[Temporary skin replacement with lyophilized swine skin and foam substances--comparative experimental studies].

Using the skin replacement materials Corethium 2, Coldex, Epigard and SYSpur-derm, both fresh wound and wounds up to 6 days old were covered in rats. The histological work-up 1-7 days later showed material-dependent differences in the wound reaction. Under Corethium 2, a heavy layer of wound exudate formed on the first day both on fresh and granulating wound surfaces. Beneath this layer granulation tissue formed that was of regular thickness and well vascularized, filling the defect. Reepithelialization of the wound surface was not hindered by the material. In contrast, a strong connection was established between the wound base and the covering material when using the three foam substances Coldex, Epigard and SYSpur-derm, through the formation of granulation tissue. This process was dependent on the age of the wound. Thickening of the granulation tissue under the covering material was suppressed by the foam substances. Reepithelialization of the surface of the wound was disturbed by the growth of granulation tissue. The netlike structure of Epigard and the properties of the cover layer (fluid-resistant and protective against bacteria) make it possible for the granulation tissue to infiltrate evenly in the whole area of the wound covering and permit growth within the material right up to the surface. In addition to granulation tissue, epithelium also grows into the foam and spreads evenly. Epigard is suitable for long-term covering of large wounds. SYSpur-derm has a different fine structure than Epigard, which hinders the growth of granulation tissue, so that there are only cone-shaped connections between the wound base and the covering material. Rapid desiccation of the wound exudate cannot be prevented. Leukocytes and granulation tissue cannot spread in the foam. With SYSpur-derm an intermediate layer of wound exudate was observed more frequently than with Epigard. SYSpur-derm should be changed regularly in order to remove the detritus and exsudate from the wound. In comparison with polyurethane foam material the polyvinyl-alcohol foam (Coldex) was more complicated to handle, did not adapt as well to the wound, and dried up within a few hours. Between the base of the wound and Coldex, an intermediate layer often formed and, because of the rapid drying-out process, there was appreciably less infiltration with leukocytes and granulation tissue than with SYSpur-derm. With Coldex, as a rule the granulation tissue was more generously vascularized than in the other three materials.

Implant injector for retropectoral augmentation.

The implant injector allows an augmentation of the breast through very small incisions with silicone gel implants without endangering them mechanically. This way the risk of damaging nerves in the axillary region is minimal, operation time is shortened, and the visible scar can be reduced considerably. Contamination of the prosthesis by the skin is avoided. Lubrication of the implant injector by peanut oil (arachis oil) is necessary. Tissue reaction induced by the oil had never been seen. None of the implants broke while using the implant injector.

[Radiogenic damage of the intestine--diagnosis and therapy].

62 patients underwent surgery for intestinal radiation injuries during the past 15 years. 34 patients had previous abdominal surgery. Preoperative investigations must clarify if there is only the radiation lesion or a recurrent process of the originally radiated tumor. Rectovaginal fistulas occurred 28 times, 19 of which could only be treated by continent colostomy. 5 patients had been curatively resected with low anastomosis. A progressive stenosis leading to an ileus symptomatology appeared in 24 patients. The injured intestinal part was resected. As intestinal radiation injuries are combined with an ischemic enteropathy, anastomosis will be extremely problematic. Thus, second-look operations are recommendable for the third and seventh postoperative day. So lethality could be decreased.

[Pathophysiology and morbidity of paralytic ileus (including peritonitis)].

Main complication of different diseases is the paralytic ileus. The basis of all pathophysiological mechanisms is a decrease of the immune deficiency in the intestinal wall. At first the intestinal water-, electrolyte- and protein metabolism is disturbed. Secondly we can measure an endotoxinemia and a bacteriemia with all complications. In the end a complete septic shock and an insufficiency of the organ "gut" can be observed. The patients die - in absence of treatment - because of intraabdominal sepsis and peritonitis.

Malignant tumors of the anus. A study of 106 cases.

This study of 106 cases of malignant tumors of the anus has shown that the diagnosis was often made late. In 15 percent of the patients the tumor was at the anal margin; in 61 percent, in the anal canal, and in 24 percent, the tumor was so extensive that its origin could not be determined clinically. In nearly half of the cases, the cancers were advanced and showed perirectal metastases. Surgical treatment was the rule, with local excision for carcinomas of the anal margin and abdominoperineal resection for tumors of the canal. The general prognosis over a period of several decades remains poor. We found a 20 percent 5 year survival rate for epidermoid carcinoma of the anal canal, excluding deaths from causes other than cancer during the follow-up period.

Adenocarcinoma↗

[Late results following compression treatment of deep 2d and 3d degree burns].

Pressure therapy following burns is necessary for at least 9 months, normally 12 to 15 months. Results are dependent on the age of the patient, anatomical localization and type of reepithelization. Side effects such as pruritus and swelling of maturing scars are minimized. The initially vulnerable epithelium is protected.

[Mammoplasty planning using the Strömbeck technic].

Sagging of the breast may occur within the first year after mammaplasty. This may be prevented by shortening the distance between the areola and submammary fold to less than 25% of the distance between the sternal notch and the new site of the nipple. If this manoeuvre is combined with other modifications in relation to the lateral portion of the breast and the pedicles of the nipple, the results of Strömbeck's mammaplasty can be improved.

Beyond the statistics of adolescent smoking.

Statistical studies can identify the demographic characteristics of the adolescent smoking population but cannot reveal how clusters of demographic categories combine in the culture of the community to form salient social categories, or how social processes link these categories to smoking and smoking-related behavior. Because smoking and smoking-related behavior function as a key social symbol, anti-smoking campaigns that are based on an inaccurate understanding of the social context in which smoking occurs can reinforce this behavior. Participant observation in a suburban high school suggests that adolescents begin smoking as part of a complex symbolic process growing out of the process of social differentiation between future members of the working class on the one hand and the middle class on the other. It points out inadequacies in two existing anti-smoking programs in the schools that result from ignoring the social dynamics of smoking.
