Gastrointestinal tract anomalies--neonatal medical problems.

The role of early diagnosis on the short-term prognosis of 55 infants with gastrointestinal tract malformations admitted to three neonatal intensive care units is reviewed. In this series antenatal diagnosis does not appear associated with better prognosis. The severity of the disorder and the association with other malformations are the most important determinants of outcome.

CT findings in eclampsia.

Cranial CT was performed in five patients with eclampsia: densitometric modifications were present in all cases. Only one patient, who later died, displayed multiple foci of cerebral haemorrhage; all others presented bilateral, symmetrical, nonenhancing hypodensities with mass effect interpreted as cerebral oedema. At CT performed 7-14 days after interruption of the pregnancy, these characteristic hypodense lesions were no longer present while neurological symptoms disappeared.

[Experimental intrauterine surgery: the creation of a congenital diaphragmatic hernia in a sheep fetus].

Up to 50% of the neonates operated during the first hours of life for a congenital diaphragmatic hernia die. The presence of a severe lung hypoplasia, which is at the origin of hypoxia, acidosis, increased pulmonary vascular resistance and right to left shunt, explains the poor clinical results, in spite of surgical success and intensive therapy. A modern approach to the problem includes treatment with pulmonary vasodilator drugs, whose effects are still discussed. Perhaps the prenatal recognition of the defect could improve prognosis. Lots of experience are needed to know more on the subject. This could be obtained with an experimental approach on newborn animals with a CDH. The article relates authors experience in the creation of a CDH in a foetus of sheep, by means of a surgical intervention during its intrauterine life.

[Posterobasal infarction and right bundle branch block: value of computerized vectorcardiography].

The object of this study was to determine the vectocardiographic criteria of posterobasal myocardial infarction associated with right bundle branch block. Seventeen patients were examined; all had clinical and enzymatic evidence of myocardial infarction, associated with the unusual appearances of right bundle branch block with an isolated R wave in V1 and V2. The Frank X, Y, and Z axes were treated by computer to obtain a detailed octonal study of the QRS and T loops. Vectors of special interest were determined: the maximum maximorum vector and the QRS half surface vector (module, azimuth, elevation, appearance time). The octonal and total surface are of the QRS and T loops and the spatial angle of the maximal vectors of the two loops were calculated. These cases were characterised by clockwise rotation of the QRS loop in the horizontal plane. The maximal QRS vector had a decrease module (1,29 mV +/- 0,49), an azimuth greater than 20 degrees (42,8 degree +2- 20,3) an appearance time of 55,7 +/- 12,1, and an elevation which was either positive or negative depending on the presence or absence of an associated left anterior hemiblock. The half-surface vector was superimposed on the maximal vector: module (1,09 mV +/- 0,39), azimuth (45,2 degrees 23,5(and appearance time (62,0 +/- 11,7 ms). The posterior surfaces were nil or negligible (less than 1 p. 100). The T loop had a clockwise rotation in the horizontal plane and its maximal vector projected anteriorly in 8 patients. The appearances observed in these 17 cases were different to those of isolated right bundle branch block. The classical causes of anterior ORS loops could be excluded easily: right ventricular hypertrophy, Wolff-Parkinson-White syndrome, intraventricular conduction defect. The following criteria were retained in the diagnosis of posterobasal infarction associated with right bundle branch block:--principally, clockwise rotation of the QRS loop in the horizontal plane;--anterior displacement of the maximal vector with an azimuth of over 20 degrees;--disappearance of the posterior forces;--associated with the usual criteria of right bundle branch block;--clockwise rotation of the T wave loop in the horizontal plane.

Human milk: relationship of fat content with gestational age.

In order to assess the suitability of early human milk as a food for preterm infants, concentrations of fat were analyzed in milk samples representative of complete 24-h expressions, obtained serially over the first 30 days of lactation from 47 mothers delivering at term (FT) and 25 mothers delivering prematurely (PT). During the first postpartum month milk fat increased with progressing lactation both in FT and PT mothers' group. The increase was significant over the first 10 day period. During this stage the fat content was significantly higher in the milk from PT mothers than in FT mothers. A significant inverse correlation of fat content with gestational age was established. On the basis of these results, the higher energy intake obtained with PT milk suggests that is may be reasonable to prefer the use of mothers' own early milk than pooled milk as the more appropriate feeding for premature infants.

Fetal lung hypoplasia: biochemical and structural variations and their possible significance.

Quantitative biochemical criteria for lung growth and maturation were compared with the histological appearances in hypoplastic lungs from 20 fetuses and newborn infants. Cases associated with oligohydramnios showed a characteristic series of changes with narrow airways, retardation of epithelial and interstitial growth, delay in development of blood-air barriers, and low concentrations of phospholipid phosphorus, lecithin phosphorus, total palmitate, and lecithin palmitate. The growth and maturation arrest appeared to affect the peripheral part of the acinus. Examples of other types of lung hypoplasia showed different features. Hypoplastic lungs from infants with normal or increased amniotic fluid were of mature structure with phospholipid concentrations similar to those of infants with normally developed lungs at term. The hypoplastic left lung in 2 cases of congenital diaphragmatic hernia had an immature structure with low phospholipid concentrations, whereas the right lung has structurally and biochemically more mature. It is suggested that fetal lung growth may be impaired by any influence which reduces thoracic volume but that maturation arrest is due specifically to loss of the ability to retain lung liquid.

[Gaseous myelography. Accidents and complications. Case reports].

Attention is called to the risks of gas myelography and two cases of complications are described. One of these, observed personally, was fatal and its possible pathogenetic mechanisms are illustrated. Stress is laid on certain rules and precautions that should be respected in radiological and anaesthesiological practice. Insufflation pressure and volumes of injected air deserve particular attention and scrupulous intraoperative control cut of vital parameters must be carried and appropriate attention paid to patient hydration during the postoperative period.
