The value of the thromboelastogram as means of monitoring the sub-cutaneous preventive action of calcium heparinate.

The index of the thrombodynamic potential (I.T.P.) as one of the biological means of controlling the prophylactic effect of sub-cutaneous injections of Calcium Heparinate has been studied in 34 spinal cord injuries during the acute phase. Individual tolerance variations, some of them important, have been noted as well as the necessity of adjusting the doses, according to the time after injury. A biological hypercoagulability has been noted within the first and third month after injury. The interest and the cost of such a method are discussed and further comparative biological investigations, using other methods, have started.

Elongation of the active anterior wall of the uro-genital pelvic diaphragm, a late unusual complication of paraplegia.

The situation of the usual bladder, prostate, membranous urethra channel, can vary, according to the morphology of the perineum which can be overstretched. A case of a young man with a T10 complete upper motor neurone lesion is presented. The normal anterior angulation at the prostate and membranous urethra junction was reduced anteriorly and pushed backwards, thus causing an added indirect factor of dysuria. The mechanism is discussed in comparison with other such late, but often overlooked consequences of alterations of the pelvic floor during micturition.

Survey of the neurological evolution of 300 spinal cord injuries seen within 24 hours after injury.

A grouped Franco-Swiss study on 300 cases of spinal cord injury gives an overall view of the neurological evolution of the spinal cord injuries or cauda equina lesions. The importance of initial and repeated examinations is stressed. Sensory sparing distant to the lesion is of great importance, especially that of pain. A great number of patients showing neurological progress was observed at the end of the first month and at the beginning of the following three months. The influence of steroids is tentatively discussed. No complications due to this therapy have been observed on the condition that their use is very limited in time. Statistically speaking, there has been no difference of evolution in the two groups, the majority having been treated orthopedically.

Reflections on the intensive care of acute cervical spinal cord injuries in a general traumatology centre.

Fifty-one cases of cervical spinal cord injury were reviewed. The importance of the immediate comprehensive care after onset may be achieved successfully even in a General Traumatology Centre. The great majority of these patients were treated by orthopaedic methods as far as their vertebral injury was concerned. Mortality related to the spinal cord injury is 7.8 per cent. The methods used and our development are based on principles established by Sir Ludwig Guttmann; they have been shown to be durable and important.

Traumatic rupture of the aorta and paraplegia.

During the past 10 years, 15 traumatic ruptures of the aorta isthmus have been diagnosed. The most likely mechanism is that of a brutal deceleration. Three patients developed a complete mid-thoracic flaccid paraplegia. One died in a few hours of a complete rupture, the second after a few days from renal insufficiency, the third 12 days after, having been operated upon on the same day of the accident. All had associated lesions. Diagnosis can be very difficult in these patients, especially in the first few hours. Paraplegia is here considered as an aggravation ischaemic symptom with anuria and loss of blood circulation in the lower limbs. In delayed cases the vascular mechanism of the infarction of the spinal cord is discussed, either by compression of the intercostal arteries or by direct compression of the aorta itself. In our cases, the most likely mechanism is that of an obstruction of the lumen of the aorta by a torn inner coat. These patients must be transferred as quickly as possible to a cardiovascular surgical unit and operated upon immediately. Three cases of preoperative, rapidly progressive paraplegia with neurological recovery after the operation, have been reported in the literature.

Three unusual cases of spinal cord injury in childhood.

Three cases of unusual traumatic spinal cord injuries are reported. The cases were referred to the neurosurgical unit of Nancy, one tetraplegia and one paraplegia caused by transfixing wounds which were overlooked after the injury. The transfixing wound caused an associated meningitis with tetraplegia, and in the second case the initial small plural effusion associated with paraplegia even, in a very short while, evoked a possible diagnosis of leukaemia amongst others, until the knowledge of trauma had been ascertained, the child being then immediately transferred to the neurosurgical unit. Spanking, in the last case, was the cause of an important luxation of T12-L1, at first with a complete paraplegia, and was associated with the fact that the child was only seen a few days after by a doctor and immediately referred. The child made a full recovery from his paraplegia.

Our surgical approach towards the treatment of urethrocele and urethral fistula.

A new technique, described in the text, has been elaborated inspired by that of Monseur (1968) for urethral techniques. It has been performed with success in three paraplegics and in one incomplete tetraparesis. The plastic reconstruction of the diseased part of the urethra after excision of the stenosis and a fistula or diverticula by rotation and fixation to the subcavernal groove creates, in fact, an enlarged neo-urethra rendering the recurrence of the primary lesion practically impossible. This technique, first applied to spinal cord injuries, has been extended with permanent success to other lesions, such as tuberculosis stenosis and malformations.

Treatment results of spinal cord injuries in the Swiss Parplegic Centre of Basle.

The results concerning the last 117 acute traumatic spinal cord lesions admitted in the Swiss Paraplegic Centre of Basle have been analysed as regards the rate of neurological recovery. The treatment not only concerns the vertebral damage but also the medical aspects of the spinal cord lesion itself. The routine use of combined Dexamethasone, Rheomacrodex and Hydergine in the very early treatment might have accounted for encouraging results which need to be confirmed in the future.

Impairment of erection after external sphincter resection.

Two cases of temporary loss of reflex erectile activity after transurethral bladder-neck and external sphincter resection are reported in two tetraplegics. Only recently such occurrences have been reported in the literature associated with external sphincterotomy. The reasons are discussed and most likely direct damage to erectile tissues is the cause of such a complication.
