Comparison of duplex ultrasound with angiography in assessment of carotid bifurcation disease.

This is a study comparing duplex ultrasound against the "gold standard" of angiography in assessing atherosclerotic disease of the carotid bifurcation, prior to prospective endarterectomy surgery. Thirty-nine patients were studied with both sonography and angiography studies being performed within one month of each other. Plaques were described by sonography as being "smooth" or "irregular" in surface and "homogeneous" or "heterogeneous" in composition. Ultrasound showed an overall 92% sensitivity, against the standard of angiography, in its ability to assess the degree of internal carotid stenosis. There was only a 63% sensitivity with the common carotid arteries and only a 65% sensitivity with the external carotid arteries. Ultrasound did not show a high accuracy in detecting plaque ulceration when compared against angio-graphy.

[Epidemiology of suicide in Austria 1980 to 1990].

This article investigates the suicide rates in Austria from 1980 to 1990 according to age groups and the methods used. In the years investigated the overall suicide rate increased until 1986, followed by a decline until 1990. The suicide rate in women shows a gradual linear increase with age, whilst in men there is a biphasic curve with a steep increase in teenagers and young men in their early twenties, followed by a plateau and a second steep increase as from 65 years of age, whereby the increase from "60-64 years" to the age group "over 85 years" is 134%. Regarding the age groups the decline over the past years in women is biggest in the groups "35-39 years" and "65-69 years", with men a decrease is present in the age groups from 20 to 50 years. Concerning the methods used, hanging is the most common mode of suicide in both women (36.4%) and men (48.8%). Suicide in the elderly--particularly in old men--which has increased internationally over the past few decades is targeted as the major area for vigorous preventive measures in Austria.

[Coincidence of lung diseases caused by Mycobacterium fortuitum and esophageal achalasia--case report and review of the literature].

We report on a case of lung infection due to mycobacterium fortuitum in conjunction with esophageal achalasia. A review of literature and a discussion on pathophysiological, clinical and therapeutical aspects are included. We conclude that bacterial poly-resistance in case of infections resembling tuberculosis should prompt a search for atypical mycobacteria. Patients suffering from nontuberculous mycobacteriosis of the lung should be subject to oesophageal examination.

Identification of a cDNA clone specific for the oligodendrocyte-derived repulsive extracellular matrix molecule J1-160/180.

A cDNA clone specific for the oligodendrocyte-derived extracellular matrix glycoproteins J1-160/180 was obtained from a lambda ZAPII expression library using polyclonal antibodies generated against mouse J1-160. The library was constructed from poly(A)(+)-RNA isolated from O1 antigen-positive rat oligodendrocytes. The cDNA clone expressed a fusion protein that was recognized by the J1-160/180-specific monoclonal antibodies 596, 619, and 620, and, weakly, 597. The fusion protein was not recognized by polyclonal antibodies to mouse J1/tenascin. The cDNA clone with an insert of approximately 5.6 kb in size contained the nucleotide sequence coding for the amino acid sequence of the N-terminus of a tryptic peptide derived from mouse J1-160. The developmental and tissue distribution of the mRNA recognized by the cDNA clone is in agreement with the described expression of the J1-160/180 proteins.

Clinical and epidemiological aspects of dementia in the elderly.

Dementia of the Alzheimer's type (DAT) is the most significant disease of the aging brain. Descriptive epidemiology of DAT found a constant doubling of prevalence rates every 5 years. Analytic epidemiology so far failed to reliably detect risk factors for DAT other than age. This might depend on the difficulties encountered in the clinical diagnosis and differential diagnosis of dementia in the elderly, which are discussed with special reference to 1) the definition of dementia, to 2) the grading of severity of dementia, to 3) the differentiation between dementia and depression, and to 4) the differentiation between multi-infarct dementia and DAT.

Acid phosphatase-1(1), a tightly linked molecular marker for root-knot nematode resistance in tomato: from protein to gene, using PCR and degenerate primers containing deoxyinosine.

With a view to cloning the root-knot nematode resistance gene Mi in tomato by chromosome walking, we have developed a molecular probe for the tightly linked acid phosphatase-1 (Aps-1) locus. The acid phosphatase-1 allozyme (APS-1(1], encoded by the Aps-1(1) allele originating from Lycopersicon peruvianum, was purified to apparent homogeneity from tomato roots and suspension cells. Microsequencing of CNBr and tryptic peptides generated from APS-1(1) provided a partial amino acid sequence, which accounted for approximately 23% of the protein and revealed two stretches of homology with soybean proteins KSH3 and VSP27, comprising 22 matches within 26 amino acid residues. The partial amino acid sequence information enabled us to isolate a 2.4 kb genomic Aps-1(1) sequence by means of the polymerase chain reaction (PCR), primed by degenerate pools of oligodeoxyribonucleotides, synthesized on the basis of the amino acid sequences. Synthesis of the 2.4 kb PCR product was specific for genomic templates carrying the L. peruvianum Aps-1(1) allele. Crucial to the priming specificity and the synthesis of the 2.4 kb genomic sequence was the use of degenerate primer pools in which the number of different primer species was limited by incorporating deoxyinosine phosphate residues at three and four base ambiguities. In using cDNA as a template, a 490 bp sequence was obtained, indicating a high proportion of intron sequences in the 2.4 kb genomic Aps-1(1) sequence. The Aps-1(1) origin of the PCR product was confirmed by RFLP (restriction fragment length polymorphism) analysis, using both a chromosome 6 substitution line and a pair of nearly isogenic lines, differing for a small chromosomal region around the Aps-1/Mi loci.

Characterisation of a Xp21 microdeletion syndrome in a 2-year-old boy with muscular dystrophy, glycerol kinase deficiency and adrenal hypoplasia congenita.

We report a 2-year-old boy with Duchenne muscular dystrophy (DMD), glycerol kinase deficiency (GK) and adrenal hypoplasia congenita (AHC). At three weeks of age, the patient was hospitalized for the first time with symptoms of hypotone dehydration because of AHC. At present, he shows severe muscular hypotonia and developmental delay. The patient and his family were referred to us for prenatal diagnosis and carrier testing in the mother of the patient and the mother's sister, respectively. The patient's DNA was examined by Southern blot and polymerase chain reaction analyses, using cDNA and genomic probes within and around the dystrophin (DYS) locus. A deletion was revealed, spanning DXS28, the whole dystrophin locus, DXS84 and DXS148, whereas DXS67, DXS68 (pter) and OTC (cen) were found to be retained. The cytogenetically visible microdeletion was also seen in the patient's mother, but not in the mother's sister or the patient's maternal grandmother. Our findings support the locus order pter-DXS67-DXS68-DXS28-AHC-GK-DMD-cen.

Differential diagnosis of histogenetically distinct human epithelial renal tumours with a monoclonal antibody against gamma-glutamyltransferase.

The localization of membrane-bound gamma-glutamyltransferase with monoclonal antibody (mAb) 138H11 proved to be of value for differential diagnosis of renal cancer, since it correlated with the histogenetic profile of human epithelial renal tumors. Immunoreactive gamma-glutamyltransferase was located in the proximal tubule in all normal human kidneys (15/15) examined thus far by both ultrastructural and immunohistochemical techniques. From 68 epithelial renal cancers tested 31/31 clear-cell carcinomas and 15/16 chromophilic carcinomas expressed the target epitope of mAb 138H11. In contrast, 0/11 oncytomas, 0/9 chromophobic carcinomas, and 0/1 Duct-Bellini carcinoma were immunoreactive. These results support a model of histogenesis and classification of epithelial renal tumours, according to which clear-cell and chromophilic renal carcinomas originate from transformed proximal tubule cells, whereas oncocytomas, chromophilic and Duct-Bellini carcinomas originate from cells of the collecting duct.

Alzheimer lesions in the entorhinal region and isocortex in Parkinson's and Alzheimer's diseases.

Neuropathologic examination in elderly individuals and patients with Parkinson's disease with and without dementia reveals abundant isocortical amyloid deposits with no or only a few neuritic plaques, neuropil threads (NT), and neurofibrillary tangles (NFT), whereas NT and NFT may be present only in the entorhinal region of the parahippocampal cortex. In Down's syndrome, Alzheimer's disease, and Parkinson's disease, early neuronal degeneration with deposition of NT and NFT may selectively involve layer pre-alpha (II) of the entorhinal region (Brodmann 26 area) forming the origin of the glutamatergic perforant pathway. Its bilateral destruction isolates the hippocampus from isocortical influx. Comparative studies in a series of aged subjects and those with Parkinson's disease show that psychostatus correlates better with the number of NT and NFT in the entorhinal region than in hippocampal area CA-1 and isocortex. This pattern of neuronal degeneration may explain cognitive impairment in early stages of both Alzheimer's and Parkinson's diseases.

3-(2-hydroxyphenyl)catechol as substrate for proximal meta ring cleavage in dibenzofuran degradation by Brevibacterium sp. strain DPO 1361.

Brevibacterium sp. strain DPO 1361 oxygenates dibenzofuran in the unusual angular position. The 3-(2-hydroxyphenyl)catechol thus generated is subject to meta ring cleavage in the proximal position, yielding 2-hydroxy-6-(2-hydroxyphenyl)-6-oxo-2,4-hexadienoic acid, which is hydrolyzed to 2-oxo-4-pentenoate and salicylate by 2-hydroxy-6-oxo-6-phenyl-2,4-hexadienoic acid hydrolase. The proximal mode of ring cleavage is definitely established by isolation and unequivocal structural characterization of a cyclization product of 2-hydroxy-6-(2-hydroxyphenyl)-6-oxo-2,4-hexadienoic acid, i.e., 3-(chroman-4-on-2-yl)pyruvate.

Prospective neuropathological validation of Hachinski's Ischaemic Score in dementias.

The sensitivity and specificity of Hachinski's Ischaemic Score (IS) in the diagnosis of the vascular aetiology of dementia was studied in a series of 32 demented patients, dementia of the Alzheimer type (16), multi-infarct dementia (7), mixed dementia (6), Pick's disease (3), with neuropathological diagnosis as the point of reference. The IS distinguished between primary degenerative dementia and multi-infarct or mixed dementia. As single features of the IS "a positive history of stroke" and "a fluctuating course" showed differing prevalences in the latter two diagnostic categories. The IS labelled 21% of patients with primary degenerative dementia as having a vascular aetiology. The uncritical application of the IS to large samples in epidemiological studies may cause incorrect labelling of a significant proportion of patients with primary degenerative dementia as vascular dementia. These results are based on observations of long-term inpatients and depend on neuropathological criteria. While the definite diagnosis of DAT by threshold criteria concerning plaque and tangle counts is well established, neither clinical nor pathological evidence of stroke necessarily means that cerebrovascular disease has anything to do with a patient's dementia.

Novel metabolite structures from biotransformation of a sesquiterpenoid ketone by selected fungal strains.

The sesquiterpenoid ketone, 1,4,4-trimethyltricyclo[5.4.0.0(3.5)]undec-7-en-9-one (1), was subjected to microbial transformation by six fungal strains: Aspergillus niger ATCC 9142, Aspergillus ochraceus DSM 824, Beauveria bassiana ATCC 7159, Cunninghamella echinulata ATCC 9244, Rhizopus arrhizus ATCC 11.145, and Absidia blakesleeana ATCC 10.148. Four main metabolites were formed from 1: 10(R)- and 10(S)-hydroxy-1,4,4-trimethyltricyclo-[5.4.0.0(3.5)]undec-7- en- 9-one (2 and 3, respectively), besides 4(R)- and 4(S)-(hydroxymethyl)-1,4-dimethyltricyclo[5.4.0.0(3.5)]undec -7-en-9-one (4 and 5, respectively). Compounds 2-5 were isolated with varying percentages from the respective transformations, and their structures established unequivocally by a combination of spectroscopic methods. Metabolites 2 and 3 are products of hydroxylation at C-10, in either R- or S-position; in 4 and 5, one geminal CH3 group each on the cyclopropane ring has been transformed into a CH2OH function.

Hosts on which nymphal Ixodes ricinus most abundantly feed.

To identify hosts that may serve as European reservoirs for the agent of Lyme disease, Borrelia burgdorferi, we determined whether nymphal Ixodes ricinus feed mainly on particular mice (Apodemus flavicollis or A. agrarius), voles (Clethrionomys glareolus) or on sand lizards (Lacerta agilis) and whether the abundance of these hosts corresponds to the seasonal activity of the subadult stages of the vector tick. In all sites, the mice appeared most heavily infested by larvae; at least seven parasitized each mouse, about three per vole and four per lizard. Many fewer nymphal I. ricinus parasitized A. flavicollis and C. glareolus than did larvae. Although more than 30 times as many larval than nymphal ticks parasitized the two most abundant hosts (C. glareolus and A. flavicollis), about 15 times as many fed on A. agrarius and twice as many on lizards. Nymphal and larval ticks fed on rodents at about the same time. Lizards were most abundantly parasitized by nymphs somewhat earlier than by larvae. Early in the season of transmission of Lyme disease, virtually all A. agrarius as well as lizards were potentially exposed to spirochetes borne by nymphal I. ricinus. We concluded that larval and nymphal I. ricinus differentially parasitize different hosts. Because so many of these nymphs feed on them, A. agrarius may more effectively serve as reservoirs for the agent of Lyme disease than do other putative reservoir hosts. The presence of lizards may inhibit transmission.

[Alzheimer dementia. A clinical long-term study with quantitative neuropathology].

Alzheimer's disease causes about 80% of dementias in old age. The pathological hallmarks of Alzheimer's disease are senile plaques (SP) and neurofibrillary tangles (NFT), which to a lesser degree can also be found in the brains of mentally intact elderly. The question whether SP or NFT or any other process are primarily correlated to severity of dementia can only be answered in prospective longitudinal clinical and neuropsychological studies with quantitative neuropathological investigation. We report the correlations between mini-mental-state scores and lesion counts in 3 isocortical and 3 hippocampal areas in a consecutive series of 19 Alzheimer's patients studied prospectively in the Vienna Longitudinal Study on Dementia. Lesion counts increased at very late stages of dementia and were rather low in mild to moderate severity of dementia. Mildly demented patients with very slow progression of dementia also had rather high lesion counts. Neurofibrillary changes in NFT and neuritic plaques were correlated with severity of dementia, but diffuse plaques, i.e. SP without neuritic degeneration, were not at all correlated with severity of dementia. We speculate that NFT and SP do not represent the primary process which leads to dementia.

[Clinical findings in patients with lung diseases caused by non-tuberculous mycobacteria in former East Germany and Hungary].

The importance of lung diseases caused by nontuberculous mycobacteria is increased. Clinical findings in 54 patients of the former GDR and 110 patients from Hungary were investigated. Most of the patients were men in average age of 55 years. More than 80% of the patients had an exposition and disposition, for instance, pre-existing pulmonary damage, such as healed tuberculosis, chronic bronchitis or emphysema. X-ray findings are presented and discussed. Signs and symptoms of disease are very similar to the pulmonary tuberculosis.

[Clinical and histomorphologic findings in Wegener's granulomatosis].

The Wegener's granulomatosis is a rare primary systemic vasculitis. It is diagnosed more frequently in the last years. Because of various manifestations (rhinitis, sinusitis, otitis, arthralgie, "red eye", neuritis, carditis) and different symptomatics the disease is misjudged over month and years. Clinical findings, problems of bronchopulmonary and histomorphological diagnostics and differential diagnosis are discussed. The prognosis is favourable in cases of early and timely diagnosis and therapy.

[Differential diagnosis of dementia diseases. A prospective clinical study with neuropathologic diagnostic verification].

We present clinico-pathological correlations for a consecutive series of 44 demented patients in the Vienna longitudinal study on dementia. Prospective clinical diagnosis used the DSM-III-R and the NINCDS-ADRDA criteria. Not only the clinical, but also the neuropathological diagnosis of DAT is based on exclusion criteria, and depends on the interpretation of minimal vascular lesions. Although we did not exclude atypical cases from the study, 80% of diagnoses could be validated at autopsy. Nevertheless, our set of clinical criteria needs further validation in patients in the earliest stages of dementia.
