[Dissolution kinetics of synthetic hydroxyapatite and human enamel crystals].

A comparative study of the dissolution of synthetic hydroxyapatite and human enamel crystals has been realized in a buffered (sodium citrate - citric acid) solution in presence of variable quantities of sodium fluoride. The pH of the solutions used was 3.7 and 5.6. The dissolution curves of the synthetic hydroxyapatite and the human enamel crystals were similar. In all cases, the dissolution mechanism was characterized by a first step with a rapid kinetic corresponding to the formation of a central cavity extending parallel to the c axis of the apatite crystal and observed with the electron microscope. In a second stage, the dissolution kinetic was much slower corresponding to a saturation of the buffered solution and an extension of the crystal destruction towards its lateral external surfaces. The dissolution kinetic was faster in a more acidic solution and the total dissolution at a given moment varied inversely to the fluoride amount.

[Prognostic value of the Lauren and Ming classifications in gastric adenocarcinoma. Multidimensional analysis].

Of the various pathology classifications for gastric carcinoma, those of Lauren and of Ming have been claimed to be of prognostic value. We therefore reviewed the charts of 101 patients (mean age 64 +/- 11 years) treated for gastric carcinoma between 1960 and 1983 at the Institut Curie. We excluded the following patients: 1) those with distant metastases; 2) those who were not operated on; 3) those who underwent laparotomy only, and 4) those who had a palliative procedure. The type of surgery performed was partial gastrectomy in 73 cases and total gastrectomy in 28. Pathology was reviewed according to the WHO, Lauren, and Ming classifications. Were taken into consideration: 1) whether the surgeon believed that removal of tumor was grossly complete or not, 2) the degree of parietal involvement according the pTNM classification system, and 3) the number of positive lymph nodes. Survival was analyzed by the Kaplan Meier method after exclusion of all postoperative deaths. Mean overall survival was 30 months, while that for patients undergoing grossly complete removal of tumor was 34 months. In univariate analysis, no correlation was found between survival and pathology according to Lauren's or Ming's classifications, sex, parietal involvement, or age. Survival was statistically correlated with the size of the tumor (p = 0.015), the gross completeness of surgery (p = 0.008), the type of surgery performed (prognosis was better after partial gastrectomy than after total gastrectomy) and the number of positive lymph nodes (p = 0.0001).(ABSTRACT TRUNCATED AT 250 WORDS)

[Responses of somatotropin to stimuli after brief administration of estradiol in short children].

Ethinyl estradiol has been administered orally, 100 micrograms per day during three days, to enhance the growth hormone (GH) response to usual pharmacological stimuli. 102 prepubertal short patients aged 2 to 17 years with height between 2 to 6 SD below the mean, were studied. Human growth hormone (hGH) treatment was given only to those patients whose GH response was still below 10 ng/ml after estradiol. Under hGH treatment, their growth rate increased twofold, as much in patients with partial GH deficiency as in those with complete GH deficiency. It is concluded that the lack of GH response after estradiol priming contributes to the assessment of the indication for treatment with hGH. However, since it has not been possible to give this treatment to very short children whose GH response became normal after priming, this study does not allow to preclude the effect of hGH in such conditions. Thus estradiol priming must not be included among the practical criteria leading to therapeutic decision in doubtful cases.

[Association of hyperlipidemia, acute pancreatitis and diabetic keto-acidosis. A case report (author's transl)].

A 32-year-old man was admitted semi-comatose with digestive troubles to the hospital. A diabetic keto-acidosis was discovered. The diabetes was known by the patient for one year and a half, but it was not treated. Furthermore the laboratory studies showed a hyperlipidemia (type V) and an increase in pancreatic enzymes. The hyperlipidemia disappeared with insulin therapy. The chronology of the facts was probably: a hyperlipidemia associated with the diabetes, responsible of an acute pancreatitis, which induced a keto-acidosis.

[Sneddon syndrome: 9 cases].

We observed a series of 9 patients (1 male, 8 females, mean age 49 years) who had experienced cerebral vascular events with livedo racemosa (Sneddon's syndrome). Vascular dementia occurred in 3 patients and in the 6 others there was a single or several acute cerebral ischaemic events. Angiography of the brain revealed multiple distal arterial occlusions in 5 cases and a moya-moya type collateral network in 2. Positivity for anticardiolipid antibodies fluctuated in 4 cases and there was a lupic syndrome in 2. Systemic lupus erythemosus was diagnosed in the last patient. Mitral valve defects were seen in 5 patients, including 3 due to post-rhematitis sequelae which became symptomatic before the appearance of signs of neurocutaneous involvement. Among these three patients, laboratory tests revealed a lupus band in one, anticardilipid antibodies in another and obliterating fibrous endartiritis of the renal arteries in the third. Sneddon's syndrome presents with heterogeneous signs related to its complex pathophysiology.
