Is a long delay necessary before appendectomy after appendiceal mass formation? A preliminary report.

The standard treatment for an appendiceal mass is conservative therapy followed by appendectomy after 6 to 10 weeks. With the advent of antibiotics designed to prevent the growth of anaerobes, early appendectomy can now be carried out without complication. The authors studied 56 patients with appendiceal mass formation, 26 (group A) treated conventionally and 30 (group B) treated by early appendectomy. In group B, the infection rate was 17%, the mean operating time was 38.7 minutes, the mean hospital stay was 15 days and there was an early return to work. The corresponding parameters for group A were an infection rate of 8%, a mean operating time of 35.2 minutes, a hospital stay of 19.1 days and a late return to work. Furthermore, 15% of the patients in group A had a recurrent acute episode during the waiting period. Overall, early appendectomy appears to be a safe and cost-effective treatment for appendiceal mass formation.

Pancreatic abscess due to Salmonella typhi.

Isolated involvement of the pancreas in Salmonella typhi bacteraemia is rare. A case of pancreatic abscess due to S. typhi is reported which was managed conservatively.

Prediction of postoperative retention in elderly patients undergoing nonprostatic surgery by preoperative uroflowmetry.

500 patients above 50 years of age, undergoing lower abdominal/perineal surgery and having no urinary complaints were assessed by measuring the preoperative maximum flow rate (Qmax). They were subsequently observed for urinary retention in the postoperative period. The analysis of the results has shown a definite relation between the preoperative Qmax and the probability of urinary retention in the postoperative period and uroflowmetry has been found to be a good screening procedure for the detection of cases which could possibly have urinary trouble requiring significant intervention in the postoperative phase, thereby changing the priority of surgical attention.

Angiomyolipoma of kidney.

Angiomyolipoma is an uncommon benign tumour accounting for less than 1% of all surgically excised tumours of the kidney. It may present as an isolated lesion or in association with tuberous sclerosis. An impalpable tumour is often detected incidentally during investigation of a case for some urinary tract pathology. CT scan has proved to be a useful investigation for pre-operative diagnosis; it may however, be of no value if the fat content of the tumour is low. Here a case of angiomyolipoma of left kidney is reported where pre-operative diagnosis was suggested by CT scan and histopathology confirmed it.

Maximum flow rate-guided dilatation in patients of stricture urethra: a controlled study.

Maximum flow rate (MFR) is an objective uroflowmetric parameter for assessment of urethral stricture. Its use as a guide to assess the frequency of dilatation of the urethra on an outdoor basis could make the approach very scientific and objective. 70 cases were evaluated in two groups, study and control, each of 35 patients. Dilatation in the study group was guided by serial measurement of MFR, while in the control group, stricture was dilated on a traditional scale. A comparison between the two groups showed a statistically significant decline in the frequency of dilatation in the study group. A routine practice of MFR-guided dilatation in stricture clinics is recommended by us.

Tumorigenicity of di-tert-butyl-substituted hydroquinone and hydroxyanisoles in the forestomach of Syrian golden hamsters.

The di-tert-butyl-substituted hydroxyanisoles 2,5-di-tert-butyl-4-hydroxyanisole (2,5-DTBHA) and 3,5-di-tert-butyl-4-hydroxyanisole (3,5-DTBHA) are contaminants of the commercial antioxidant butylated hydroxyanisole. The tumorigenicity of 2,5-DTBHA, 3,5-DTBHA, 2,5-di-tert-butyl hydroquinone (2,5-DTBHQ), a demethylated analog of 2,5-DTBHA, and 3-tert-butyl-4-hydroxyanisole (3-BHA) were investigated. Dietary 3-BHA, which has been shown previously to induce neoplastic lesions in the hamster forestomach, was found to cause the development of forestomach papilloma in 75% of the hamsters in 24 weeks at 1% addition to the diet. General thickening of the forestomach lining with moderate to severe hyperkeratosis and hyperplasia were observed in 14 (88%) of the 16 hamsters in this group. Inflammation was generally found in the forestomach of these animals. Similar observations were found in 14 (88%) of the 16 hamsters in the group that was fed 1% DTBHQ. Severe hyperplasia and papilloma found in the forestomach of these hamsters were similar in severity to those in the 3-BHA group. Unlike the 3-BHA and 2,5-DTBHQ groups, the forestomachs of 1% 2,5-DTBHA and 3,5-DTBHA treated animals were not different from the hamsters in the control group. Only mild hyperplasia at or near the limiting ridge was found in the forestomach of some of these animals. Hyperkeratosis or inflammation were not observed. Steric factors may play a role in the lack of tumorigenicity of 2,5-DTBHA and 3,5-DTBHA, while the formation of a stable free radical from 2,5-DTBHQ may be the activated species responsible for its tumorigenicity.

Intestinal malrotation in adults.

Intestinal malrotation presenting with abdominal problems are not common beyond the neonatal period. An adolescent or adult harboring this anomaly may suffer from delay in diagnosis and inappropriate treatment including surgery. We report three patients, aged 17, 31 and 50 years, with intestinal malrotation presenting as an appendicular problem (2) and chronic abdominal pain (1). The former mode of presentation has not been reported so far.

Affinity of 2-(tetrahydroisoquinolin-2-ylmethyl)- and 2-(isoindolin-2-ylmethyl)imidazolines for alpha-adrenoceptors. Differential affinity of imidazolines for the [3H]idazoxan-labeled alpha 2-adrenoceptor vs the [3H]yohimbine-labeled site.

A series of 2-(tetrahydroisoquinolin-2-ylmethyl)- and 2-(isoindolin-2-ylmethyl)imidazolines were prepared and tested for alpha 1- and alpha 2-adrenoceptor affinity with radioligand binding. Several compounds, 5-fluoro-(5h), 5-chloro-(5j), 5,8-dimethoxy- (5r), and 5,8-dimethoxy- (5r),1-methyl- (5s) 2-(tetrahydroisoquinolin-2- ylmethyl)imidazoline, were found to be selective alpha 2-adrenoceptor ligands on the basis of displacement of [3H]yohimbine from rat cerebral cortical membranes. One compound, 2-[(8-chloro tetrahydroisoquinolin-2-yl)methyl]imidazoline (5m), showed a 36-fold difference in affinity for the [3H]idazoxan-labeled alpha 2-adrenoceptor relative to the [3H]yohimbine-labeled site, which may be evidence for alpha 2-adrenoceptor subtypes.

Toxic psychosis due to chloroquine--not uncommon in children.

Toxic psychosis due to Chloroquine is a relatively uncommon occurrence in childhood. This entity must be kept in differential diagnosis in a case of unexplained psychosis, before resorting to a sophisticated array of detailed investigations. The purpose of this is to familiarize pediatricians with this relatively uncommon entity. The authors encountered four cases of psychosis with a wide variety of symptomatology over a period of the past 18 months.
