Diagnostic value for rheumatoid arthritis of antiperinuclear factor at the 1:100 threshold. Study of 600 patients and meta-analysis of the literature.

Six hundred sera from patients with chronic rheumatic diseases including 429 with rheumatoid arthritis were tested in a blind fashion for IgG antiperinuclear factor using an indirect fluorescent antibody assay on buccal cells. Using the dilution of 1:100 found to be optimal in an earlier study, 283 of the 429 (66%) rheumatoid arthritis sera and 22 of the 171 (13%) control sera were positive for antiperinuclear factor. Titers were higher in the rheumatoid arthritis group than in the control group. A meta-analysis of studies that used sera diluted 1:80 or 1:100 (2853 sera including 939 from patients with rheumatoid arthritis, 1539 from patients with other rheumatic diseases and 375 from healthy controls or patients with unclassified rheumatic diseases) yielded a sensitivity of 0.75, a specificity of 0.93, and a positive predictive value of 0.87. These data demonstrate that antiperinuclear factor testing contributes usefully to the diagnosis of rheumatoid arthritis.

Complete amino-acid sequence of the beta-subunit of VTX from venom of the stonefish (Synanceia verrucosa) as identified from cDNA cloning experiments.

A cDNA encoding a subunit of the verrucotoxin (VTX) has been identified from a cDNA library derived from stonefish venom glands. It encodes a polypeptide of 708 amino-acid residues, followed by a 3'-untranslated region of 895 bp long. The ORF contains the complete mature sequence of the beta-subunit of the VTX, as inferred from both the presence of an identical N-terminus sequence and 96% homology among the 506 amino terminus residues found in the partial sequence of the beta-subunit of the stonustoxin from Synanceia horrida (Ghadessy, F.J., Jeyaseelan, K., Chung, M.C.M., Khoo, H.E., and Yuen, R. (1994) Toxicon 32, 1684-1688). Upstream the mature sequence, we noticed the presence of an incomplete peptide of a 13 amino acids, whose unusual primary structure supports the idea of the existence of a propeptide and/or of a new secretion signal.

Cardiotoxicity of verrucotoxin, a protein isolated from the venom of Synanceia verrucosa.

Freshly purified but unstable verrucotoxin (VTX) and a more stable proteic complex of the toxin (p-VTX) were isolated from the venom of the stonefish Synanceia verrucosa and applied to frog atrial fibres. VTX and p-VTX decreased the amplitude and the duration of the stimulated peak tension and accelerated the relaxation phase of the contraction. The negative inotropic effect of p-VTX decreased with increasing the external Ca concentration ([Ca]o) in the Ringer solution. The negative chronotropic effect induced by p-VTX was insensitive to change in [Ca]o. It is reversed by glibenclamide. p-VTX shortened the duration of the plateau and the repolarizing phase of the action potential. Glibenclamide but not tetraethylammonium reversed the p-VTX-induced shortening of the AP repolarizing phase. The data suggest that the toxin isolated from the venom of S. verrucosa inhibits Ca channels and might activate ATP-sensitive potassium channels in frog atrial heart muscle.

[A baobab solution for the prevention and treatment of acute dehydration in infantile diarrhea].

OBJECTIVE: The purpose of this study was to compare the clinical efficacy of a local solution based on "pain de singe", fruit of baobab (Adansonia digitata), and the WHO standard solution in the treatment of children with acute diarrhea and resulting mild to moderate dehydration. METHODOLOGY: A prospective clinical trial comparing the local solution to the standard WHO solution was conducted; in children of 6 months or older (mean age = 16.6 +/- 8.8 months), 79 received the WHO solution and 82 the local solution, and were followed for a period ranging from 4 to 48 hours. Evolution of diarrhea and weight gain were evaluated as variables of interest. RESULTS: WHO solution was found to be superior, but not statistically significant in term of duration of diarrhea and weight gain. DISCUSSION: The pain de singe based solution presents additional advantages: nutritional, economic and cultural; we recommend it to promote use of TRO at home.

[Technical quality of prenatal visits in Senegal].

The objective of this study is to estimate the quality of antenatal care in Senegal. A survey was conducted in 49 health centers near 70 health workers practising antenatal cares for pregnant women. The quality of cares was assessed on 13 essential actions, which were classified in 4 components. The global score of quality and the specific score of each component were calculated and analysed according to the qualification and the duration of time in the work of the health workers, the location and the equipment of the centers. Global quality of prenatal cares was linked with the qualification of agents. The component "screening for risk factors" had low and worse score and "reception of pregnant women" also. These 2 components were not linked with the qualification of agents, contrary to the 2 others, pregnancy and mother health surveillance. The duration of time in service and the equipment had no influence in global and specific scores. The health workers qualification was linked with many components of quality, but a good basic training is not sufficient to provide prenatal care of high quality. It must be define a framework with screening for unfavorable ends of pregnancy.

Presence of norepinephrine and other biogenic amines in stonefish venom.

Although fish venoms exert a cardiovascular effect, the presence of adrenergic substances was not previously demonstrated. Chromatographic analysis with electrochemical detection showed the presence of substances co-migrating with norepinephrine, dopamine and tryptophan. Serotonin, which was thought to be implicated in the intense pain following fish envenomation, was not detected. Norepinephrine was identified as a component of the stonefish Synanceia verrucosa venom by gas chromatography-mass spectrometry.

International trade in services: a growing trend among highly skilled migrants with special reference to Asia.

"An essential component of trade in services is cross-border movements of service providers.... On the one hand, there are international movements of skilled transients who emigrate for individual reasons in a long term perspective. On the other hand, there are skilled temporary migrants who perform services abroad without the intention or right to settle or seek employment in the host country. This study aims to clarify these differences using the example provided by the Asian Pacific region. Moreover, this study shows that the dynamics of skilled international migration is largely determined by the circulatory movement of skills of international service providers and has emerged as an essential component of economic development strategy of the countries in the region."

Exhaled nitric oxide during acute changes of airways calibre in asthma.

It has been shown that endogenous nitric oxide (NO), measured in exhaled air, is increased in asthmatic subjects and after allergen challenge in sensitized animals. NO is also a paracrine molecule with some, though weak, bronchodilator effects. However, whether the amount of endogenous NO that originates in the lungs can reflect the degree of bronchial tone and airways calibre in asthmatic subjects has not yet been investigated. The aim of this study was, therefore, to determine whether NO production could be modified by acute changes of airways calibre in mild, nonatopic, asthmatic subjects. NO output was measured in the exhaled air of 14 steroid-free asthmatics, 8 steroid-treated asthmatics and 21 control subjects. In seven steroid-free asthmatics, exhaled NO was measured after methacholine challenge, and then after salbutamol-induced bronchial dilatation. Exhaled tidal breathing was collected for 30 s and NO in the exhaled air was measured with a chemiluminescence analyser. Both NO concentration and its output were significantly higher in the steroid-free asthmatic patients (15.6 +/- 1.5 parts per billion (ppb) and 6.3 +/- 0.7 nmol.min-1, respectively) as compared with the control subjects (8.9 +/- 1.0 ppb and 3.5 +/- 0.3 nmol.min-1, respectively; p < 0.001 for both) and with the steroid-treated asthmatic patients (11.3 +/- 3.3 ppb and 3.7 +/- 0.9 nmol.min-1, respectively; p < 0.05 for both). Neither methacholine-induced bronchial obstruction nor salbutamol-induced bronchial dilatation caused a significant change in exhaled NO. We conclude that NO production is higher in steroid-free than in steroid-treated asthmatics and in control subjects. Furthermore, NO production is not affected by acute pharmacologically-induced changes of airways calibre in asthmatic subjects. Our results suggest that NO production is a marker of airways inflammation rather than an endogenous modulator of bronchial tone in asthma.

Enzymatic properties of the stonefish (Synanceia verrucosa Bloch and Schneider, 1801) venom and purification of a lethal, hypotensive and cytolytic factor.

The crude venom of the stonefish (Synanceia verrucosa) possesses numerous enzymatic properties (hyaluronidase, eight esterases and ten aminopeptidases). Verrucotoxin was isolated by DEAE and hydroxyapatite chromatography, followed by FPLC gel filtration on Superdex 200 HR 10/30. It was found to be a glycoprotein with a mol. wt of 322,000 +/- 2000, comprising four subunits, 2 alpha (83,000) and 2 beta (78,000). Verrucotoxin, as the crude venom, is lethal for mice, haemolyses washed rabbit erythrocytes and induces a fall in arterial pressure in anaesthetized rats.

Does crude venom of the stonefish (Synanceia verrucosa) activate beta-adrenoceptors in the frog heart muscle?

Venom isolated from the stonefish Synanceia verrucosa was assayed in concentrations of 0.07 and 5.7 micrograms/ml on frog atrial fibres and myocytes. Venom, less than 2.9 micrograms/ml, dose-dependently increased the amplitude and the duration of the stimulated peak tension, lengthened the time constant of the relaxation phase and shortened the duration of the action potential (AP). The concentration of venom 5.7 micrograms/ml decreased the amplitude of the peak tension, induced a contracture, reduced the amplitude of the plateau and shortened its duration as well as the repolarizing phase of the AP. The positive inotropic effect induced by the venom (2.9 micrograms/ml) on the contraction was inhibited dose-dependently by propranolol but was unchanged by the alpha-adrenergic antagonists urapidil and yohimbine, the adenyl cyclase activity remaining sensitive to forskolin. Venom, adrenalin and propranolol competed for a common site. Venom (2.9 micrograms/ml) increased both the Ca and the delayed outward K currents of enzymatically isolated atrial myocytes. The data suggest that the venom activates adrenoceptors, essentially beta-adrenoceptors.

Endogenous pulmonary nitric oxide production measured from exhaled air is increased in patients with severe cirrhosis.

BACKGROUND/AIMS: Endogenous pulmonary nitric oxide production may be increased in severe cirrhosis and contribute to pulmonary vasodilation. This study assessed pulmonary nitric oxide production by measuring nitric oxide in the exhaled air in patients with severe cirrhosis and examined the relationship between exhaled nitric oxide and pulmonary hemodynamics in these patients. METHODS: Nitric oxide concentrations and production were measured in the exhaled air in six Child-Pugh class C patients with cirrhosis and 21 non-smoking healthy controls. Systemic and pulmonary hemodynamics were measured in patients only. RESULTS: Nitric oxide concentration (32.0 +/- 1.7 (mean +/- SEM) vs. 8.9 +/- 1.0 ppb) and production (9.2 +/- 1.3 vs. 3.1 +/- 0.3 nmol/min) in exhaled air were significantly higher in patients than in controls. Patients had high cardiac output (8.5 +/- 0.9 l/min), low pulmonary and systemic vascular resistance (57 +/- 10 and 767 +/- 93 dyn.s.cm-5, respectively) under baseline conditions. A significant negative correlation was found between pulmonary vascular resistance and exhaled nitric oxide production (r=0.943, p=0.05) but not between cardiac output or systemic vascular resistance and nitric oxide measured in exhaled air. CONCLUSIONS: Endogenous pulmonary nitric oxide production measured from exhaled air is increased in patients with cirrhosis and liver failure. Increased in patients with cirrhosis and liver failure. Increased nitric oxide production may also contribute to cirrhosis-induced pulmonary vasodilatation.

Sterilization reversal: fertility results.

Fertility outcome following sterilization reversal by laparotomy was evaluated. We studied all sterilization reversals performed between January 1978 and December 1991; a total of 226 women were treated. Tubal anastomosis was performed according to the rules for microsurgery. The microsurgical technique comprised two main phases: preparation of the healthy tube segments and the anastomosis carried out on two layers. It was possible to study the subsequent fertility of 206 patients in all, as 20 patients were lost to follow-up (8.8%). Cumulative pregnancy rates were evaluated by life-table analysis. The overall intrauterine pregnancy rate, including births and miscarriages, was 69.9% (144 patients) after 2 years. The cumulative intrauterine pregnancy rate was 62% at 18 months. Analysis of the fertility results demonstrated that age was the most significant predictive factor. The cumulative intrauterine pregnancy rate at 2 years was 83.5% (61 cases) for patients < or = 33 years, 70% (35 cases) for patients aged 34-36 years, 62.5% (30 cases) for patients aged 37-39 years, and 51.4% (18 cases) for patients aged > or = 40 years. Even for patients aged approximately 40 years, microsurgical repermeabilization can be retained as the first choice rather than in-vitro fertilization.

[Myxoma of the left atrium: a cause of retinal artery embolism not to be ignored. Apropos of a case].

We report a case of retinal artery obstruction in a 56-year-old woman which proved to be a first symptom of a left atrial myxoma. The patient also had an inflammatory syndrome and a transient embolism of the cerebellum and the left leg. Two dimensional echocardiography showed an atrial mass compatible with myxoma. This diagnosis was confirmed by histological analysis of the surgically-removed tumour.
