Effect of 24 hour fast in obese children.

Effects of a 24 hour fast were studied in 21 obese children aged 7 to 14 and in 8 controls. Mean blood glucose (BG) during fast dropped more in controls (0.88 to 0.54 g/l) than in obese (0.90 to 0.63 g/l) Plasma cortisol changes were similar in the 2 groups, FFA increased (p less than 0.01) in the 2 groups, but the 24 hour mean level was higher in controls (4.0 mEq/l) than in obese (2.06 mEq/l). At the end of the fast, a ketonuria was present in all obese children except 2. Serum alanine dropped similarly in obese (28 to 24 muM p. cent ml) and in controls (30 to 22 muM p. cent ml). All obese exhibited at the end of the fast a significant rise (p less than 0.01) of branched chain aminoacids, not observed in controls. Responses to glucagon (0.03 mg/kg I.M.) were studied before and after fast. At time 0, BG response was higher and more prolonged in obese in spite of hyperinsulinism. At time 24 hours, BG raised from 0.50 to 0.74 g/1 and insulin from 8 to 35 muU/ml in controls, while in obese BG raised from 0.63 to 1.06 g/l and insulin from 25 to 88 muU/ml. Concomitant hyperinsulinsim and biological criteria of hypoinsulinism demonstrated in obese children the peripheral resistance to insulin. The contrast between a normal degree of protein gluconeogenesis and a reduced rate of fat mobilization during fast may be a major biological feature of obesity in childhood.

[Semi-micromethod for the assay of vitamin E. Use in neonatal pharmacology].

A micromethod using 250 mul of plasma was described for assaying vitamin E. The complete extraction of the plasmatic vitamin E was made possible. The sensitivity and occurency of this method are particularly suitable in pediatric determinations. The vitamin E plasmatic levels were measured in the low birth-weight newborn given or not a 10 mg vitamin E supply in the feeding.

A randomized trial of team care following stroke.

A randomized controlled trial was conducted to examine the effects of interdisciplinary team care on acute hospitalized stroke patients. After obtaining baseline information on 42 stroke victims receiving conventional care in a general hospital, 130 stroke patients were stratified and randomly assigned either to Traditional or Team care. Assessments by independent evaluators permitted comparisons between Team and Traditional groups with reference to patient survival, motor performance and functional abilities. Data obtained prospectively from charts and treatment logs allowed the care process across groups to be compared. Results demonstrated that Team and Traditional patients fared similarly in survival. However there was an unexpected difference in survival depending upon sex. For motor performance, male survivors performed better with Team care and female survivors with the Traditional method. In terms of functional abilities, male patients receiving Team care again performed better than their Traditional counterparts, whereas in women there was no difference between the treatment groups.

[The efficacy of water ozonation on the inactivation of oocysts of Cryptosporidium].

Cryptosporidium oocysts are currently found in surface waters. Their incomplete removal by sand filtration and their high resistance to chlorinated disinfectants make possible waterborne cryptosporidiosis outbreaks after drinking treated water. Because ozone is an alternative to chlorine for final drinking water disinfection the aim of our study was to determine the ozonation conditions able to eliminate the infectivity of oocysts. Feces from horses naturally infected by C. parvum were mixed with distilled water. The obtained suspension was clarified by passing through a graded series of four sieves (pore sizes, 315, 200, 125 and 63 microns). The clarified suspension was decontaminated by adding a 10% sodium hypochlorite solution. After a 10 minute contact time at room temperature, the chlorine residual was neutralised by the exactly requisite volume of a 0.01 N sodium thiosulfate solution. Oocysts in the suspension were purified by centrifugation and washing of pellets in distilled water. Oocysts in the finally obtained suspension were numerated by counting in a Malassez haemocytometer. Ozonated water containing dissolved ozone residuals ranging from 0.44 mg/l to 1.09 mg/l were inoculated with a number of oocysts ranging from 1.10(6) to 1.10(8). Contact times with ozone were 4.6 or 8 minutes. After destroying the ozone residual by sodium thiosulfate, ozonated oocysts were administered by oral route to male Sprague-Dawley rats weighing between 180 to 200 g and beforehand immunosuppressed by a regimen of 25 mg hydrocortisone acetate subcutaneously twice a week, 5 weeks before and 3 weeks after Cryptosporidium challenge. Oocyst shedding was daily controlled by phase contrast microscoy in smears of fecal suspension mixed with carbolfuchsine solution.(ABSTRACT TRUNCATED AT 250 WORDS)

[Radiculomyeloencephalitis caused by Angiostrongylus cantonensis in children].

A case of radiculoencephalomyelitis caused by central nervous system involvement by Angiostrongylus cantonensis is reported in a 14 month-old girl. Evolution was spontaneously favourable, which is not usual in severe types. Lesions concerned the nevraxe and especially the spinal cord and are due to the migration of the larvae, creating eosinophilic inflammatory reactions.
