[Auriculo-ventricular perforation in bacterial endocarditis. Surgical treatment].

The authors report a case of bacterial endocarditis during which the precise nature of the heart lesions were only discovered at operation. There left ventricle and right atrium. The characteristics of the systolic murmur and the findings on phonocardiography together with left ventricular angiography had suggested mitral incompetence. There were no conduction disorders in this patient which might have helped in preoperative diagnosis. In connection with this case, the authors recall the characteristics of aneuryms of the membranous septum and congenital or acquired communications between the left venticle and the right atrium.

[Salivary calcinosis and the immunologic context].

On the strength of three new cases, it is suggested that the term "calcifying parotiditis" must be replaced by "salivary calcinosis". This suggestion is based on clinical and pathological grounds. An analysis of 11 cases of salivary calcinosis encountered in the last eight years points to clinical analogies between this condition and Gougerot-Sjögren's disease as well as to the fairly frequent involvement of the immune system in the calcinoses.

An investigation of the complement system in patients with periodic disease (results from 29 cases).

The complement system was investigated in 29 patients suffering from authentic periodic disease. A statistically significant increase in C4, also in total complement and C3 could be demonstrated. It is possible that the increase in C4 was due to the macrophages which are always present in the infiltrates of periodic disease. This biological observation is of clear practical importance for the diagnosis of the condition both before and after colchicine therapy.

[Development and long term prognosis of lupus erythematosus (apropos of 124 cases)].

The authors report their experience of the course of lupus erythematosus. The ten year survival rate in 124 patients was 79.3 %. This good result is attributed to the effective treatment of the disorder using corticosteroids; often associated with ummuno-suppressants in severe cases. However, the side-effects of these drugs remain a source of concern and the ideal treatment for lupus has yet to be discovered.

[Disorders of intracardiac conduction during ankylosing spondylarthritis. Apropos of 19 cases].

The authors have collected 19 cases of ankylosing spondylitis with an alteration of intracardiac conduction. The lesions are usually situated high in the bundle of His, as shown by successive electrocardiograms and endocavitary studies. Progression by regressive acute episodes and the response to anti-inflammatory agents suggests that the disorder of conduction is inflammatory in origin. Syncopal attacks are rare (1 case out of 19) and a pacemaker is rarely indicated. Almost half the patients had aortic insufficiency, one patient had tricuspid stenosis and two patients had heart failure in the absence of any valvular lesion. The associated ankylosing spondylitis is characterised by the severity of the inflammatory signs (average sedimentation rate 50 mm in the first hour) and by the extent of peripheral articular involvement and extra-rheumatological manifestations. Almost one in two patients had iritis, with the same proportion applying to a past history of Reiters syndrome.

[Research of an association between HL-A antigens and systemic scleroderma].

49 unrelated subjects suffering from systemic scleroderma were typed for 28 HL-A antigens, without any particular significant association of an antigen with the disease or one of its manifestations being noted. In addition, 13 patients from the same family were genotyped for HL-A. Transmission of the disease through 4 generations does not seem to be linked to a particular haplotype and no pair of sibling HL-A identical patients were seen in the same generation. By contrast, two pairs of sibling patients were HL-A different. Nevertheless, other cases, and in particular familial, will be necessary before an association between the genes of susceptibility to S.S. and a gene in the chromosomal HL-A region may be definitely eliminated.

[Cardiovascular manifestations of pancreatitis. Apropos of 4 cases].

The authors report four cases of cardiovascular signs in pancreatitis in patients aged between 31 and 42 years. They then consider the main aspects observed. T-wave disorders, pericarditis, myocardial infarction and coronary heart disease and arterio-pancreatic syndrome. They consider the points in common between myocardial infarction and acute pancreatitis and then review the various pathogenic theories proposed.
