Mild ventriculomegaly as a counselling challenge.

OBJECTIVE: Our purpose was to evaluate the outcome of a group of fetuses with mild ventriculomegaly. METHODS: We retrospectively collected all cases of antenatally diagnosed mild enlargement of the lateral cerebral ventricles (transverse diameter of the atrium between 10 and 15 mm) between 1992 and 1997. Cases were included in the study if no other ultrasonic anomalies (including soft markers) were found. TORCH screening, karyotyping, search for associated anomalies and neurological examination (including imaging) were performed. Outcome information was available for all cases up to 30 months. RESULTS: Fourteen cases were examined. TORCH was always negative, 1 case with a microdeletion of chromosome 1 was identified. During the course of pregnancy, 2 fetuses showed progression to frank hydrocephalus. Five cases of agenesis of the corpus callosum, 1 of lissencephaly and 1 of heterotopia were eventually diagnosed. After birth 2 syndromes were identified. Three babies died during the follow-up period, among the survivors 4 had severe neurological damage, 2 of them had no associated dysmorphic findings. DISCUSSION: A heterogeneous group of central nervous system disorders may manifest antenatally as mild ventriculomegaly. The current management may not be able to identify dangerous conditions that present subtle dysmorphic features, or be able to predict abnormal outcome in most of the cases.

[Ehlers-Danlos syndrome and pregnancy. Report of a case with favourable outcome].

Ehlers-Danlos syndrome is a heterogeneous group of diseases, where connective tissue laxity is the main feature. Several pregnancy and delivery complications, mainly related to vascular and uterine rupture or to soft tissue tears, are described. A case of a full term pregnancy is reported and the clinical value of obstetrical management is discussed.

[Ultrasonic assessment of the state of the cervical canal at the term of pregnancy].

BACKGROUND: To assess if ultrasonic measurement of the cervical canal has carries, at term of pregnancy, a relationship with the onset of labour. To assess if cervical length identifies women at risk to deliver beyond 42 completed weeks of gestation (287 days). METHODS: Prospective case-control study, carried out between 37 and 40 weeks of gestation in uncomplicated, singleton, cephalic presentation, pregnant women. SETTING: outpatient ultrasound Department of a tertiary referral centre, serving > 1000 pregnancies. Weekly measurements of the cervical canal by transvaginal ultrasound probe. RESULTS: Inverse relationship between sonographic measurements and advancing gestational age. At 38 weeks of gestation, significant difference in the ultrasonic recordings between patients delivered before and beyond 41 completed weeks. CONCLUSIONS: It is possible to identify as early as 38 weeks of gestation those women that will deliver beyond 41 weeks. There is a subgroup of nulliparous patients that will probably be candidates to induction of labour.

[Rhabdomyolysis in meningoencephalitis caused by echovirus].

We report one case of atraumatic rhabdomyolysis associated with Echo group viral encephalitis, not complicated by acute renal failure. Clinical and bio-humoral outlines are described, characterized by favourable evolution despite high muscular enzymatic peaks. We emphasize that the positive course is probably due to the early institution of prophylactic measures, and that, anyway, the subject still remains exposed to major rhabdomyolysis risk in the presence of stress factors, including general anesthesia.

[Idiopathic retroperitoneal fibrosis].

Retroperitoneal fibrosis is an uncommon disease, characterized by the replacement of normal retroperitoneal tissue with fibrosis and/or chronic inflammation. In two thirds of the cases retroperitoneal fibrosis is idiopathic (IRF), whereas in the remaining ones it is secondary/associated to cancer, infections, drugs, autoimmune disease and vasculitis. IRF appears as a dense, fibrous plaque that usually arises between the level of the lower aorta and the common iliac arteries. As the plaque progresses, it engulfs the adjacent structures (e. g., ureters). In its early stages IRF is characterized by a rich infiltrate of lymphocytes, plasma cells and macrophages interspersed within fibroblasts and collagen bundles. In its advanced stages it becomes relatively avascular and acellular with abundant collagen bundles and scattered calcifications. The pathogenesis is unknown: some Authors suggest that IRF is a consequence of a local autoimmune reaction against atherosclerotic plaque antigens whereas others propose that it is the manifestation of a systemic autoimmune disease. The presenting signs and symptoms are non-specific; systemic manifestations (fever, anorexia, weight loss), often associated with local symptoms, are usually found to be related to the entrapment of retroperitoneal structures. The most common local symptom is lumbar and/or abdominal pain. The treatment can be surgical and/or medical: the former is required when obstructive complications are present; the latter, associated or not with surgery, can significantly improve the outcome of IRF patients and usually modifies the natural history of the disease. Steroids and tamoxifen are the most used drugs, whereas other agents such as azathioprine, methotrexate and cyclosporine are usually given to non-responder patients.

Post-treatment residual tissue in idiopathic retroperitoneal fibrosis: active residual disease or silent "scar" ? A study using 18F-fluorodeoxyglucose positron emission tomography.

OBJECTIVE: Medical treatment is often effective in idiopathic retroperitoneal fibrosis (IRF) but frequently leads to residual retroperitoneal masses that may represent active disease or simply consist of inactive fibrotic tissue. 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET) is a functional imaging modality that reliably assesses disease activity in a number of inflammatory diseases including IRF. We used 18F-FDG PET to evaluate the metabolic activity of residual masses in a series of IRF patients. METHODS: We studied 7 consecutive IRF patients, all of whom presented constitutional symptoms and/or pain, and had high acute-phase reactant levels; 6 had ureteral involvement. IRF was diagnosed by means of computed tomography (CT), which revealed a peri-aortoiliac mass in all cases. Three patients underwent surgical ureterolysis and 2 received ureteral stents. Subsequently, 5 patients received prednisone, one sequential treatment with prednisone and tamoxifen, and one prednisolone plus methotrexate. All of the patients underwent 18F-FDG PET at varying times after the end of treatment. RESULTS: The presenting signs/symptoms improved in all patients and the levels of acute-phase reactants significantly decreased or normalised. Ureteral obstructive disease resolved in all cases. Post-treatment CT revealed a considerable reduction in the amount of IRF but all of the patients had a residual retroperitoneal mass. PET revealed slight aorto-iliac 18F-FDG uptake in only one patient; all of the others were negative. No patient relapsed during the follow-up. CONCLUSIONS: Post-treatment residual masses are frequent in IRF patients but, in most cases, probably represent metabolically inactive tissue.

[Opinions of women regarding a planned home birth project in Apulia (Southern Italy)].

A growing desire among women to move away from interventions and hospitals to more "natural" childbirth has emerged over several years. The role and autonomy of midwives, a favourable opinion among women about home childbirth and better cooperation between hospitals and district health facilities are all fundamental in order to implement a home birth project. The opinion of women about "planned" home birth was investigated in the Puglia region through a survey conducted among a sample of women who had just delivered in hospital and a representative sample of the general female population. From 20 to 30% of women in the Puglia region would be basically in favour of home birth project. The main difference between the two groups of women concern the higher proportion of caesarean deliveries among mothers who had just delivered and their demand for more specific training of involved health personnel. Both samples are agreed on the specific training of women during pregnancy and on the presence of a specialist during home delivery. The results of the investigation seem to be in favour of planning a home birth project in the Puglia region.

[A comparative study of skinfold thickness and bioelectric impedance analysis in the evaluation of body composition of children and adolescents].

A comparative study as carried out between skin-fold thickness tests and bioelectric impedance analysis (BIA) to assess the validity of the latter as a method for evaluating body composition in childhood and youth. The percentage values of total body fat obtained using both methods in 117 boys and girls aged between 8 and 14 appear to be significantly correlated (p less than 0.01) in both sexes (males: r = 0.891; females: r = 0.830) and in each group of subjects subdivided according to stage of puberal development (8-11 years: r = 0.905; 12-14 years: r = 0.864) or the degree of physical activity performed (active: r = 0.880; sedentary: r = 0.915). The results of the study confirm that BIA represents a valid and reliable method to analyse body composition even during circumpuberal development in phase opposition. Moreover, BIA provides an estimate of total body fat which is independent of the differences in distribution between surface and deep deposits.

Detection of Salmonella in dry foods using refrigerated pre-enrichment and enrichment broth cultures: summary of collaborative study.

A collaborative study was conducted to compare the productivity of refrigerated pre-enrichment and enrichment broth cultures with the U.S. Food and Drug Administration culture methods for detection of Salmonella. The refrigerated pre-enrichment and selective enrichment broth culture methods for detection of Salmonella in dry foods have been adopted first action by AOAC INTERNATIONAL.
