Plasmapheresis in the treatment of thrombotic thrombocytopenic purpura: a report of five cases.

Thrombotic thrombocytopenic purpura, an acute clinical disorder of uncertain aetiology, has previously been associated with almost 100% mortality. Dramatic response to exchange transfusion has been reported, and there is some evidence that a circulating immune complex may be involved in the pathogenesis. Therefore, it was decided to employ large-volume plasma exchange by means of a continuous-flow cell separator in the management of five patients with this disease. Three patients showed a dramatic response, which was manifested by rapid correction of the neurological deficit and of the abnormalities in laboratory findings. We conclude that the thrombotic thrombocytopenic syndrome should be regarded as an indication for emergency plasma exchange.

Profound first-degree atrioventricular block. A 30-year study.

We report the findings in a 60-year-old patient with a 30-year history of profound first-degree heart block with no associated cardiac abnormalities. The sinus node and the His-Purkinje system were normal. The time for atrioventricular nodal conduction was prolonged, with normal refractoriness both before and after administration of atropine, suggesting that the atrioventricular node may be lengthened, although electrically normal.

Facilitating care after perinatal loss. A comprehensive checklist.

Each year, one out of five women suffers the loss of a pregnancy. Such losses are more than statistics: to many mothers, they represent the death of a longed-for child. While increasing attention has been directed toward meeting the needs of these women, the services provided are often fragmented. The use of a comprehensive checklist for providing care facilitates continuity and consistency of care and offers structure and direction for the caregiver. In this manner, vital aspects of care are not omitted as the bereaved woman moves through the health-care delivery system.

Concurrent systemic lupus erythematosus and salmonellosis.

Salmonellosis was diagnosed in 4 patients with systemic lupus erythematosus (SLE). Three patients were taking prednisolone, and 3 had evidence of nephritis. All patients were febrile with clinical evidence of lupus activity at the time of diagnosis. Two patients had evidence of hyposplenism during the infection. Because salmonellosis manifests clinical symptoms like those of active SLE, the diagnosis of the salmonella infection was delayed. In 3 patients, the salmonella infection localized to a site of clinical SLE involvement and in all 4 patients, it occurred simultaneously with other bacterial infections. Multiple factors, including impaired mononuclear phagocytic system function, may predispose SLE patients to bacterial infections, especially intracellular parasites such as salmonella.
