[Experimental investigation on non-specific foreign body sarcoma induction of Al2O3-ceramic implants (author's transl)].

This investigation presents experimental results on possible non-specific foreign body sarcoma induction by Al2O3-ceramic implants. Solid and porous discs, subcutaneously implanted in rats produce a sarcoma rate of 17.4 resp. 17.8 p.c. while roughly perforated discs initiate sarcomas at a rate of 25.4 p.c. These values were found to be lower than comparative values in the literature produced with different metals and medical grade plastics. Powdered alumina implants did not produce any sarcomas. Only this latter result can mainly be transferred into prognostic assumptions concerning the clinical application of Al2O3-ceramic. The results are discussed extensively in the lights of the existing literature, this indicating that the tested ceramic itself may not possess a significant sarcogenetic potency in humans.

Intraosseous ganglion (author's transl).

The intraosseous ganglion must be always included in the differential diagnosis of cystic epiphysial bone lesions, which are localised in the vicinity of a joint, especially since this bone disease can be succesfully operated on. The preoperative diagnosis can be established with the help of conventional and tomographic x-rays examination. A communicating chanel, which is recognizable in tomographic cuts as a lucent line between the ganglion and a joint, is a pathognomic finding. The interpretation of the radiological symptoms can be difficult in those cases in which the lesions are not yet well developed or in which concomitant degenerative joint changes are present.

[4-7 year results of total hip arthroplasty with special reference to late complications].

The 4-7 year results of total hip arthroplasty (95 hips, McKee-Müller and Charnley-Müller) are presented in detail both from a clinical and radiological point of view. About 83% of the hips showed a good and excellent clinical condition at the time of reevaluation. Deep infection was observed in 4.2% of the hips, 11.2% of the hips had already been reoperated at the time of reevaluation because of aseptic loosening of one or both components. Another 3.5% of the hips showed clinical and radiological evidence of aseptic loosening, some more 4.9% of the hips had radiological evidence of loosening without clinical signs at the time of reevaluation. Furthermore, 6.8% of the hips had radiological evidence of third degree periarticular calcification influencing the clinical state of the hip in a negative manner. Over all serious complications which gave or will give rise to reoperation were 30.6%. The causes for serious complications are discussed extensively. It is proposed to restrict indication for total hip arthroplasty and to reemphasize hip-conserving procedures more seriously.

[What has brought us the Colonna plastic? (author's transl)].

From 1960--1967 a series of 34 hip joints with high dislocation were treated by Colonna capsular arthroplasty. 28 hip joints were followed up. Clinical results revealed excellent results in 3 hip joints, good results in 5 hip joints, fair results in 14 hip joints and poor results in 6 hip joints. 15 hip joints were without complaints. Radiologically there was only one hip with an excellent result, 8 times the result was good, 10 times fair and 9 times poor. Poor results tended to occur in hip joints with bilateral involvement and when deformation of the epiphysis developed. The capsular arthroplasty may be a useful operation if there is a consequent preoperative traction, a consequent splinting after the operation, a good technique according to Colonna's regimen and if the child is between 3 and 7 years old.

Evaluation of a bioglass-coated Al2O3 total hip prosthesis in sheep.

A bioglass-coated fully dense alumina total hip was evaluated in sheep without using polymer bone "cement." After 3 months following surgery, both the acetabular cup and femoral stem component were firmly attached to bone. Histologic and radiographic analysis of the bone-implant interface showed regions of woven and even lamellar bone formation where high load transmission occurred. In regions of low loading or poor fit, a pseudosynovial fibrous lining was present.

Biomechanically induced tissue reactions after Al2O3-ceramic hip joint replacement. Experimental and early clinical results.

A biomechanical concept of interface maturation around cement-free implanted alumina-ceramic hip endoprostheses in sheep is explained. Provided there is rigid primary stability of the implant within the bone bed, hard tissue formation along the support areas of the implant is mainly dependent on implant shape and main local stress direction. Three subsequent phases of interface maturation can be distinguished, finally resulting in firm fixation of the implant by mechanical interlocking of supporting bone and ceramic. A new composite alumina-metal hip endoprosthesis for partial cement-free implantation (cup) is presented. The technique, postoperative care, and encouraging early results in 5 patients are demonstrated. A new ceramic femoral component for cement free implantation is proposed.

Morphological and biomechanical aspects of Al2O3 ceramic joint replacement. Experimental results and design considerations for human endoprostheses.

Four different acetabular replacement designs and 2 femoral head replacement designs made out of high density alumina ceramic have been tested in sheep with postoperative survival times of up to 11 months (Frialit 99: Al2O3 99.6%, MgO 0.4%). It can be concluded from these experiments that a cement free implantation technique with alumina requires a primary rigid fixation of the implant to bone, with the possibility for new bone growth along the ceramic and into grooves and macroscopic holes for further interdigitation. Three different solutions for the acetabular component can be recommended for human application. In addition, an all ceramic femoral prosthesis has been tested for bending strength in its neck region. Twenty prostheses have been factured, all of which showed values for bending strength far higher than the physiological neck fracture level (900 kp/cm2). The present state of development of all ceramic prostheses is discussed. Up to now, the development of fibrous tissue between the bone and the ceramic implant has caused reservations with human implantation. The direct definite anchorage problem has to be resolved first.

[The alumina oxide ceramic-metal composite endoprosthesis. A new hip endoprosthesis for partially cement free implantation (author's transl)].

The favorable combination of properties of dense, pure Al2O3-ceramics as a material for endoprostheses of the high load bearing joints have been confirmed in more than 4 years of experimental work with different types of animals. During this period Al2O3-ceramic parts for total hip replacement have already been developed and tested as well as the special instrumentation and the technique of the surgical procedure concerned. Based on these experiences a new total hip endoprostheses together with its complete technique for the operation is presented with which an improved alternative for replacements of Weber-Huggler-prostheses is offered and the application of total hip replacements can be extended to patients below 60 years of age.

[An X-ray approach to differentiate benign and malignant bone tumors (author's transl)].

A critical analysis of X-ray signs induced by the interaction of a bone tumor with bone itself is presented. These enables the physician to recognise a pattern of characteristic findings for each particular tumor. Clinical examples are demonstrated illustrating at the same time a schematic classification of those signs on a roentgenogramm which in many cases facilitate the prediction of the specific diagnosis and in consequence the prognosis of the lesion. The need for an effective cooperation between the involved specialties is stressed.
