Three- to five-year results with the cementless Harris-Galante acetabular component used in hybrid total hip arthroplasty.

In this paper we present our 3- to 5-year results after hybrid total hip replacement using the cementless porous coated Harris-Galante acetabular component and the cemented Griss femoral component in 39 patients with 40 implantations. Postoperatively, mild to moderate pain was experienced by 16.1% of patients, mostly following hard activity. A slight to moderate limp occurred in 24.2%. All patients were able to walk more than 1000 m. Full-time use of support was required by 2.8%. The postoperative range of motion was increased in 93.9%. Radiographically, none of the acetabular or femoral components had to be classified as unstable. Only one acetabular component displayed complete (i.e., along all interface zones) radiolucency and was therefore classified as possibly unstable.

Ten- to 14-year results of a ceramic hip prosthesis.

One hundred total hip arthroplasties with a cementless aluminum oxide socket and ball with different types of cemented metal femoral stems were performed from 1974 to 1979. All were primary implantations. These cases were recently reviewed in a ten- to 14-year follow-up evaluation. Despite extensive laboratory testing of the ceramic material, a significant number of cases resulted in a very unfavorable result such as extensive wear, ball fracture, or migration of the socket. Whereas the patients had died or could no longer be traced in the case of 23 prostheses, there were 25 prostheses that had already been exchanged at the time of review for mechanical reasons (two stem fractures, eight ceramic ball fractures) and for component loosening attributable to wear of the alumina components. The remaining 52 prostheses still in place had good clinical results in 80%. However, roentgenographic evaluation of these prostheses revealed signs of loosening in six femoral components and in three sockets. Migration of more than 5 mm was evident in 24 sockets (46% of surviving prostheses). Although this had caused loosening in only three sockets at the time of review, further migration up to the limits of the acetabular region would be a major reason for future failures.

[Craniocerebral trauma and aseptic osteonecrosis. Steroid-induced sequelae after therapy of brain edema].

In this publication we present three cases of avascular osteonecrosis (AON) of the femoral head and the talus in adolescent and young adult patients following short-term, high-dose steroid therapy for cerebral trauma. All patients were proven to be free of other risk factors for AON. The latency period between the steroid therapy and the occurrence of AON ranged from 12 to 60 months; steroid therapy lasted for 12 to 16 days with a total dosage equivalent to 2370-7180 mg prednisolone. Our case reports are added to a review of the literature with 14 similar cases reported receiving short-term, high-dose steroid therapy; 2 of them were also after brain trauma. To the best of our knowledge, 1 of our cases shows the longest latency period ever described after such therapy, and all our cases had a shorter therapy time than reported by other authors who have mentioned brain trauma patients. Additionally, bilateral necrosis of the talus due to short-term, high-dose steroid therapy has also not previously been described. Whereas AON is quite common after long-term steroid therapy (e.g., for immunosuppression and rheumatic disorders), our cases prove that serious complications can occur even after a short period of emergency steroid therapy in cerebral trauma, a possibility that is generally unknown. The minimal dosage and the maximum time steroids can be applied to exclude the risk of AON has not yet been determined, although we do recognize that steroids are useful for the prevention of brain edema.(ABSTRACT TRUNCATED AT 250 WORDS)

Vertebral body replacement with homologous femoral head transplants.

From 1978 to 1989, we carried out 15 resections of single vertebral bodies, with homologous femoral head transplantation, for malignant and benign lesions of the spine. The advantages of this procedure are compared to fixation with bone cement, autologous bone grafts and other materials. The results are discussed.

Bone marrow obtained during hip surgery: a novel source for studies of hemopoiesis in human long-term bone marrow culture (LTBMC).

Studies with human bone marrow cells are often impaired by the poor quality of sternal aspirates due to varying numbers of contaminating blood cells before enrichment procedures and insufficient progenitor cell yields. In this study we report on experiments performed with human bone marrow cells isolated from a) spongiose bone fragments collected during hip surgery in patients with osteoarthritis or b) sternal aspirates. After Ficoll-Histopaque density gradient centrifugation absolute cell numbers were always lower in the samples obtained from sternal aspirates. However, both sources proved to yield the same proportions of the respective myeloid cell populations. Human long-term bone marrow cultures (LTBMC), semi-solid agar assays (CFU-GM day 14), and 3H-thymidine incorporation assays proved the comparability of the two bone marrow sources.

[Valgus osteotomy and lengthening of the femoral neck in secondary coxa vara in adolescents and adults].

Shortening of the femoral neck and proximal displacement of the greater trochanter are main problems of coxa vara after avascular necrosis of the capital epiphysis. In 24 patients an osteotomy to lengthen the femoral neck was performed to normalize the anatomic and functional situation. A clinical and radiological follow-up showed good or satisfactory results of the operation in 23 cases. Elimination or relief of pain and a negative Trendelenburg sign are important short-termed aims. Prevention or deceleration of degenerative changes in the hip joint are primary long-termed aims.

[2- to 5-year follow-up of cementless implantable knee joint prosthesis of the Miller-Galante type].

Based on a prospective study protocol, the two- to five-year results of the Miller-Galante cementless total knee arthroplasty are presented. Sixty-four implants were placed in 59 patients: 60 implantations were cementless and 4 cemented. Clinically, the scores for pain, range of motion, walking and stair climbing improved significantly. Radiographically, the uncemented components displayed no signs of definite or possible loosening, whereas one of the cemented components was found to be definitely loose. On histological evaluation of the retrieved implants, all components had bone ingrowth.

[Wound secretions in standard orthopedic surgery interventions using a new kind of closed vacuum drainage. A report of experiences].

One year's experience with 400 closed Redyrob drainage systems in 267 male and female patients has yielded encouraging results, which led to the replacement of the drainage system formerly used in the orthopaedic university clinic in Marburg by the new system. This makes it possible to regulate the suction strength between 0 and 900 mbar and to couple two suction tubes with one container. Drainage without suction and changing of the containers, which may be necessary in the case of large wound surfaces, can be performed without opening the system, which would endanger both patients and clinic personnel. Additional containers were only needed in 2.3%, because of the better filling volume. Thus, the new drainage system also proved to be superior with respect to cost efficiency and disposal. The total amount of wound secretion never exceeded 1620 ml, depending on the type of surgery performed, and there were no cases of infection or bleeding complications. The suction was regulated in 74% of all systems, and after most of the operations for insertion of endoprostheses. The handling of the systems was classed by the staff on the ward as "very good" or "good" in 96% of all cases. These results are discussed in the light of important aspects of wound drainage already mentioned by other authors.

Automated percutaneous lumbar discectomy with and without chymopapain pretreatment versus non-automated discoscopy-monitored percutaneous lumbar discectomy. An experimental study in human cadaver spines.

Percutaneous lumbar discectomy has gained growing interest during recent years as an alternative to open surgery for protrusions and non-sequestrated subligamentous intervertebral disc herniations. As a less invasive method it competes with chemonucleolysis. At least two modifications are known to date: automated percutaneous lumbar discectomy (APLD) with a 2-mm suction probe and non-automated, discoscopy-monitored percutaneous lumbar discectomy with a suction rongeur and a motor-driven shaver (NAPLD). In this study these two methods are compared for the amount of material extracted, dependence upon the degree of degeneration of the disc and intrinsic technical problems, using 20 human cadaver lumbar specimens for experimental testing. Total nucleotomy was not possible with either method. APLD yielded significantly less material and proved to be less effective in severely degenerated intervertebral discs than the NAPLD procedure, as preexisting gaps within the degenerated nucleus pulposus allowed the tissue to shift away from the tip of the 2-mm probe and facilitated displacement of the probe within the anulus fibrosus. By contrast, the rongeur, which first cuts the material to be removed and then carries it away by suction, was much more effective. Further data to support the advantages of non-automated percutaneous nucleotomy are discussed. Pretreatment of the disc with chymopapain did not result in a higher yield of nucleus material when combined with APLD.

Ultrasound examination for the early determination of dysplasia and congenital dislocation of neonatal hips.

Hip sonography enables an accurate and clinically relevant evaluation of hip maturation during the first days of human life. Experience has shown that an integration of hip sonography into neonatal screening programs is useful and necessary because clinical and even roentgenographic examination does not always establish a confirmed diagnosis of dysplasia.

Dumb-bell ganglioneuroma of the spine misinterpreted as progressive idiopathic scoliosis. Case report.

A giant ganglioneuroma generating a progressive scoliosis in a 16-year-old girl is presented. The interval between the start of the orthopaedic treatment and the diagnosis of the true nature of the disease was more than 4 years, thus allowing the development of a giant partly intracanalicularly partly retroperitonealy expanding tumor mass. The report emphasizes and describes the combined neurosurgical, general surgical and orthopaedic surgical treatment and presents the results of light- and electron microsopical, immunohistochemical and quantitative neurochemical investigations of the resected tumor.

Rapidly developing scoliosis in an adolescent due to spinal osteoblastoma. A case report.

A benign osteoblastoma of the spine should be considered a possible cause of painful scoliosis, especially in young adults. This report concerns a 17 year old patient with a lesion of the second lumbar vertebra where her painful progressive scoliosis was mistaken as idiopathic. Considering pain as a common presentation for the osteoblastoma, the diagnosis can be made early through X-rays, bone scan and computed tomography. The only adequate therapy is total removal of the tumor and the surgeon should make every effort to resect this lesion in sano to achieve permanent relief of pain and full alignment of the spine.

[Therapy of bone and joint changes in Albers-Schönberg osteopetrosis].

The history, genetic heterogenity, clinical appearance, radiologic morphology and specific pathology of Albers-Schönberg disease are described, with special reference to its orthopedic aspects, on the basis of 14 case histories. The orthopedic problems (osteomyelitis, delayed fracture and osteotomy healing, growth deviations of long bones, spinal pathology, arthrosis and fatigue fractures) are described in detail and documented. The disease is often recognized only by chance when routine X-ray examinations are performed. More serious manifestations of the disease, however, such as spontaneous osteomyelitis, fatigue fractures, coxarthrosis or gonarthrosis with or without growth aberrations, pose difficult problems of treatment. Delayed callus formation is routinely observed; this demands special osteosynthesis techniques must be constantly borne in mind during and after operative care.

[Osteosyntheses and replacement of vertebral bodies in spinal tumors].

The modern concepts for resection and stabilization of primary and metastatic spinal tumors are described in detail. The indications and operative procedures, including the current concepts of stabilization (osteosynthesis, autologous, homologous and artificial vertebral body replacement materials), are presented with special reference to applications to the cervical, thoracic, and lumbar spine. Indications for operative treatment in metastatic spine involvement are especially emphasized and documented with clinical examples. The paper includes an extensive list of recent publications.
