Pulmonary lesions due to opportunist mycobacteria. (Review includes 30 cases of M. kansasii infections).

The apparent increase in the incidence of opportunist mycobacterial pulmonary infection is assessed. The clinical, bacteriological and radiological characteristics of 34 cases of such infections seen over a period of ten years in Sheffield are reviewed. The typical radiological appearances consisting of cavities, fibrosis and opacities are described and illustrated. There has been in the past conflicting reports on the specificity of such radiological features; it is concluded that although such appearances are not pathognomonic, their presence should arouse suspicion of such an infection and this should be confirmed or refuted by bacteriological investigations.

Effect of cigarette smoking on competence of the pylorus: preliminary study.

The competence of the pylorus was studied in 13 normal volunteers and nine dyspeptic patients by means of a radiological technique involving intubation of the duodenum. The pylorus was found to be competent in most normal individuals and in fewer than half of the dyspeptic patients. However, on smoking a cigarette, appreciable increase in duodenogastric reflux was seen in nine out of the 13 normal volunteers and in seven out of the nine dyspeptic patients. Such induced pyloric incompetence and the resultant regurgitation of duodenal juices may explain the increased incidence of gastritis and gastric ulceration in heavy smokers.

Pyloric reflux and the healing of gastric ulcers.

Pyloric reflux is rare in healthy subjects but is common in those with benign gastric ulcer. Healing of a gastric ulcer is associated with a diminution of reflux, and occasionally the pylorus becomes fully competent.

Pyloric incompetence in chronic non-specific lung disease.

Of 19 patients with chronic non-specific lung disease, chosen without reference to dyspeptic symptoms, 17 (89%) exhibited duodenogastric reflux of barium indicating pyloric incompetence. In the one patient who proved to have a gastric ulcer the amount of reflux diminished when the ulcer healed. The degree of pyloric incompetence was not found to be related to the severity of arterial blood gas or pH disturbance or to the presence of right heart failure. All patients who exhibited reflux had evidence of obstructive airways disease and most were receiving bronchodilators.We believe that reflux of detergent bile into the stomach may explain the high incidence of dyspepsia and acute gastric ulceration in chronic non-specific lung disease.

Pyloric regurgitation and gastric ulcer.

Using a radiological test for pyloric regurgitation described by Capper, Airth, and Kilby (1966), it was confirmed that the pylorus is normally competent but incompetent in gastric ulcer and chronic alcoholic gastritis and often incompetent in chronic duodenal ulcer. The significance of pyloric regurgitation in the aetiology of gastric ulcer is discussed.
