[Trichobezoar as a rare cause of ileus of the small intestine].

For several weeks a 15-year-old girl had complained of increasing abdominal pain with vomiting. On admission to hospital the bloated abdomen was diffusely sensitive to pressure and the bowel sounds were high pitched and loud. Erythrocyte sedimentation rate was increased to 23/40 mm and the white cell count to 12,000/microliters. Ultrasound examination revealed an echo-dense area with dorsal echo loss at the gastric side of the pylorus. X-ray films of the stomach showed fluid levels and a soft-tissue mass in the left upper abdomen. At laparotomy a large (12 x 6 cm) trichobezoar was found in the middle of the small intestine and two smaller ones at the pylorus. Subsequently the patient admitted to trichophagia but refused any psychological treatment.

[Venous end-to-end anastomosis with non-absorbable and absorbable screw flange rings].

Forty veno-venous anastomoses performed on femoral blood vessels of dogs showed good results using a new flange ring system. In 15 cases Palacos rings were used for the anastomoses. Follow-up studies during a period of 8 days to 1 1/2 years showed only one occlusion. Twenty-five anastomoses were performed with absorbable rings of vicryl. Follow-up studies of these anastomoses during a period of 10 days to 26 weeks showed two stenoses and two occlusions. Dissolution of the Vicryl rings lasts up to 26 weeks.

[Treatment of malignant lesions of the esophagus with the thermoprobe].

In inoperable stenosing tumours of the oesophagus the problem arises of maintaining oral food intake. As malignant cells are more cold-sensitive than normal cells oesophageal tumours may be "cold-coagulated" with special probes. Preliminary results with this treatment in 28 patients are reported.

Esophageal Neoplasms↗

[Results of Implantation of Aorto-Femoral Bifurcation Prostheses (author's transl)].

From June 1974 to December 1975, 139 aorto-femoral Y-grafts were implanted at the surgical clinic of the University of Münster. The operation mortality was 4.3 percent. Among the postoperative complications were sexual disfunction and paraesthesia. For the ileo-femoral occlusions the postoperative success rate was 90 percent, combined ileo-femoro-crural changes showed good results in 60 percent of the cases. The results show that isolated reconstruction in the pelvic area is successful in most cases, even in combined ileo-femoro-crural obstruction. Only in exceptional cases a second operation (reconstruction of femoral artery) is necessary after a period of about one year.

[Surgical management of achalasia].

As long as its etiology is uncertain, achalasia can only be treated symptomatically. Basically, conservative dilation treatment and operative cardiomyotomy can be performed. Although dilation treatment can save the patient the operative procedure, it has relative disadvantages. For this reason since 1973 we treat achalasia through cardiomyotomy with Nissen's fundoplication method. We have operated on 25 cases using this method. These are examined and the results reported.

[Juxtapapillary diverticula of the duodenum and bilio-pancreatic symptoms (author's transl)].

Juxtapapillary diverticula of the duodenum are more frequent than has been assumed up to now. Among 2100 patients undergoing duodenoscopy 135 duodenal diverticula have been found. These diverticula may cause symptoms of diverculitis as well as of bilio-pancreatic disease. Such diverticula ought to be treated by surgery. In our department diverticula are turned inward, or a papillo-diverticulotomy is done depending upon the localization of the diverticula.

Cholangitis↗

[Artery end to end anastomoses with screw-dense-fixation-rings (author's transl)].

In study of screw-dense-fixation-ring method in 15 cases of a dog's femoral artery, end to end, was reconstructed. The anastomoses is achieved by vessels of 3 - 6 mm diameter. Within a period of 1 - 6 months post-operatively, average of 3 months, angiographic control of all cases showed no complications such as aneurysm or stricture. The construction of screw-dense-fixation-ring seems to give a considerable result, because the power which is keeping the rings together, can be controlled.
