Cerebral metabolic protection in patients submitted to carotid tromboendoarterectomy (TEA) can be made by means of drugs, both in the clamping acute intraoperative phase and in the immediate post-operative period. The knowledge that DPH has the property of reducing CMRO2, the lactates production and of increasing the cerebral level of glucose, glycogen and phosphocreatinine, has persuaded us to use this drug instead of barbiturate, as a therapeutic protection to prevent hypoxic damages to the nervous cell. Our series include 12 patients submitted to carotid TEA in whom cerebral metabolic protection has been obtained by means of DPH at the dosage of 15-17 mg/kg body wt. injected intravenously in about 15' just before clamping. Using this type of pharmacologic protection, we have not observed any of the undesired effects in the cardiocirculatory system described in the literature. The prompt awakening, the absence of neurological deficits, the absence of side-effects suggest that DPH can be used to provide a cerebral metabolic protection during TEA.
A population of 140 fourth graders participated in a combined audiometric and tympanometric screening project. Pass/fail criteria were established for both subtests. The children were compared between the two tests with the following findings: (1) 52.2% passed both tasks and 5.0% failed both; (2) 36.4% passed the pure-tone task and failed the tympanometric procedure; and (3) 6.4% failed the pure-tone observation and passed the tympanometric study. Of the total population, 42.8% were inconsistent for the norms established for the two tasks. Since the ideal goal of a hearing screening task is to locate children with hearing losses and/or medically related hearing problems, the routine use of tympanometric screening as an integral part of a hearing conservation program is proposed.
Van der Hoeve's syndrome is the all-round variant of Lobstein's disease (late osteogenesis imperfecta) and is characterized by the concurrent presence of osteoporosis, blue sclerae, and deafness. The literature has reported sporadic cases of a combination of algodystrophic syndrome and Lobstein's disease. Our case represents a classic example: a 50-year-old woman with blue sclerae since birth, bilateral deafness, major dental alterations, vitreous osteoporosis and a history of pathological fractures who, two years from the onset of menopause, complained of pain involving the right ankle. After admission to our Department, a diagnosis of algodystrophy combined with van der Hoeve's syndrome was made on the basis of her clinical history, objective signs and x-ray results. Treatment consisted of i.v. clodronate disodium at a dose of 300 mg daily in 250 ml saline solution for 7 consecutive days. At the end of the treatment cycle the patient reported an improvement in her symptoms with nearly total regression of right ankle pain, while swelling disappeared within a few weeks.
