[Recurrent febrile infections of the urinary tract and diverticulum of the urachus].

The urachus can present a partial or complete obliteration defect. If the defect is situated in its vesical extremity, it then forms a diverticulum. The authors report a case of diverticulum of the urachus in a young adult, responsible for recurrent febrile urinary tract infections since the beginning of adolescence. The diagnosis was suggested by ultrasonography and magnetic resonance imaging and confirmed by cystoscopy and histological examination. Treatment consisted of complete surgical resection via a suprapubic incision.

[Palliative care of tumors of the excretory urinary tract].

The aim of palliative care for urinary tract cancer is to preserve the quality of life. Various urological disorders may need care: hematuria, micturition trouble, upper urinary tract obstruction complicated with pain or renal insufficiency. Endourological procedures have widely improved therapy. Physical pain and moral pain are considered as a global approach. Prescription of antalgic drugs and the progression of their dose are best managed when palliative care unit and urological unit act as a both.

[Subcutaneous administration of interleukin-2 in ambulatory treatment of patients with metastatic renal cancer. Three-year results of the SCAPP I program].

We report a french experience of subcutaneous administration of interleukin-2 in treatment of patients with metastatic renal cell carcinoma. Thirty-nine patients with metastatic renal cell carcinoma were included in the study. During the 10-week induction period, interleukin-2 was administrated subcutaneously 5 days a week for 8 weeks. The weekly dosage were 90 MIU during weeks 1 and 6; 63 MIU during weeks 2 to 4 and 7 to 9. After evaluation, responders and patients with stable disease received maintenance treatment which was discontinued upon the appearance of disease progression or unacceptable toxicity. During the maintenance period, interleukin-2 was administered 5 days a week for 4 weeks followed by a 2-week rest period. The weekly dosages were 90 MIU in week 1 and 63 MIU in weeks 2 to 4. After completion of induction treatment, 7 of 39 evaluable patients (18%) had objective responses with 1 complete response. A diminution of dose or interruption of treatment occurred with 7 patients because severe toxicity. Other systemic side effects in the remaining patients were acceptable. Seventeen patients received maintenance treatment. The median follow-up of all the patients included was 21 months. The 1, 2 and 3 years survivals were 64%, 33% and 22% respectively. This multicentric trial confirms the efficacity of subcutaneously-administered interleukin-2 in patients with metastatic renal cell carcinoma in terms of both response rate and survival. Unfortunately, increasing total doses of administrated interleukin-2 does not seem to increase efficacity according to response rate, but is more toxic.

[Intradiverticular bladder tumors. Five case reports].

OBJECTIVE: To define the diagnostic and therapeutic approach to intradiverticular bladder tumour. METHODS: Five cases are reported in which initial diverticulectomy was followed by cystectomy in one case, systemic MVAC chemotherapy in 2 cases and concomitant radiotherapy-chemotherapy (CRC) in 2 cases. RESULTS: One patient has died (MVAC), one patient is in partial remission (MVAC) and three patients are in complete remission (1 cystectomy and 2 CRC). CONCLUSION: Intradiverticular bladder tumour is a disease with a poor prognosis, whose histological features require aggressive treatment. Concomitant radiotherapy-chemotherapy appears to be a useful alternative.

[Evaluation of clinical competence in urology: innovative approach based on performance observation].

OBJECTIVE: The authors present a pilot project for the evaluation of clinical skills in urology using a method of evaluation based on observation of real performance. MATERIAL AND METHODS: An objective and structured clinical examination (OSCE) applied to urology was developed according to a precise predetermined design: 1) Identification of the objectives to be evaluated. 2) Choice of sampling of clinical situations representative of routine urological practice. 3) Construction, on the basis of these cases, of physician-patient interaction stations and question stations, with, for each case, weighting of objectives, construction of observation grids and writing of instructions for candidates, simulated patients and observers. RESULTS: An OSCE circuit of 10 clinical cases and 16 stations was constructed. The main poles of activity and urological settings were represented. Objective complementary investigations, diagnosis and treatment received the highest weightings. The reliability coefficient, the content validity and the construct validity will be verified on the basis of the overall score obtained by candidate. CONCLUSION: Establishment of the score and the expected results in terms of reliability, validity, and feasibility are discussed. The psychometric qualities of the OSCE have been demonstrated. Although a single instrument is not sufficient, it is currently the measuring tool which most closely approximates ideal evaluation of clinical skills. If the feasibility of this method is confirmed, this pilot project in urology could provide a new approach to evaluation strategies and could participate in the current reflection concerning urology training.

[Value of helical CT scan in the preoperative assessment of the ureteropelvic junction syndrome].

INTRODUCTION: The search for anatomical variants of the renal blood supply is an important element in the choice of surgical technique for the treatment of ureteropelvic junction syndrome. MATERIAL AND METHODS: Prospective study of 17 patients (mean age: 33 years), assessed by spiral CT with acquisition of overlapping thin sections during the arterial phase. The renal blood supply anatomical findings were then compared with operative findings in all patients. RESULTS: We observed a solitary renal artery in 8 cases, a lower pole pedicle crossing the junction in 9 cases and vascular abnormalities in 4 cases. All of these radiological findings were confirmed intraoperatively. Spiral CT, with rapid acquisition during the arterial phase and overlapping thin sections, allows accurate analysis of the renal blood supply. Axial sections are sufficient to provide the necessary anatomical information. Three dimensional reconstructions allow a better spatial visualization of the various structures. CONCLUSION: This examination allows visualization of a possible lower pole artery associated with UPJ syndrome, with identical accuracy to that of renal arteriography, but constitutes a less invasive and less expensive investigation.

[Ultrasonography of the urethra: prognostic value of the ultrasonographic structure of corpus spongiosum peristenotic fibrosis: prospective study of 33 patients].

OBJECTIVES: The contribution of ultrasonography to pretreatment morphological assessment of strictures of the anterior urethra and assessment of the risk of recurrence after internal urethrotomy. MATERIAL AND METHODS: 33 patients (16-89 years) operated by internal urethrotomy for stricture of the anterior urethra and followed for at least 6 months. Preoperative urethral ultrasonography, recording the number, length and degree of strictures and echostructure of the peristenotic fibrosis of the corpus spongiosium. RESULTS: Ultrasound visualization of all urethral strictures, with no false-positives and no false-negatives. 11 patients relapsed after a mean interval of 5.7 months (1-16 months), 22 patients did not present recurrence: mean interval: 15.5 months (6-36 months). Corpus spongiosum fibrosis associated with urethral stricture is isoechoic to the corpus spongiosum (19 cases) or hyperechoic to the corpus spongiosum (14 cases). No statistical correlation was observed between the echostructure of the fibrous tissue and the risk of recurrence after internal urethrotomy. CONCLUSION: Ultrasonography allows excellent analysis of the morphological characteristics of a stricture of the male anterior urethra. In our experience, and in contrast with the limited data of the literature, no correlation was observed between the echostructure of the peristenotic fibrosis and the risk of recurrence after internal urethrotomy.

Preliminary results of muscle cuff cervicoplasty in the ewe for the treatment of urinary incontinence.

OBJECTIVE: Muscle cuff cervicoplasty consists of dissecting a strip of the superior slip of the levator ani muscle, by sectioning it flush with the symphysis pubis, and applying it like a cuff around the urethra below the bladder neck. METHODS: We conducted a preliminary study in 12 ewes, including preoperative urodynamic assessment, intravenous urography with retrograde cystography on day 15, and another urodynamic assessment 1 and 3 months after the operation. Two animals died, but the other 10 animals were able to be evaluated. An increase in the urethral functional length with an infracervical plateau was revealed on all postoperative urethral profiles. Only transrectal electrostimulation, performed on the last 5 ewes, demonstrated an elevation of the closure pressure by 5-10 cm H2O. At sacrifice of the animals at 4 months, in situ stimulation of the muscle strip induced muscle contraction in 7 of 10 cases. Histological examination demonstrated persistence of muscle fibers in the cuff, despite the presence of marked fibrosis. RESULTS: The preliminary results are encouraging: urinary continence was improved with a minimal risk of retention, and cervicoplasty appears to retain its contractile activity, suggesting the possibility of a long-term sphincteric and proprioceptive action. CONCLUSION: This technique could be proposed in women suffering from urinary stress incontinence with sphincter hypoactivity.

Results of artificial sphincter in severe anal incontinence. Report of 14 consecutive implantations.

PURPOSE AND METHODS: Fourteen AMS 800 (American Medical Systems, Minneapolis, MN) urinary artificial sphincters have been consecutively implanted in 13 patients with total incontinence for stool of various causes (traumatic or postoperative, 7; congenital, 3; neurologic, 2; idiopathic, 1). No proximal stoma was constructed but was already present in one patient before implantation. RESULTS: Sepsis occurred in two patients. Removal of sphincter and colostomy was necessary in three patients: one of these two had developed sepsis, one had perineal ulceration before activation of the sphincter in a severely scarred perineum, and one had severe pain in a radiation-injured anorectum. Sphincter-related failure occurred once by rupture of the cuff in a constipated woman after two years of satisfactory function. Reimplantation of a new cuff restored normal continence in this patient. After median follow-up of 20 (range, 4-60) months, nine of ten patients with a functioning sphincter were continent for stool, and five were also continent for gas. Failure occurred in one patient because the cuff was too large to occlude the anal canal. This patient is awaiting reimplantation. Four patients experienced easily controlled difficulties with evacuation of feces. Anal pressure with inflated cuff varied from 43 to 94 (mean, 58 +/- 12) cm H2O. CONCLUSION: These results show that an artificial sphincter has a role in the treatment of severe anal incontinence when local therapies are not applicable or have failed.

Subcutaneous recombinant interleukin-2 (rIL-2) in out-patients with metastatic renal cell carcinoma. Results of a multicenter SCAPP1 trial.

BACKGROUND: This multicenter phase II trial was conducted in order to evaluate the efficacy and toxicity of the subcutaneous route of administration of rIL-2 in the treatment of patients with metastatic renal cell carcinoma and to check whether an increased cumulative dose of rIL-2 increases efficacy. PATIENTS AND METHODS: Thirty-nine patients with metastatic renal cell carcinoma were included in this study. During the induction period, rIL-2 was administered subcutaneously 5 days a week for 8 weeks. The weekly dosages were 90 MIU during weeks 1 and 6;63 MIU during weeks 2 to 4 and 7 to 9. After evaluation, responders and patients with stable disease received maintenance treatment which was discontinued upon the appearance of disease progression or unacceptable toxicity. During the maintenance period, rIL-2 was administered 5 days a week for 4 weeks followed by a 2-week rest period. The weekly dosages were 90 MIU in week 1 and 63 MIU in weeks 2 to 4. RESULTS: After completion of induction treatment, 7 of 39 evaluable patients (18%) had objective responses (95% CI: 9% to 37%) with one complete response. Treatment was interrupted or reduced due to toxicity for seven patients: Neuropsychiatric symptoms (3 patients), joint pain (1 patient), major asthenia and anorexia (1 patient), stroke (1 patient), and septicemia (1 patient). Other systemic side effects in the remaining patients were acceptable. Seventeen patients received maintenance treatment. In none of the patients did the response status improve during this maintenance period. The median follow-up of all of the patients included was 19 months. The one- and two-year survivals were 65% and 33%, respectively, ad the median duration of response was 11 months (5 to 16+). CONCLUSIONS: This multicentric study confirms the efficacy of subcutaneously-administered rIL-2 in patients with metastatic renal cell carcinoma in terms of both response rate and survival. The role of a maintenance therapy needs further evaluation.

[Diagnostic value of Stamey's test in chronic prostatitis].

OBJECTIVES: Analysis and technical details of Staomey's fractionated urine cultures, the reference method for the evaluation and bacteriological diagnosis of chronic bacterial prostatitis, but the application of which has been rarely described in the literature. METHOD: Stamey's test is based on comparative analysis of urine samples representative of the urethra, bladder and prostatic secretions obtained by prostatic massage. This method allows the demonstration of bacteria in the urine or prostatic secretions in the presence of bacterial prostatitis, in contra with non-bacterial prostatitis and prostatodynia. The sampling conditions require a sufficiently full bladder and the samples must be collected according to rigorously sterile procedure. The first step of the examination must not be preceded by urethral swabbing and the urine samples must have a well defined volume. Prostatic secretions are obtained by a prolonged massage of each lobe of the prostate gland. RESULTS: Bacterial prostatitis is confirmed by the presence of bacteria in the prostatic secretions and U3 in numbers largely exceeding the bacterial counts of the other samples. In the case of lower urinary tract infection, the test must be repeated after disinfection of the bladder urine. The pathogenic role of Gram positive bacteria is confirmed by isolation of a high bacterial count on several occasions. The pathogenic role of Ureaplasma urealyticum and Chlamydiae trachomatis is more controversial, while the role of Trichomonas vaginalis is unlikely and fungal prostatitis is very rare. Semen culture is less reliable than Stamey's test in the diagnosis of prostatitis. CONCLUSION: The diagnosis of chronic prostatitis is difficult due to the absence of typical clinical symptoms, specific ultrasonographic signs and the sometimes difficult interpretation of the culture results. Stamey's test is a reference diagnostic examination provided it is performed according to a rigorous methodology.

[Spontaneous perirenal hematoma: diagnostic and therapeutic approach].

OBJECTIVE: Spontaneous perirenal haematomas are relatively rare. The authors report 6 recent cases and try to propose a diagnostic and therapeutic strategy based on the aetiology. MATERIAL AND METHODS: From January 1993 to May 1995, 6 patients between the ages of 36 and 61 years were admitted to hospital for spontaneous perirenal haematoma, presenting in the form of acute lumbar pain associated, in one half of cases, with signs of internal bleeding. RESULTS: In each case, CT examination allowed precise assessment of the haematoma, but the aetiology was determined preoperatively in only 3 out of 6 patients, corresponding to a neoplastic lesion in each case (1 corticoadrenaloma, 1 adenocarcinoma, 1 angiomyolipoma). All patients were operated (3 radical nephrectomies, 2 haemostatic nephrectomies, 1 drainage of the haematoma with biopsies). No cause was found in 2 cases. CONCLUSION: Spontaneous perirenal haematomas essentially raise the problem of the aetiological diagnosis, but can sometimes be inadequate. Arteriography is useful when CT scan is negative or in the case of vascular disease. The therapeutic attitude, nephrectomy or conservative treatment remains controversial, but in view of the frequency of renal neoplastic lesions, the authors recommend nephrectomy at the slightest doubt concerning renal integrity. When no cause can be found, the assessment must be completed postoperatively with long-term, close surveillance, due to the risk of an undiagnosed neoplastic lesion.

Cefixime penetration in human renal parenchyma.

The diffusion of cefixime, a new orally active expanded-spectrum cephalosporin, was studied in 12 patients undergoing nephrectomy after receiving 200-mg oral doses every 12 h for 2 days. The patients were divided into two groups according to the time of perioperative sampling after the last dose: 4 h (group 1) and 12 h (group 2). Preoperative blood samples were taken just before administration of the last dose of cefixime. Simultaneous blood and tissue samples were collected perioperatively at 4 h (time to peak level) and 12 h (residual level). The intrarenal concentrations of cefixime were measured by an isocratic reversed-phase high-pressure liquid chromatographic (HPLC) assay. Concentrations in serum were determined by both microbiological and HPLC assays. The mean peak levels in serum were 3.41 +/- 0.43 micrograms/ml (group 1), and the residual levels averaged 1.54 +/- 1.17 micrograms/ml (group 2). The diffusion in renal parenchyma was not significantly different in the cortexes and medullas of both groups of patients: 5.7 to 6.4 micrograms/g in group 1 and 4.6 to 4.7 micrograms/g in group 2. The decrease in cefixime concentrations was slower in tissue than in serum; the ratios of concentrations in tissue to those in serum were 3.5 to 3.6 and 1.7 to 1.9 at 4 and 12 h, respectively. The intrarenal concentrations of cefixime remained higher than the MIC for the most susceptible gram-negative bacteria during the time interval between the administration of two doses.

[Laparoscopic drainage of a lymphocele after radical prostatectomy].

A laparoscopic procedure used to achieve transperitoneal drainage of a lymphocele that developed three months after radical prostatectomy for prostate cancer is described. The lymphocele wall was resected through a peritoneal opening. After drainage of the lymphocele, a corner of the omentum was inserted into the cavity. Preoperative aspiration of the lymphocele and computed tomography allowed accurate evaluation of the relationships of the lymphocele with the blood vessels, urinary tract, and digestive tract. These investigations also ruled out an infection and established the diagnosis of lymphocele. Laparoscopic drainage has the same indications as conventional internal drainage and readily allows to perform the same procedures. As a conventional internal drainage and readily allows to perform the same procedures. As a simple procedure associated with low morbidity, laparoscopic drainage could be used as the first-line treatment for large, symptomatic, recurrent lymphoceles developing after pelvic surgery, in particular after extraperitoneal procedures such as radical prostatectomy, provided there is no infection, the lymphocele is readily accessible, and the surgeon is experienced in the use of laparoscopic techniques.

[Renal metastases from cancer. Apropos of 9 cases and review of the literature].

We report 9 cases of secondary carcinomas of the kidney. The primary source of cancer was found in the lungs (7 cases), in the larynx (one case) and in the pyriform sinus. Clinical or radiologic signs of these renal metastases were comparable to primary carcinomas of kidney. The mean survival outlook for these patients was 8.1 months. Radical nephrectomy was performed in five cases. In a patient previously treated for a primary carcinoma and especially for a lung carcinoma, when a solid renal mass of kidney is discovered the hypothesis of a renal metastasis must be kept in mind. Because of lack of specific feature of these tumours and because of their often poor prognosis further investigations such as percutaneous fine needle aspiration for cytopathologic study must be considered.
