Combination of external and interstitial irradiation in the primary management of breast carcinoma.

The authors treated 83 patients with carcinoma of the breast using a combination of external and interstitial irradiation at Los Angeles County--University of Southern California Medical Center and Southern California Cancer Center between February 1, 1974, and May 31, 1977. All 30 patients who had T1 and T2 lesions, and who were followed for a minimum period of 24 months (24-50 months) are clinically still free of disease. Thirty-nine of 53 patients with T3 and T4 lesions, who were followed for a minimum of 30 months (30-56 months) are still alive with local control. Details of treatment techniques, dosimetry, results and complications are presented.

Afterloading interstitial implant in head and neck cancer.

Seventy-nine patients with head and neck malignancies were treated using afterloading interstitial iridium 192 techniques at Los Angeles County-University of Southern California Medical Center and Southern California Cancer Center from Feb 1, 1974 to Nov 30, 1975. Twelve of the 13 T1 and T2 lesions and 17 of 25 T3 and T4 lesions remain locally controlled to a minimum follow-up period of 24 months. Surgical salvage in case of treatment failure is possible with low morbidity because of relatively low external irradiation to the total volume. Twenty of 41 patients who were reirradiated for persistent or recurrent cancers still have local control to a minimum follow-up period of 24 months. We discuss sites of involvement, implantation techniques, reactions, and complications.

Evaluation of heroin maintenance in controlled trial.

Ninety-six confirmed heroin addicts requesting a heroin maintenance prescription were randomly allocated to treatment with injectable heroin or oral methadone. Progress was monitored throughout the next 12 months by research workers operating independently of the clinic. Heroin can be seen as maintaining the status quo, with the majority continuing to inject heroin regularly and to supplement their maintenance prescription from other sources; it was associated with a continuing intermediate level of involvement with the drug subculture and criminal activity. Refusal to prescribe heroin while offering oral methadone constituted a more confrontational response and resulted in a higher abstinence rate, but also a greater dependence on illegal sources of drugs for these who continued to inject. Those offered oral methadone tended to polarize toward high or low categories of illegal drug use and involvement with the drug subculture, and were more likely to be arrested during the 12-month follow-up. There was no difference between the two groups in terms of employment, health, or consumption of nonopiate drugs. Refusal to prescribe heroin resulted in a significantly greater drop out from regular treatment.

Aging, memory, and the cholinergic system: a study of dichotic listening.

Impairment of cholinergic neural function has been proposed as a cause of memory and cognitive (M/C) disorders of the aged since with cholinergic blockade young subjects show the M/C pattern of the elderly. To validate this hypothesis, we compared the performance of young normal subjects, young scopolamine-treated subjects, and aged subjects, using a different type of behavioral task, a dichotic listening test, which primarily measures neural channel capacity. With simultaneous dichotic presentation, undrugged young subjects reported both messages correctly in 35% of trials. Aged subjects scored double-correct responses in only 19% of trials, a highly significant difference (p less than 0.001). Following treatment with 1.0 mg scopolamine, young subjects also scored double correct responses in only 19% of trials, a highly significant reduction from their undrugged performance (p less than 0.01), but virtually the same as that of normal aged subjects. Similar findings were obtained with a staggered dichotic presentation. These results extend the observation that cholinergic blockade in young subjects reproduces the pattern of M/C decline of the aged; thus providing further for the hypothesis that this decline may result from impaired function in cholinergic neurons. The neurobiologic implications of cholinergic impairment are discussed.

Description of an afterloading 192Ir interstitial-intracavitary technique in the treatment of carcinoma of the vagina.

This is a preliminary report of an afterloading 192Ir interstitial-intracavitary technique used in the treatment of carcinoma of the vagina. Thirteen cases of carcinoma of the vagina, stages I through III, were treated at the Southern California Cancer Center and University of Southern California Radiation Medicine Unit between November 1976 and February 1978. The Syed-Neblett perineal template is described in detail, as are the precise clinical application techniques. The hallmarks of the method are: 1) ease of use by the physician, 2) homogeneous radiation dose distribution, and 3) differential radiation dose delivery capabilities. The technique is believed to be an improvement over previous modalities of therapy reported in the literature.

Age-dependent and strain-related differences of virulence of Semliki Forest virus in mice.

Factors that influence the virulence of Semliki Forest virus for mice have been studied. In vivo experiments showed that maximum brain infectivities following i.p. inoculation of adult mice with a virus strain of low virulence were less than those found in mice showing clinical signs in virulent infections. The virulent strains generally caused death before antibody could be detected in brain tissue; the less virulent strain caused an infection that was neuroinvasive, but infectivity increased less rapidly in the brain and allowed antibody to intervene before clinical signs were apparent. IgG3 antibody was first detected in brain tissue coincident with the beginning of the decline of brain infectivities in avirulent infections; other sub-classes of IgG antibody were not detected until later. In vitro experiments showed differences in abilities of SFV strains to replicate in tissue culture and adult mouse brain tissue, and differences in susceptibility to infection of brain tissue from mice of various ages. These in vitro results could provide a basis for differences of virulence.
