[Diagnosis of ocular chlamydiosis by gene amplification (polymerase chain reaction or P.C.R.)].

Identification of conjunctival chlamydioses could be very difficult since this disease is extremely latent. Moreover previous instillation of topical antibiotic provokes a negativation of D.I.F. and of cultures. P.C.R. is very useful, it was demonstrative in 41 cases out of 47 cases; D.I.F. was positive only in 18 cases. P.C.R. is a very promising method to recognize chlamydiae; identification of different serovars is processing.

[Serological confirmation of HIV infections in hospital practice: analysis of indeterminate western blot results].

Among 770 Western blots for HIV-1 confirmation on sera from subjects at high risk for HIV infection, 4.3% (33 cases) were indeterminate. Isolated, stable, reproducible anti-gp 160 reactivity, highly suggestive of a nonspecific reaction, was found in 16% of cases. There were three other probably nonspecific patterns with anti-gp 160 and either anti-gp 41 or anti-gp 120 reactivities and thin, atypical bands. Two patterns with anti-p24 and either anti-gp 160 or anti-gp 120 reactivities were consistent with HIV-1 seroconversion. Reactivity directed solely against gag products was seen in 18% of cases. The repeat test, performed in 16 cases, showed an identical pattern in 3 cases, a modified pattern in 3 cases, negative results in 9 cases, and seroconversion in 1 case. No case of HIV-2 infection was detected. Indeterminate Western blot results reflect nonspecific reactivity in most instances but should nevertheless lead to the exclusion of technical artefacts, seroconversion, and HIV-2 infection.

Comparative study of in vitro inhibition of activation of the classical and alternative pathways of human complement by the magnesium and sodium salts of the anti-inflammatory peptide N-acetyl-aspartyl-glutamic acid (NAAGA).

The inhibitory activity of the sodium salt of the anti-inflammatory peptide N-acetyl-aspartyl-glutamic acid (NAAGA) on activation of the classical and alternative pathways of human complement was compared with that of the clinically used magnesium salt of NAAGA (NAAGA-Mg). Sodium salt of NAAGA (NAAGA-Na) inhibited both pathways of activation in a dose-dependent manner at concentration ranging from 1 to 10 mM by acting on formation and/or function of the C3 convertases as shown by the inhibitory capacity of the peptide on the release of the C3 cleavage fragment C3b and C3a. NAAGA-Na was as effective as NAAGA-Mg in inhibiting classical pathway activation at concentration above 10 mM. NAAGA-Na was more effective than NAAGA-Mg in inhibiting the alternative pathway since the sodium salt did not interfere with Mg-dependent formation of the alternative pathway C3 convertase.

Tolerance to or facilitation of pharmacological effects induced by chronic treatment with the beta-adrenergic stimulant clenbuterol.

The effect in mice of chronic administration of the beta-adrenergic agonist clenbuterol was studied using four pharmacological tests, two of which are predictive of antidepressant activity in man. The suppressive effect of acute administration of the beta-adrenergic agonist in two tests unrelated to antidepressant activity, locomotor activity and investigation of food, was attenuated after chronic treatment. However, in the two tests which are predictive of antidepressant activity, antagonism of reserpine induced hypothermia and potentiation of yohimbine toxicity, the effect of clenbuterol was greater following chronic treatment.

[Transurethral resection of the prostate. Our experience in a series of 244 cases. Mid-term results].

Two hundred and forty four patients were treated by transurethral resection of the prostate (68% for adenomas and 32% for epitheliomas) between June 1980 and August 1982. The indications for surgery are based on obstruction of the urinary outflow tract, and the weight of prostatic tissue, which can best be assessed by ultrasound. TURP is the technique of choice for small and medium-size prostates. Hemorrhage is the most frequent complication at operation. Resorption TURP syndrome is extremely rare if an isotonic liquid is used. Urinary tract infections are best treated preoperatively. 2.6% of the patients suffered from urethral stricture despite the use of a 24 French resectoscope and early removal of the catheter. The greater the surgeon's experience, the lower the incidence of definitive urinary incontinence.

[Our experience with the treatment of urethral stenosis in a series of 90 cases at the Strasbourg Hospital Center].

Ninety patients presenting with urethral stenosis were treated surgically at the Strasbourg Hospital, involving 100 internal urethrotomies and sixteen urethroplasties. The authors consider that short bulbous stenosis, with no previous treatment, should be treated endoscopically; that, if the stenosis recurs after two internal urethrotomies, or if the stenosis is infected or located in the pendulous or membranous urethra, a urethroplasty is indicated, as in cases of long bulbous stenosis; and that, even though the rate of complications is high following Johansson's urethroplasty, this procedure is indicated in cases of infection. The use of free full-thickness grafts led to a 100% success rate.

[Epidemiology of renal epithelioma in adults].

The frequency of renal epithelioma is very high in the Bas-Rhin department (France) (9,92 per 100 000 among the male population). The various risk factors are considered: the ethnic group, genetic factors, tobacco, alcohol; all these factors are not very important. But the intake of animal lipids and proteins could be an important factor to induce renal epithelioma.

[Personal experience of urate lithiasis (author's transl)].

In Alsace, urate lithiasis accounts for 32.7% of cases of lithiasis requiring hospitalisation. Fifty nine per cent of patients are hyperuricaemic, and only 10% have typical gout. 14.6% are hyperuricosuric but all have isoaciduria. A male disorder in 3/4 cases, it occurs above all after the age of 50. The relatively late onset, combined with marked obesity (mean weight 81.5 kg) is considered by the authors to be an argument proving the role of dietary excesses in the aetiology of this lithiasis. Excessive protein would seem to be the essential feature.

[Feasibility of radiological exploration of the main bile ducts for stones (author's transl)].

The results of cholangiography conducted pre-and post operatively (through Tatute), and during choledochotomy on 100 patients for confirmed or suspected bile duct stones are analyzed. Pre-operative opacification of the main bile ducts resulted in 13 p. cent of false positives and in a similar number of false negatives. Results of investigation during operation produced similar errors respectively in 14 and 2p. cent o of cases. Several recommendations are outlined to limit the proportion of unnecessary choledochotomies without increasing the number of neglected stones.

Systemic effects of colchicine on phosphate metabolism in innervated and denervated, slow and fast muscles of the rat.

A single systemic injection of 75 micrograms colchicine/100 g body weight increases the permeability to inorganic phosphate of both fast gastrocnemius and extensor digitorum longus muscles and of the slow soleus muscle. In the two fast muscles, there is a significant interaction between colchicine treatment and 5-d- or 30-d- surgical denervation. In the slow soleus muscle there is no interaction between 5-d-colchicine treatment and the initial decrease in phosphate flow due to 5-d-surgical denervation but a significant interaction between colchicine treatment and the secondary increase in phosphate permeability observed after a 30-d-surgical denervation. Thirty days after a single systemic injection of colchicine the muscle phosphate metabolism is still perturbed especially in the slow soleus muscle in which the initial decrease in radioactive uptake associated with a 5-d-surgical denervation is inapparent. It is proposed that colchicine induces denervation-like alterations in the Pi- and POAS metabolism of skeletal muscles through mechanisms which are also activated -- partly or entirely -- by surgical denervation. These alterations do not result from the interruption of the axonal flow. A direct effect on the muscle membrane seems less likely than a disinhibition or activation of the muscle protein synthesis system.
