Fracture of polycarbonate edgewise brackets: a clinical and SEM study.

Fracture experience and fracture form of polycarbonate edgewise brackets, were assessed in a clinical study using four force controlled mechanical systems. The incidence and types of bracket fracture were investigated macroscopically and by Scanning Electron Microscopy (SEM). It was concluded that the engagement of both round and square archwires and stainless steel ligaturing were aetiological factors in bracket fracture.

Occurrence of a fetal fibroblast phenotype in familial breast cancer.

We have previously shown that fetal and adult human skin fibroblasts display distinctive migratory phenotypes when cultured on 3-dimensional collagen gels in vitro. In the present study, we have used this information to assess the migratory behavior of fibroblasts obtained from patients with either benign or malignant breast disease, and correlated this with the presence of a family history of breast cancer. We have observed that fibroblasts from 17/34 patients with no previous family history of breast cancer displayed fetal-type behavior in our assay system; in contrast, fibroblasts from 15/16 patients with a positive family history of breast cancer behaved abnormally. This apparently increased probability of expressing a fetal-type migratory phenotype in the patients with a family history is statistically significant (p less than 0.008). Skin fibroblasts obtained from 2 healthy and unaffected first-degree relatives (one male and one female) of patients with a family history of breast cancer also exhibited a fetal-type migratory phenotype.

Skin fibroblasts obtained from cancer patients display foetal-like migratory behaviour on collagen gels.

When plated on the surface of collagen gel substrata, all types of fibroblasts rapidly begin to migrate down into the three-dimensional collagen matrix. We have previously demonstrated that normal (adult and foreskin), foetal and transformed fibroblasts may be distinguished from each other by virtue of their differential migratory response to changes in cell density. The effects of cell density on fibroblast migration into the gel may be expressed by a single numerical value, the 'cell density migration index' (CDMI). We now present evidence that ostensibly normal skin fibroblasts obtained from the majority of patients we examined with carcinoma of the breast, malignant melanoma, familial polyposis coli, retinoblastoma and Wilms' tumours display aberrant CDMI values falling within the foetal range. Skin fibroblasts obtained from the majority of patients examined with genetic or chronic diseases (e.g. rheumatoid arthritis, Duchenne muscular dystrophy) displayed CDMI values falling within the normal range.

Fibroblasts from patients with breast cancer show abnormal migratory behaviour in vitro.

In culture on collagen gels normal and transformed fibroblasts can be distinguished by their differential migratory response to changes in cell density. The effects of cell density on fibroblast migration may be expressed by a single numerical value, the cell density migration index (CDMI). Tumour-derived fibroblasts and skin fibroblasts from the majority of 24 breast-cancer patients examined had CDMI values characteristic of transformed cells. There was a significant correlation between the expression of abnormal CDMI values by tumour-derived fibroblasts and the presence of lymph-node metastases. Fibroblasts from 10 patients with benign breast disease had normal CDMI values.

Diagnosis and management of psoas abscess in Crohn's disease.

Four patients with Crohn's disease complicated by psoas abscess are described. These patients did not have general or abdominal signs of sepsis but they all showed wasting of the right quadriceps femoris and hip flexion. It is important to examine the hip and thigh in patients with known Crohn's disease who are on medical treatment. Surgical treatment of a Crohn's psoas abscess should include resection of recurrent disease, fashioning of the anastomosis remote from the abscess cavity, and long-term (21 days) drainage of the abscess cavity to avoid the possibilities of faecal fistula or recurrent abscess.

Recognition of metastatic bone disease in cancer of the breast by computed tomography.

Computed tomography has been used to evaluate the significance of positive bone scans in 44 patients with carcinoma of the breast. Of patients with apparently early cancer of the breast who had a positive bone scan 76% were found to have benign joint disease, while 33% of patients who developed a positive bone scan after mastectomy also had benign disease on CT scan. Seventy five per cent of patients with apparently early disease but a positive bone scan who were shown to have metastases died within 30 months. All the patients have been followed for 5 years and only one false positive CT scan was reported.

Serum glycoprotein hormone alpha subunit, hormone receptors and disease stage in patients with breast cancer.

The concentration of the common alpha subunit of the glycoprotein hormones was high in the serum of 21/56 (38%) of premenopausal patients and 22/106 (21%) of postmenopausal patients with primary breast cancer, at the time of presentation. 7/59 (12%) of patients with benign disease also had high alpha subunit levels. Tumour cytosol oestrogen and progesterone receptor status was determined in 80% of the patients with cancer, and there was a trend towards higher alpha levels in patients without receptors, but this was not statistically significant. In the premenopausal patients with cancer there was a significant correlation between alpha subunit level and disease stage, R = 0.47, P = 0.0001, but not in the postmenopausal patients. In view of the correlation with disease stage, high levels of alpha subunit in premenopausal patients with breast cancer at presentation with the primary tumour may indicate poor prognosis.
