computed tomography and magnetic resonance imaging demonstrated adipose tissue extending from the anterior portion of an enlarged optic canal posteriorly to involve the ipsilateral optic chiasm. To our knowledge, this is the first reported case of an intracanalicular lipoma.

Quantitative anatomy of the thoracolumbar epidural space.

Quantitative measurements of the epidural space between T-7 and L-4 were made in the sagittal and coronal planes utilizing x-ray films made after the injection of iodized oil into the epidural space in the low thoracic and upper lumbar areas. These data reveal a 1-mm ventral epidural space and a 2-mm lateral epidural space, with a sawtooth shape to the dorsal epidural space measuring between 1.1 and 2.9 mm at the rostral lamina and between 3.8 and 6.5 mm at the caudal lamina. Additionally, five patients with chronic pain were studied by computed tomography of T-8 to T-12, with confirmation of the sawtooth shape of the dorsal epidural space. Computed tomography showed the measurements of the epidural space at the rostral lamina to vary between 1.3 and 1.6 mm and those at the caudal lamina/interlaminar space to range from 6.9 to 9.1 mm.

Evaluation of the lumbar spine. A comparison between computed tomography and myelography.

The CT scan and the metrizamide myelogram are compared in evaluating multiple disorders of the spine. Examples are shown of degenerative disc disease, including lateral discs, isodense discs, and degenerative changes, which cause varying degrees of stenosis of the spinal canal and neuroforamina. Examples of discitis and neoplastic involvement of the vertebral bodies and spinal canal are also shown. Finally, several examples of dysraphic conditions of the spine are presented. CT examination for disc disease is still most often used as an adjunct to the myelogram. However, as knowledge and experience is gained about its value in diagnosing all abnormalities of the spine, its use will increase, and CT may become the primary method of evaluation, after ordinary radiographs.

Partial regressive occipital vertebra.

Craniocervical developmental anomalies can be quite varied and many involve the proatlas and its remnants. A previously undescribed variant consisting of a remnant of the proatlas and its transverse processes is reported and the developmental anatomy of this region is summarized.

Three-dimensional reconstruction of an iliac artery aneurysm in a man with obstructive uropathy.

Reconstruction of three-dimensional images is performed either by scanner-based systems or by independent computers. The advantages of a stand-alone system include compatibility with multiple imaging modalities, more advanced technology, and increased user interaction. This report illustrates the use of a stand-alone system for the less commonly utilized purpose of three-dimensional reconstruction of abdominal soft tissues. It further demonstrates the merit of three-dimensional imaging as an important diagnostic adjunct and as a valuable source of preoperative surgical information.
