DNA flow cytometry of non-Hodgkin's lymphomas: correlation with cytologic grade and clinical relapse.

In this prospective study, we correlated cytological grading and clinical follow-up of non-Hodgkin's lymphomas (NHL) with DNA flow cytometry (FCM) data from fine-needle aspiration biopsy (FNAB) material. FNAB was performed from 34 successive cases of NHL, and the aspirated material was analyzed by DNA FCM. Cytological subtyping and grading were done by modified Working Formulation. Cases were followed up for 2.5 yr, and cytological grading, clinical follow-up, and DNA FCM data were correlated. There were 8 cases of low, 14 intermediate, and 12 high-grade NHL. None of the cases of low-grade NHL showed DNA aneuploidy. Of the 8 DNA aneuploid cases, 5 were of intermediate and 3 were of high-grade NHL. Mean growth fraction (GF) of low-grade, intermediate-grade, and high-grade NHL was 6.1, 9.4, and 19.4, respectively. DNA aneuploidy was statistically significant (P < 0.05) between low- vs. intermediate- and high-grade NHL. Growth fraction was also statistically significant between low- vs. high-grade NHL. Six cases showed clinical recurrence, and one case died within 6 mo of diagnosis, due to widespread involvement of NHL. DNA aneuploidy and mean GF of recurrent and nonrecurrent cases were statistically significant (P < 0.05) irrespective of grading of NHL. DNA aneuploidy and GF are not well-correlated with morphological grading (by the Working Formulation). However, these two criteria are important for assessment of early clinical relapse of NHL.

Quantitation of spermatogenesis by DNA flow cytometry from fine-needle aspiration cytology material.

DNA flow cytometry (FCM) was performed from fine-needle aspiration cytology (FNAC) of testis in 15 cases of male infertility to quantitate spermatogenesis. The results were correlated with FNAC findings. DNA FCM showed a ploidy relationship of haploid (1N) > diploid (2N) > tetraploid (4N) in cases of normal spermatogenesis. A ploidy relation of 2N > 1N > 4N was observed in cases of hypospermatogenesis or maturation arrest. In Sertoli cell-only cases, there were only 2N populations of cells. With the help of DNA FCM, a rapid and objective assessment of spermatogenesis is possible from FNAC of the testis.

Deep-seated thoracic and abdominal masses: usefulness of ultrasound and computed tomography guidance in fine needle aspiration cytology diagnosis.

Fine needle aspiration cytology (FNAC) was performed under ultrasound and CT guidance in 120 cases. These included abdominal masses (85 cases) and thoracic masses (35 cases) biopsied over a two and a half year period (March 1996 to September 1998). The aim of this study was to assess the contribution of clinico-imaging evaluation and image-guided FNAC to the management of patients with deep-seated mass lesions. Aspirations in the abdomen were performed from various anatomic sites such as liver (56 cases), lymph nodes (18 cases), gastrointestinal tract (three cases), pancreas (six cases), and kidney (two cases). In the thorax, biopsy was performed in the lung (19 cases) and mediastinum (13 cases). In 112 cases (93.4%) FNAC was diagnostic. Of the lesions that were successfully aspirated, 85% were < or = 5 cm in size. No major complication was encountered. All the successful aspirates could be defined as malignant or non-malignant, but tissue differentiation was possible in 63.7% of malignant lesions and 53.8% of benign lesions. Combined clinical and imaging evaluation for malignancy showed 80% sensitivity and 59% specificity. Although clinicoradiological parameters themselves have certain limitations in diagnosing benign versus malignant lesions, in conjunction with guided FNA they are very accurate and safe in diagnosing deep-seated mass lesions in the thorax and in the abdomen. However, the role of FNA in tissue differentiation of solid lesions such as lymphoma requires further study.

Nuclear image morphometry and cytologic grade of breast carcinoma.

OBJECTIVE: To correlate visual cytologic grade with automated nuclear morphometry of carcinoma of the breast. STUDY DESIGN: We randomly selected 24 histologically proven infiltrating ductal carcinomas of the breast and 10 benign breast lesions (fibroadenoma). Hematoxylin-eosin-stained fine needle aspiration cytology (FNAC) smears were selected for both cytologic grade and automated image morphometry. The same hematoxylin-eosin-stained FNAC smears were studied for area, convex area, standard deviation of nuclear area, diameter, perimeter and convex perimeters of nucleus. At least 100 cells from each case were measured with an image cytometer. RESULTS: Mean nuclear area, standard deviation of nuclear area, nuclear diameter, convex area, convex perimeter and perimeter were significantly increased from benign versus grade 1 carcinomas and grade 1 versus grade 2 and 3 carcinomas (one way ANOVA test). However, there was no significant difference in grade 2 versus grade 3 carcinomas. CONCLUSION: Automated image cytometry rapidly and successfully measures various nuclear parameters. The measurement of various nuclear parameters would be helpful in future applications of automated diagnosis and grading of breast carcinomas from cytologic material.

DNA flow-cytometry on fine needle aspiration cytology of lymph node: how helpful is it?

The diagnostic value of DNA content analysis by flow cytometry (FCM) has rarely been evaluated in combination with fine needle aspiration cytology (FNAC). In this present study, the value of DNA FCM in distinguishing malignant lesions from benign enlarged lymph nodes on FNAC material has been investigated. DNA FCM was done from FNAC materials of 58 cases of lymph node swellings Becton Dickinson's flow cytometer (USA) along with 'Cell Quest' program was used for the analysis of DNA ploidy and S & G2-M phase cells. There were 16 cases of reactive lymphoid hyperplasia, 8 cases of metastatic carcinoma and 34 cases of non Hodgkins lymphoma (NHL). DNA FCM showed 12 aneuploid case and 46 diploid case. All the cases with DNA aneuploidy were malignant. None of the benign cases showed aneuploidy on DNA aneuploidy were malignant. None of the benign cases showed aneuploidy on DNA histogram. Out of the 46 diploid cases, 11 cases showed high S & G2-M Phase cells (> 10%). Nine out of these 11 cases were malignant on FNAC. The sensitivity and specificity of DNA FCM were 50% and 87.5% respectively. None of the low grade NHL was detected by DNA FCM. In conclusion, DNA aneuploidy and high G2-M phase cells (> 10%) are good indicators of malignancy in lymph node aspirate. However DNA FCM is relatively costly and thereby its role as an adjunctive technique needs careful re-evaluation.

Correlations between apoptotic and proliferative indices in cervical intraepithelial neoplasia and carcinoma.

Aims of the present paper was to study cell death by apoptosis and cell proliferation in normal cervical biopsies, cervical intraepithelial neoplasms (CIN) and squamous cell carcinomas of cervix (CaCx). There were each seven cases of normal cervical biopsy, CIN1 and CIN2 along with 10 cases of CIN3 and 14 cases of CaCx. Percentage of apoptotic cells and bodies (i.e. apoptotic index, AI) and mitoses (i.e. mitotic index, MI) and turnover index (TI - AI + MI) were counted in formalin fixed, paraffin embedded, haematoxylin and eosin stained slides. AgNOR stain was done and mean AgNOR dots per cell was also estimated. AI, MI and TI were correlated with histology grade of CIN and invasive carcinoma cervix. Mean AI, MI, TI and AgNOR count increased from lower to higher grades of CIN. AI, MI & TI raised significantly from CIN3 to carcinomas; AI, TI & AgNOR count raised significantly from CIN1 to combined CIM2 & 3; TI & AgNOR count were high in CIN1 to CIN2; AI & TI were significantly raised in normal to CIN1. In conclusion, TI is probably more important for cell kinetic analysis of CIN and carcinoma of cervix because it reflects the frequency of two important events i.e. mitosis and cell death. Sudden increase of AI, MI, TI count from CIN3 to CaCx may indicate the possibility of genetic alteration of cells of CIN3 which induces a frank malignant transformation from CIN3 to CaCx.

Isolation and characterization of human monoclonal antibodies to digoxin.

Fab preparations of sheep polyclonal anti-digoxin Abs have proven useful for reversal of the toxic effects of digoxin overdoses in patients. Unfortunately, the use of foreign species proteins in humans is limited because of the potential for immunological responses that include hypersensitivity reactions and acute anaphylaxis. Immunization of recently developed transgenic mice, whose endogenous micro heavy and kappa light chain Ig genes are inactivated and which carry human Ig gene segments, with a digoxin-protein conjugate has enabled us to generate and isolate eight hybridoma cell lines secreting human sequence anti-digoxin mAbs. Six of the mAbs have been partially characterized and shown to have high specificity and low nanomolar affinities for digoxin. In addition, detailed competition binding studies performed with three of these mAbs have shown them to have distinct differences in their digoxin binding, and that all three structural moieties of the drug, the primary digitoxose sugar, steroid, and five-member unsaturated lactone ring, contribute to Ab recognition.

A population-based survey of the management of women with cancer of the cervix.

The aim of this study was to investigate the influence of diagnostic throughput on survival outcome for women with cancer of the cervix. We conducted a case note review of 359 women in Lancashire and Greater Manchester diagnosed with cancer of the cervix during 1990, identified from records held by the North Western Regional Cancer Registry. Univariate and multivariate survival analyses were undertaken to investigate the influence on survival of woman, disease and treatment related factors. Following adjustment for woman- and disease-related factors there was no evidence of a statistically significant association between diagnostic throughput and survival. The findings of this study do not support the need for any change in the referral patterns to gynaecologists of women with symptoms suggestive of cancer of the cervix.

Cytologic grading and DNA image cytometry of breast carcinoma on fine needle aspiration cytology smears.

OBJECTIVE: To correlate the cytologic grade of breast carcinoma with DNA image cytometry (ICM) and nuclear area on fine needle aspiration cytology (FNAC) smears. STUDY DESIGN: In this prospective study, FNAC material from 28 breast carcinomas were studied for cytologic grade and DNA ICM. Breast carcinomas were classified as grade 1-3 (low to high). DNA histograms were classified by the modified Auer method. Degree of hyperploidy (DH), ploidy balance (PB) and nuclear area (NA) were measured on Feulgen-stained smears by a CAS 200 image cytometer. Cytologic grade was correlated with DNA ICM findings and NA. RESULTS: There were 3 cytologic grade 1, 13 grade 2 and 12 grade 3 breast carcinomas. Seven of eight cases of hypertetraploid aneuploidy were grade 3 tumors. All cytologic grade 1 tumors were diploid. There were significant differences in DH, PB and NA in different grades of breast carcinoma (one-way ANOVA). CONCLUSION: DNA image cytometry in combination with cytologic grading might offer additional information for the characterization of breast carcinomas diagnosed by FNAC. These observations are of particular interest with the introduction of preoperative chemotherapy.

Molecular mechanics aided design of antineoplastic agents from ruthenium coordinate complexes.

Through energy minimization using molecular mechanics force field four ruthenium cordinate complexes have been synthesized. Compound I to IV showed antineoplastic activity with varying degree on EAC bearing mice. Mode of action may be through inhibition of antioxidant property of tumor cell as evident from lipid peroxidase activity. Among the complexes Bis pyridine tetrachloro ruthenium exhibits highest order of activity with respect to increase mean survival time, inhibition of tumour volume, total blood count, hemoglobin and lipid peroxidase activity.
