Conduct disorder with and without mania in a referred sample of ADHD children.

OBJECTIVE: To test the hypothesis that dysphoric and non-dysphoric types of CD could be distinguished from one another in their patterns of familiality, adversity, and comorbidity. METHODS: We examined 140 ADHD and 120 normal controls at baseline and 4 years later using assessments from multiple domains. We compared ADHD subgroups with and without conduct (CD) and bipolar (BPD) disorders on psychiatric outcomes at a 4-year follow-up, familial psychopathology and psychosocial functioning. RESULTS: We found that ADHD children with both disorders had higher familial and personal risk for mood disorders than those with CD only, who had a higher personal risk for antisocial personality disorder. Among ADHD probands, having both CD and BPD was associated with poorer functioning and an increased risk for psychiatric hospitalization. DISCUSSION: Although preliminary, our findings suggest that the distinction between dysphoric and non-dysphoric CD may be clinically meaningful. If confirmed, our findings could have important diagnostic and therapeutic implications for the management of antisocial youth.

Effect of increased maxillo-mandibular relationship on isometric strength in TMD patients with loss of vertical dimension of occlusion.

The effect on isometric strength of the shoulders and limbs while biting in habitual occlusion, on a bite-elevating appliance and on a placebo appliance was analyzed. Twenty female volunteer patients, presenting with temporomandibular pain dysfunction syndrome and obvious loss of vertical dimension, served as subjects. All were weaker to the manual application of the isometric Deltoid Press (IDP) when biting, as opposed to maintaining the mandible in an unsupported rest position. Two intraoral appliances were fabricated for each subject: a bite-elevating appliance (BEA) set by a functional criterion of peak strength to the IDP and a placebo appliance which did not interfere with occlusion but was "set" with a mock IDP procedure. Testing was carried out by the Neuromuscular Research Testing Laboratory of the Neurology Department of Tufts New England Medical Center. Testing was independent of the dentist who fabricated and set the appliances. A standard neuromuscular test with the Maximal Voluntary Isometric Contraction apparatus was used to assess strength of right and left shoulder, elbow and knee flexion and extension as is routinely performed with all neuromuscular disease patients. Twelve strength tests were carried out for each of three conditions: 1. Baseline-biting in habitual occlusion; 2. Elevated-biting on the BEA; and 3. Placebo-biting with the placebo appliance inserted. The order of conditions 2 and 3 was counterbalanced without knowledge of the subjects. Twelve repeated measures ANOVAs (each subject as their own control) were conducted for each of the 12 strength measures. All F-tests indicated a significant main effect for treatment differences (p < 0.0001). Mean strength biting on the BEA was consistantly greater (p < 0.001) than Baseline or Placebo strength. Baseline and Placebo condition were equivalent. These findings confirmed previous observations at this TMD Center: individuals with loss of vertical dimension of occlusion respond to a bite raising appliance by increased isometric-strength.

Stromal cell progesterone and estrogen receptors during proliferation and regression of the decidua basalis in the pregnant rat.

This study examines the distribution and abundance of progesterone receptors (PR) and estrogen receptors (ER) in the decidua basalis (DB) during proliferation (Days 8-12 of gestation) and regression (Days 14-21) in the rat. Stromal cells of the DB and metrial gland exhibited strong nuclear immunostaining for PR throughout gestation. Nuclear localization of ER was detectable only between Days 8-12. The heavily granulated natural killer cells were always negative for PR and ER. DB were dissected between Days 8 and 17 to measure progesterone (P4)-binding sites and receptor proteins by Western blotting. The latter revealed four specific PR isoforms: B (110 kDa), A1 (90 kDa), A2 (76-82 kDa), and C (60-64 kDa). Stromal cell nuclei contained more than 50% of P4-binding sites during DB proliferation but less than 22% during regression (p < 0.05). PR-A and PR-B expression was greatest at proliferative stages (p < 0.05). PR-C increased in relative abundance during DB regression. Two ER isoforms of 66 kDa and 49 kDa were revealed. The 66-kDa ER, the most abundant form, was maximally expressed during proliferation, declining 71% by Day 12 (p < 0.01), whereas the 49-kDa form accounted for up to 90% of ER during regression. Northern blot analysis revealed three prominent transcripts of approximately 11, 7, and 4 kilobases (kb) for PR mRNA, which declined markedly at Days 14 to 17 (p < 0.05), and one of 6.0 kb for ER mRNA, which declined markedly on Day 17 (p < 0.05). Our study establishes that the DB expresses heterogeneity of receptor message and proteins. We propose that preferential expression of receptor isoforms in late pregnancy limits P4 action and promotes DB regression in spite of invariant levels of serum P4, P4-binding sites, and total receptor protein.

Texas dental hygienists benefits and satisfaction survey.

The purpose of this study was to determine the satisfaction and fringe benefits of Texas dental hygienists and compare the data to a recent national study. A blind survey was forwarded to 5,294 active, licensed dental hygienists to obtain data for Texas. Responses were received from 2,172 dental hygienists, a 41 percent response rate. The greatest number of responses came from the larger cities in Texas: Dallas, Houston, Austin, and San Antonio. Fifty-eight percent of Texas hygienists receive paid vacation, 25 percent medical insurance, 37 percent sick pay, and 26 percent receive some type of retirement. Nationally, the figures are higher. The survey also sought to determine satisfaction levels. Most dental hygienists are satisfied with their salary (65%), their career (85%), and feel secure in their employment (92%), while only 46 percent are satisfied with benefits. The survey results are inconclusive for hygienists' in the state of Texas based on the 41 percent response rate. Practical information may assist employers in recruitment and retention of a dental hygienist into the practice.

Study of O-sialylation of glycoproteins in C6 glioma cells treated with retinoic acid.

When treated with retinoic acid in vivo, C6 glioma cells show an enhancement of CMP-Neu5Ac:Gal beta 1-3 GalNAc-R alpha-2,3 sialyltransferase activity. A 300 kDa glycoprotein was detected by lectin affinoblotting in retinoic acid-treated C6 cells which stained weakly or not at all in control cells. Comparative studies with different lectins demonstrated that this glycoprotein contains alpha 2,3 Neu5Ac Gal-GalNAc O-glycan moieties. Cultures in the presence of an inhibitor of O-glycan synthesis (N-acetylgalactosaminide alpha-O-benzyl) demonstrated that enhancement of staining of the 300 kDa glycoprotein was not due to the increase of the alpha 2,3 sialytransferase but to the de novo synthesis of the polypeptide chain of this glycoprotein.

The frequency response of ion channel currents in a combined diffusion and barrier type model.

The dynamics of ion transport through channels has been calculated using a small signal analysis method. This is applied to the equations describing the rates of ion transport through channels represented by a pair of entrance barriers in series with an interior diffusion regime. The responses to combinations of applied DC and AC electric fields, assumed to be constant in space, have been calculated and are represented as complex admittances. On examining the admittance properties, the channel is found to behave electrically as a double reactance network, which can give rise to resonance effects. A further interesting property is the limiting slope of the current amplitude at high frequencies, which is found to exhibit an f-2/3 dependence for small entrance barriers and an f-1 dependence for large entrance barriers. This points to the possibility of using noise measurements for drawing conclusions about ion transport properties through channels.

Regulation of the estrogen receptor in the decidua basalis of the pregnant rat.

This study examines the distribution of the estrogen receptor (ER) in the mesometrial decidua basalis (DB) and the chorioallantoic placenta between Days 8 and 21 of pregnancy (Day 1 = presence of vaginal sperm) and its regulation by estradiol and progesterone. Immunocytochemistry revealed that ER was localized within nuclei of cells of the DB but not in trophoblastic cells (Day 10) or in cells of the junctional zone (JZ) or labyrinth zone (LZ). Western blot analysis and estradiol binding assays of DB, JZ, and LZ also revealed that only DB expressed ER. Native ER (66 kDa) was most abundant on Days 8 and 9, and declined 67% on Day 11 (p < 0.01), becoming barely detectable by Day 17. A truncated ER moiety (49 kDa) gradually increased, becoming the dominant form on Day 13. The effects of estradiol and progesterone on ER were studied during periods of growth and decline of DB (i.e., Days 8-10 and 12-14, respectively). Rats were ovariectomized on Day 8 or 12 and treated with estradiol daily (0.2, 0.75, or 2 micrograms, s.c.), with a progesterone pellet, or with both, for 48 h. Progesterone, but not estradiol, stimulated the 66-kDa ER moiety and ER binding activity (p < 0.01). Estradiol administered with progesterone antagonized progesterone action, at least in part, by enhancing expression of the 49-kDa ER at the expense of the native form (p < 0.01). Thus, progesterone up-regulated ER whereas estradiol down-regulated ER in rat DB.

Ovine interferon-tau regulates expression of endometrial receptors for estrogen and oxytocin but not progesterone.

Ovine interferon-tau (oIFN-tau) may stabilize endometrial progesterone receptor (PR) and/or inhibit estrogen receptor (ER) gene expression during pregnancy recognition to suppress endometrial oxytocin receptor (OTR) formation and production of luteolytic prostaglandin (PG) F2 alpha pulses. This study determined whether or not oIFN-tau stabilized PR expression in the endometrium during PR down-regulation by continuous exposure to progesterone. Twenty cyclic ewes were bilaterally ovariectomized and fitted with uterine catheters on Day 2 of the estrous cycle (Day 0 = estrus). Ewes were then assigned randomly to be treated, in a 2 x 2 factorial arrangement, with recombinant oIFN-tau (roIFN-tau; 2 x 10(7) antiviral units per ewe per day) or control proteins (6 mg/day) by intrauterine injection from Days 10 to 14, and with daily i.m. injections of 20 mg progesterone from Days 2 to 14 (P) or progesterone from Days 2 to 14 plus 50 micrograms estradiol-17 beta from Days 12 to 14 (P+E). All ewes were hysterectomized on Day 15. Endometrial PR mRNA (p < 0.01) and protein (p < 0.03) were higher in ewes receiving P+E than in those receiving P alone. However, the increase in PR mRNA and protein was not as great in the endometrium of roIFN-tau-treated ewes as compared to controls (p < 0.08, treatment x steroid). In ewes receiving P alone, PR mRNA and immunoreactive PR were localized to stroma and deep glandular epithelium and were not present in endometrial luminal and shallow glandular epithelium. Values for endometrial ER mRNA (p < 0.02) and ER protein (p < 0.01) were greater in controls than in roIFN-tau-treated ewes regardless of steroid treatment. Among controls, ER mRNA and immunoreactive ER protein were present in the luminal and glandular epithelium and were increased in the epithelium and stroma in ewes receiving estrogen. In contrast, endometrial ER mRNA and immunoreactive ER protein were very low or absent in the endometrium of roIFN-tau-treated ewes and were not increased by estrogen. Among controls, endometrial OTR density was greater (p < 0.09) in ewes treated with P+E than in those treated with P alone. In roIFN-tau-treated ewes, endometrial OTR density was lower (p < 0.01) than in the controls. Results indicate that roIFN-tau did not stabilize or prevent autologous down-regulation of PR mRNA or protein expression in the endometrium. However, roIFN-tau did suppress endometrial ER expression and OTR formation in ewes regardless of steroid treatment. The results support the hypothesis that the antiluteolytic effects of oIFN-tau are to suppress endometrial ER gene expression in the endometrial epithelium, thereby inhibiting formation of OTR and production of luteolytic PGF2 alpha pulses.

Fetal bifid sacrum artifact: normal developmental anatomy simulating malformation.

PURPOSE: To determine the clinical importance and origin of a bifid configuration of the fetal sacrum seen during routine fetal sonography. MATERIALS AND METHODS: An apparent bifid malformation of the sacral spine was seen in 24 fetuses at antenatal sonography. An attempt was made to recreate this configuration prospectively in 111 consecutive second- and third-trimester fetuses. Three cadavers were also imaged to determine the origin of this configuration. RESULTS: Outcome information was available for 22 of the 24 fetuses; all fetuses were normal. In the prospective study, the bifid sacrum configuration was recreated in 75 of the 111 fetuses studied. The configuration could never be produced earlier than 20 weeks gestational age, was seen in some fetuses at 20-25 weeks, and could always be seen after 25 weeks. Findings from imaging studies of the three cadavers revealed that this configuration was dependent on the demonstration of the alar ossification centers of the sacral vertebra. CONCLUSION: The bifid sacrum artifact is a skewed representation of normal anatomy and should not be interpreted as a true anomaly.

Ovine interferon-tau inhibits estrogen receptor up-regulation and estrogen-induced luteolysis in cyclic ewes.

This study determined whether intrauterine injection of interferon-tau (IFN tau) could block luteolysis in cyclic ewes treated with a luteolytic dose of 17 beta-estradiol benzoate (E) on day 12 of the estrous cycle. Thirty-two ewes were fitted with uterine catheters on day 5 of the estrous cycle and treated with recombinant ovine IFN tau (2 x 10(7) antiviral units/ewe/day) or control proteins (6 mg/day) by intrauterine injection from day 10 until hysterectomy. At 1900 h on day 12, all ewes received 750 micrograms E, im, and were hysterectomized 12, 24, 36, or 48 h post-E administration. Plasma concentrations of progesterone declined in control animals but increased in IFN tau-treated ewes after E injection (P < 0.01, treatment x day interaction). Likewise, total corpus luteum weight decreased in control but not IFN tau-treated ewes after E administration (P < 0.02, treatment x time interaction). In control ewes, endometrial estrogen receptor (ER) messenger RNA (mRNA; P < 0.03) and progesterone receptor (PR) mRNA (P < 0.10) increased after 12 h, whereas concentrations of ER protein (P < 0.02) and PR protein (P < 0.04) increased after 24 h. In situ hybridization and immunohistochemical analyses indicated that ER gene expression increased first in the epithelium at 12 h and then in the stroma by 48 h, whereas PR gene expression first increased in the stroma and then in the epithelium. In control ewes, endometrial oxytocin receptor (OTR) density increased (P < 0.10) after 12 h, with the largest increase occurring between 36-48 h. In IFN tau-treated ewes, endometrial ER mRNA and protein and OTR density did not increase after E administration. Levels of PR mRNA increased (P < 0.01) between 12-36 h, but decreased after 36 h. PR mRNA abundance increased between 12-36 h in the stroma, but not in the epithelium. Concentrations of PR protein were low and did not change in IFN tau-treated ewes. Immunoreactive PR protein was present at low levels in the stroma of all IFN tau-treated ewes. The results indicate that induction of luteolysis by E in control ewes involved sequential increases in endometrial ER mRNA and ER protein in the epithelium that preceded maximal increases in OTR density. Intrauterine injection of recombinant ovine IFN tau prevented luteolysis by inhibiting estrogen-induced increases in endometrial ER and OTR gene expression.

Lipomatous uterine masses: potential to mimic ovarian dermoids on endovaginal sonography.

Lipomatous uterine masses are uncommon hyperechoic pelvic neoplasms composed partly of adipose tissue. Because of the high level of echogenicity, these uterine masses can produce produce a sonographic appearance strikingly similar to the "dermoid plug" that is considered characteristic of benign cystic teratomas. The key to distinguishing the two tumors is to ascertain the parent organ: the lipomatous uterine mass should clearly originate from the myometrium. In our experience, however, the origin of some echogenic pelvic masses can be difficult to determine, particularly from the endovaginal perspective. This report describes the transabdominal and endovaginal ultrasonographic findings in three patients with lipomatous uterine masses for whom the endovaginal perspective alone, without supplemental transabdominal views, would have resulted in misdiagnosis.

Study of retinoic acid effect upon retinoic acid receptors beta (RAR-beta) in C6 cultured glioma cells.

Using monoclonal antibodies against the RAR-alpha and RAR-beta retinoic receptors, we demonstrated that these receptors were present together in C6 glioma cells as two isoforms of 50 and 55 kDa. For RAR-beta, the 50 kDa isoform predominated (60 to 80% of the total of the two isoforms). After a treatment for 48 h with retinoic acid 10 microM, the 55 kDa form was enhanced, while no effect was observed either on RAR-alpha isoforms from C6 cells and on both RAR-alpha and RAR-beta forms from neuroblastoma SKN SH SY5Y used as a control. Using purified neuronal and glial rat brain nuclei, we showed that the 55 kDa isoform from RAR-beta predominated in glial cells. These results suggest that retinoic acid treatment of C6 cells led to a partial differentiation, the enhancement of the heavy form of RAR-beta being a marker of this phenomenon.

Investigation of subjects with abnormal iron studies: role of the hepatic iron index.

AIM: Genetic haemochromatosis is a common disorder resulting in increased iron deposition in the liver and other organs but can be difficult to diagnose. The aim of this study was to assess the diagnostic value of the conventional tests for iron overload (percentage saturation of transferrin, serum ferritin and grading of iron staining on liver biopsy) and compare these with the newer quantitative biochemical measurements of liver iron. METHOD: A retrospective analysis was made of 108 consecutive patients referred for quantitative liver iron measurements. Iron studies were obtained in 66 of the 108 subjects of whom 60 had abnormal screening tests defined as percent saturation of transferrin (> 60%) and/or ferritin > 350 micrograms/L for females and > 450 micrograms/L for males. Based on clinical features, biochemical data and treatment outcome these 60 subjects were classified as either genetic haemochromatosis, nongenetic haemochromatosis or indeterminate. One patient with treated genetic haemochromatosis was excluded from subsequent analysis. RESULTS: Although the serum ferritin (p < 0.002), percentage saturation of transferrin (p < 0.001), histological iron grade (p < 0.0001) were significantly higher in the genetic haemochromatosis than nongenetic haemochromatosis group there was considerable overlap. Similarly for the hepatic iron concentration (HIC) (p < 0.0001) overlap occurred. The hepatic iron index (HIC/age) gave the best separation with only three cases being misclassified. A correlation between the HII and histological iron index (visualised iron score corrected for age) in 15 subjects gave an r value of 0.72. CONCLUSION: Based on this study we feel that in addition to visual grading of iron in liver biopsies, the hepatic iron index is helpful in establishing a diagnosis of genetic haemochromatosis.

A multicentre comparative study of serum lipids and apolipoproteins in long-term users of DMPA and a control group of IUD users. World Health Organization. Task Force on Long-Acting Systemic Agents for Fertility Regulation Special Programme of Research, Development and Research Training in Human Reproduction.

A clinical trial was conducted in three centres to assess the effects of long-term use of the injectable contraceptive depot-medroxyprogesterone acetate (DMPA) on lipid metabolism. Fifty women who had used DMPA at a dose of 150 mg every three months for 3 to 9 years were recruited in Bangkok, Christchurch and Mexico City. They were compared to a control group of 120 IUD users. Total cholesterol, LDL-cholesterol, HDL-cholesterol, total triglycerides, apolipoproteins AI, AII and B were measured throughout one injection interval. Significant findings differed between centres. Compared to their own centre controls, DMPA users in Bangkok had higher LDL-cholesterol levels; those in Christchurch had lower HDL-cholesterol, apolipoprotein (apo) AI and apo AI/B ratio and higher apo B levels; those in Mexico City had a lower apo AI/B ratio. Further changes were observed during the injection interval, some of which were correlated to changes in serum MPA levels. It is concluded that long-term use of DMPA induces moderate changes in lipid metabolism which are unfavourable in terms of risk for atherosclerosis. This should be borne in mind when weighing the overall risks and benefits of this contraceptive method for a potential user.
