Itraconazole penetrates the nail via the nail matrix and the nail bed--an investigation in onychomycosis.

Nail-matrix kinetics were studied in 21 patients (19 with onychomycosis, two with tinea corporis) as soon as taking itraconazole (Sporanox) 100 mg daily for up to 7 months. Itraconazole was detected in the distal nail as soon as 1 month after the start of therapy (42 ng/g in fingernails and 16 ng/g in toenails). During the course of treatment, this concentration rose and reached a mean of 160 ng/g in fingernail clippings and 197 ng/g in toenail clippings. Moreover, in fingernails of 12 out of 21 patients and in toenails of six out of 20 patients, itraconazole was detected in the distal nail clippings before full outgrowth of the fastest-growing nail. In most patients, itraconazole was detected in the distal nail clippings earlier than would be expected if the drug were incorporated only via the nail matrix, indicating that in addition to the nail matrix, a second route of penetration into the nail exists, i.e. the nail bed.

A double-blind vehicle-controlled study of R 68 151 in psoriasis: a topical 5-lipoxygenase inhibitor.

Recently the pharmacologic role of leukotrienes (LTs), especially that of LTB4, has been intensively investigated in psoriasis. In vitro, R 68 151 is a potent 5-lipoxygenase inhibitor and consequently reduces LTB4 formation. Therefore the role of an in vitro 5-lipoxygenase inhibitor and its clinical use in psoriasis were evaluated with topical R 68 151 in a double-blind vehicle-controlled study. Eighty-eight patients with localized psoriatic lesions were treated twice daily with R 68 151 2% ointment (n = 44) or vehicle ointment (n = 44) during 4 weeks. Most patients (n = 73) had psoriasis vulgaris (n = 37, R 68 151; n = 36, vehicle). In 27% of the R 68 151-treated patients with psoriasis vulgaris, the lesions disappeared or showed marked improvement, compared with 8% in the vehicle group (X2, p = 0.06). The scores in global evaluation, however, were significantly different between both treatment groups (p less than 0.05, Mann-Whitney U test). The improvement of the mean symptom score with R 68 151 was 46% for scaling and 34% for erythema at the end of the study compared with an improvement of 6% and a deterioration of 3%, respectively, in the control group (p less than 0.05, p less than 0.01; Mann-Whitney U test). The global evaluation in the total group of patients with psoriasis (all different subtypes) was consistent with the response rate in the group of patients with psoriasis vulgaris: 30% in the test group versus 11% in the control group (X2, p less than 0.05).(ABSTRACT TRUNCATED AT 250 WORDS)

Skin lesions as a sign of subacute pentachlorophenol intoxication.

Pentachlorophenol (PCP) and its sodium salt are frequently used in wood preservatives. Little is known about the effects on man when being chronically exposed. Only vague skin symptoms, such as rashes, acne and cutaneous infections were described. We present two cases of pemphigus vulgaris with a known non-occupational chronic PCP exposure. The clinical course and the titer of pemphigus antibodies roughly correlate with the PCP levels in serum. In one case of chronic urticaria the exacerbations also run parallel to the PCP serum levels and increased anti-skin antibodies, without any manifestation of pemphigus vulgaris. The role of PCP as one of the causes provoking pemphigus vulgaris and chronic urticaria with raised anti-skin antibodies is discussed.

The muscle pump function in chronic venous insufficiency: plethysmographic results.

The frequency and the severity of skin lesions were studied in patients with chronic venous insufficiency compared to the function of the muscle pump measured with plethysmography. The study showed that the frequency of skin symptoms parallels the muscle pump dysfunction. Measuring the ambulatory volume change on the calf level and the refilling time required for the normalisation of the calf volume after dorsiflexions of the foot gives suitable parameters for the evaluation of the muscle pump. The plethysmography is a valuable method to quantify the grade of venous insufficiency.

Influence of econazole and 5-fluorocytosine on the ultrastructure of Aspergillus fumigatus and the cytochemical localization of calcium ions as measured by laser microprobe mass analysis.

The ultrastructural changes and the localization of Ca++ ions have been investigated in Aspergillus fumigatus after exposure in vitro to econazole (50 micrograms ml-1) and to 5-fluorocytosine (100 micrograms ml-1) for 24 h. The changes obtained with econazole concerned the cell periphery, necrotization of mitochondria and hyperactivity of the central vacuole. After cytochemical Ca++ localization a marked increase in precipitate was observed on mitochondria, vacuoles and collapsed membranes, compared to the control. Laser microprobe mass analysis confirmed that the measured amount of Ca++ ions corresponded to the degree of precipitate formation in the different cellular compartments. After exposure to 5-fluorocytosine, abnormal behaviour of the nuclei and internal lipidification of the mitochondria and of the cytoplasm were seen. No discernible Ca++ activity was present on the cellular structures by cytochemical localization. Assay by laser microprobe mass analysis, however, showed a slight increase in Ca++ which points to a structural bonding of the Ca++.

Aspergillus fumigatus

Intractable diarrhea in histiocytosis-X.

In an infant with fatal disseminated histiocytosis-X, intractable diarrhea was the initial symptom and remained prominent during the further course of the disease. Intestinal biopsy revealed extensive histiocytic infiltration of the lamina propria. Although involvement of the gastrointestinal tract, mostly with diarrhea, is probably not unusual in generalized histiocytosis-X, this fact is rarely mentioned in the literature. Intractable diarrhea as a major symptom of this disease has not yet been described.

[Electron microscopic aspects of opportunistic fungi after pretreatment].

The ultrastructive of Aspergillus fumigatus, Aspergillus flavus and Aspergillus niger was studied when treated in vitro with 0.001, 0.01 and 0.1% econazole nitrate and econazole base. The very first morphological alterations appear faster than in normal-growing fungus and consist in a convoluted cytoplasmic membrane, large mesosomes and lomasomes and an extensive vacuolization. Acid phosphatase is found in the mesosomes and lomasomes and also in the center of the vacuoles. This is an early sign of fungus degradation.
